Purpose : The goal of this study is to examine the effect of robot assisted gait training (RAGT) on the kinematic factors (temporospatial gait parameters, gait cycle ratio, and gait line length) of gait in stroke patients. Methods : The subjects of this study were 24 stroke patients selected by inclusion criteria. Participants were randomly allocated to two groups: robot assisted gait training (n=11) and general neurological physical therapy group (n=11). In the robot-assisted gait training group, robot-assisted gait training was mediated for 30 minutes a day in addition to general neurological physical therapy. The general neurological physical therapy group was mediated by general neurological physical therapy for 30 minutes a day in addition to general neurological physical therapy. The number of interventions was 5 times a week for 5 weeks. In order to compare the kinematic factors of walking between the two groups, gait analysis was performed before and after 5 weeks of training using the Zebris gait analysis system. Results : As a result of the gait analysis of the two groups, there were significant differences in temporospatial gait variables (step length, stride length, step width, step time, stride time), gait cycle ratio (swing phase, stance phase) and gait line length. However, there was no significant difference in the cadence (temporospatial gait parameters) in the robot assisted gait training group compared to general neurological physical therapy group. Conclusion : It is considered to be a useful treatment for stroke patients to promote the recovery of gait function in stroke patients. Based on the results of this study, continuous robot assisted gait training treatment is considered to have a positive effect on gait ability, the goal of stroke rehabilitation. In the future, additional studies should be conducted on many subjects of stroke patients, the kinematic factors of the legs according to the severity of stroke and treatment period, and the effect of gait training.

기존의 로봇 보행 훈련은 엉덩관절과 무릎관절을 모터로 구동하여 정상적인 보행패턴을 반복적으로 훈련시키는 로봇이다. 본 실험에 중재된 로봇은 엉덩관절과 무릎관절을 구동할 뿐만 아니라 발목관절의 운동까지 조절할 수 있는 보행 훈련 로봇으로 이에 대한 보행의 시공간적인 변수와 중재효과를 알아보고자 하였다. 따라서, 본 연구는 뇌졸중 환자들의 로봇보행훈련에 대한 물리치료의 효과를 정량적으로 분석하여 향후 보행로봇을 활용한 물리치료의 근거를 제시하기 위해 로봇보조 보행훈련이 편마비 뇌졸중 환자의 보행의 시공간적 변수에 미치는 효과를 분석하고자 한다.
제안 방법
연구 대상자들은 로봇보조 보행훈련군과 일반적인 신경 물리 치료군으로 나누었으며, 로봇보조 보행훈련군은 1일 1 회 로봇 치료(30분)와 1일 1회 일반적인 신경 물리치료를 시행하였고 일반적인 신경물리치료군은 1일 2회 일반적인 신경물리치료(30분)를 환자에게 충분히 설명한 후 4 주 동안 주 5회 치료를 중재하였다. 물리치료 중 지속적인 환자의 관찰과 적절한 수행을 반복하였으며, 보행 로봇 보행훈련의 모든 조절 지표는 대상자의 신체 기능과 보행 기능에 따라 점진적으로 수정하여 지표를 조절하여 중재하였다(Fig 1).

대상 데이터
본 연구는 S시 재활전문병원에 입원하여 물리치료 중인 뇌졸중 환자를 대상으로 하였다. 연구 참여자 중에서 실험군은 1일 1회 로봇보조 보행훈련과 1일 1회 일반적인 신경물리치료를 각각 30분씩 중재하고 대조군은 1일 2회 일반적인 신경물리치료를 30분씩 중재하였다.
첫째, 뇌졸중으로 진단받고 3개월 이상 1년 미만인 자, 둘째, 한국판 간이정신 상태 검사(mini mental status examination-Korean version; K-MMSE)에서 23점 이상인 자, 셋째, 다리의 강직으로 인한 엉덩관절, 무릎관절, 발목관절에 구축이나 가동범위의 제한이 없는 자로 하였다. 위와 같은 설정 기준에 따라 적합한 대상자 22명을 대상으로 난수표를 이용한 무작위 방법으로 분류하여 로봇보조 보행훈련군 11명, 일반적인 신경물리치료훈련군 11명을 배치하였다.
본 연구의 대상자 선정기준은 다음과 같다. 첫째, 뇌졸중으로 진단받고 3개월 이상 1년 미만인 자, 둘째, 한국판 간이정신 상태 검사(mini mental status examination-Korean version; K-MMSE)에서 23점 이상인 자, 셋째, 다리의 강직으로 인한 엉덩관절, 무릎관절, 발목관절에 구축이나 가동범위의 제한이 없는 자로 하였다. 위와 같은 설정 기준에 따라 적합한 대상자 22명을 대상으로 난수표를 이용한 무작위 방법으로 분류하여 로봇보조 보행훈련군 11명, 일반적인 신경물리치료훈련군 11명을 배치하였다.
데이터처리
연구 대상자의 정규성 검증을 위해 Shapiro- wilk test를 실시하였고, 대상자들의 중재 방법에 대한 보행의 운동학적 요인을 비교하기 위하여 공분산 분석(analysis of covariance; ANCOVA)을 실시하였으며, 통계학적 유의수준은 α=.05로 설정하였다.
성능/효과
본 연구는 뇌졸중 환자에게 중재한 로봇보조 보행 훈련이 다리의 운동기능 회복과 촉진으로 인해 손상된 보행의 운동학적 기능이 향상되는 결과를 보였다. 따라서 로봇 보조 보행훈련이 뇌졸중 환자의 보행기능과 운동기능 향상에 적절한 치료적 중재방법임을 제시할 수 있을 것으로 생각된다. 향후 더 많은 환자들을 대상으로 뇌졸중의 중증도와 치료기간에 따른 효과에 대해 추가적인 연구가 이루어져야 할 것으로 생각한다.

본 연구에서 로봇보조 보행훈련군에서 보행의 좌우 기능적 대칭성이 향상되었고 특히 손상측의 흔듦기와디딤기에서 유의한 결과를 보였다. 이러한 결과들은 로봇 보조 보행훈련이 운동 학습 이론을 바탕으로 중재되는 대표적인 보행 치료로 하지의 균형 능력과 안정성을 증진하고, 낙상 공포로부터 자유로워 불필요한 근 긴장의 억제와 조기에 보행에 대한 경험이 보행 기능을 촉진한 것으로 생각된다.
후속연구
로봇보조 보행훈련으로 뇌졸중 환자의 보행기능 향상은 임상적으로 로봇치료의 중요한 기여가 있었을 것으로 생각되며 로봇보조 보행훈련이 뇌졸중 환자의 보행 기능 회복에 새로운 치료적 방법으로 사용할 수 있을 것이다. 연구대상자의 연령에 따라 보행로봇장비에 적응하는 기간이 다를 수 있으며, 로봇치료의 중재 기간을 연장하고 다양한 평가를 추가적으로 분석하는 등의 제한점을 보완할 수 있는 연구가 필요할 것으로 생각한다.
