본 연구는 AS 환자의 인구·사회학적 특성 및 질병 특성, Bath 강직성 척추염 질병활성도 지수(KBASDAI), Bath 강직성 척추염 기능 지수(KBASFI)가 이들의 삶의 질에 미치는 영향을 파악하기 위한 서술적 조사연구이다. 강직성 척추염을 진단을 받은 만 19세 이상의 환자 146명을 대상으로 온라인 설문조사를 통해 자료를 수집하였다. 삶의 질은 Ankylosing spondylitis quality of life(ASQoL)로 측정하였다. 변수 특성에 따른 삶의 질 차이를 단변량 분석하고, 삶의 질 영향요인을 살펴보기 위해 다중회귀분석을 시행하였다. 회귀분석결과 설명력은 57.8%로 KBASFI 점수(𝛽=0.622, p<0.001)와 KBASDAI 점수(𝛽=0.180, p=0.032)가 높아질수록, 가구 월 평균 소득 (𝛽=0.186, p=0.001)이 낮은 그룹이 삶의 질이 나쁜 것으로 나타났다. AS 환자들의 삶의 질 개선을 위해 질병활성도를 낮추고 기능을 개선시킬 수 있는 중재 연구가 필요하다.

The purpose of this study was to identify factors affecting the Quality of Life of Ankylosing Spondylitis Patients Based on Korean Bath Ankylosing Spondylitis Disease Activity Index (KBASDAI) and Korean Bath Ankylosing Spondylitis Functional Index (KBASFI). The subjects of this study were 19 years of age or older who were diagnosed with ankylosing spondylitis(AS). Data was collected through an online survey. A univariate analysis of differences in quality of life according to variable characteristics was performed, and multiple regression analysis was performed to examine factors affecting quality of life. As a result of regression analysis, the higher the KBASFI (𝛽=0.622, p<0.001) and the KBASDAI (𝛽=0.180, p=0.032) scores, and the lower the average monthly household income(𝛽= 0.186, p=0.001), the worse the quality of life. In order to improve the quality of life of AS patients, intervention studies that can lower disease activity and improve function are needed.

대상 데이터

본 연구는 단국대학교 생명윤리위원회(Institutional Review Board, IRB)의 심의를 거쳐 승인(승인번호: 2021-01-020)을 받은 이후 진행하였다. 본 연구는 온라인 네이버 카페인 강직성척추염연합회에서 강직성척추염을 진단을 받은 만 19세 이상의 환자를 대상으로 온라인 설문조사를 실시하였다. 2021년 2월 모집공고 글을 보고 자발적으로 연구에 참여를 희망하는 환자들이 링크를 클릭하여 설문을 진행하였다.
본 연구에서 필요한 대상자 수는 G-power 3.1.9.2 표본 크기 계산법에 따라 회귀분석, 유의수준 .05, 검정력 .80, 중간효과 크기 .15, 예측변수 12개, 탈락률 15%를 고려하여 146명의 대상자를 모집하였다. 탈락자가 없어 최종 146명을 대상으로 자료를 분석하였다.

데이터처리

⦁ 대상자의 인구·사회학적 특성 및 질병 특성, KBASDAI, KBASFI, ASQoL은 기술통계분석(빈도, 백분율, 평균과 표준편차)을 하였다.
⦁ 대상자의 특성에 따른 ASQoL은 t-test, Mann-whitney U test와 Kruskal–Wallis test, ANOVA로 분석하였다.

성능/효과

본 연구에서 대상자의 특성에 따른 ASQoL 차이를 살펴본 결과 가구 월 평균 소득, KBASDAI, KBASFI에따라 각각 차이가 있었다. 대상자의 특성을 통제한 상태에서 가구 월 평균 소득, KBASDAI와 KBASFI가 ASQoL에 영향을 미치는 요인임을 확인하였다.
