Purpose: This study aimed to identify the relationship among musculoskeletal problems, sleep problems, and self-rated mental health of home-care workers. Methods: Data were collected from 447 home-care workers spanning three occupation types: life supporters for the elderly, home-visit caregivers, and life supporters for the disabled. Musculoskeletal problems, sleep problems, and self-rated mental health were assessed using structured questionnaires. Factors affecting self-rated mental health were analyzed using multiple regression. SPSS was used to test the mediating effects of sleep problems on musculoskeletal problems and self-rated mental health. Results: Among the general characteristics, the variables that showed significant differences in musculoskeletal problems were monthly income level, caring-related career duration, weekly working hours, and occupation type; and the variable that showed significant differences in self-rated mental health was occupation type. Among the occupation types, supporters for the disabled had the most musculoskeletal problems and the lowest self-rated mental health. Musculoskeletal problems among home-care workers had a direct negative effect on self-rated mental health and indirect negative effects on sleep problems. Conclusion: Measures are needed to reduce the differences in working conditions and health status among the occupation types of home-care workers. Considering the relevance between the health issues of home-care workers, the development of a carefully designed health promotion strategy is required.

제안 방법

근골격계 자각증상은 제 5차 근로환경조사에 포함되어 있는 주관적 건강문제에 대한 총 11개 문항 중 근골격계와 관련성이 있는 3개 문항으로 간호대학 교수 1인과 현장전문가 1인의 내용타당도 검증을 통하여 선정하였다. 선정된 문항은 ‘지난 1년 동안 요통의 건강문제에 대한 경험이 있습니까?’, ‘지난 1년 동안 어깨, 목, 팔, 팔꿈치, 손목, 손 등 윗몸의(상지) 근육통의 건강문제에 대한 경험이 있습니까?’, ‘지난 1년 동안 엉덩이, 다리, 무릎, 발 등 아랫몸의(하지) 근육통의 건강문제에 대한 경험이 있습니까?’이다.
본 연구에서는 구조화된 자기 기입형 설문지를 사용하였으며, 분석에 사용된 문항은 대상자의 일반적 특성 및 직무 특성 7문항, 근골격계 자각증상 3문항, 수면문제 3문항, 주관적 정신건강 5문항으로 총 18문항이다.
본 연구의 자료수집기간은 2020년 9월 24일부터 11월 23일까지였다. 자료수집방법은 방문 돌봄 서비스 제공기관의 정해진 절차에 따라 기관장에게 연구의 목적과 설문지 내용을 설명한 후 연구참여에 동의한 기관을 방문하여 자발적으로 연구참여에 동의한 돌봄 노동자를 대상으로 실시하였다.

대상 데이터

본 연구는 경상남도 중부권에서 활동하고 있는 3개 직종(노인맞춤생활지원사, 재가방문 요양보호사, 장애인활동지원사)의 방문 돌봄 노동자를 연구대상으로 하였다. 경상남도 중부권 돌봄 노동자 지원센터에서 주관한 근로조건 및 건강수준 실태조사 참여에 동의한 기관에 소속된 돌봄 노동자 중 해당 연구참여에 동의한 자를 대상자로 선정하였다. 총 758명을 대상으로 설문조사를 실시하였고 주요 변수에 결측치가 있는 311부를 제외한 447부가 최종 분석대상이 되었다.

데이터처리

대상자가 지각한 근골격계 자각증상과 주관적 정신건강의 관계에서 수면문제의 매개효과를 검정하기 위해 Hayes (2013)의 PROCESS macro 프로그램을 이용하여 분석한 결과는 다음과 같다(Table 4). 통제변수로 월 소득수준(100만원 미만, 100~200만원 미만, 200만원 이상), 경력(1년 이하, 2~3년 이하, 4~6년 이하, 6년 초과), 주당 근무시간(15시간 이하, 16~24시간, 25~30시간, 31시간 이상), 직종(노인맞춤생활지원사, 재가 방문 요양보호사, 장애인활동지원사)을 더미변수로 처리하여 투입하였다.

성능/효과

대상자의 근골격계 자각증상은 평균 1.01±1.06점(범위 0~3점)이었고 수면문제는 평균 5.08±2.17점(범위 3~15점)이었으며, 주관적 정신건강은 평균 16.17±5.71점(범위 0~25점)이었다.
연구결과 직종에 따른 일반적 특성 중에 근골격계 자각증상에 유의한 차이를 보인 변수는 월 소득수준, 경력, 주당 근무시간과 직종이었고 수면문제에유의한 차이를 보인 변수는 월 소득수준이었으며, 주관적 정신건강 수준에 유의한 차이를 보인 변수는 직종이었다. 돌봄 노동자 3개 직종 중에 장애인활동지원사가 근골격계 자각증상을 가장 많이 호소하였고 주관적 정신건강 수준이 낮았다. 회귀분석 및 매개효과 검증결과를 종합하면, 재가방문 돌봄 노동자들의 근골격계 자각증상은 그 자체로 정신건강 수준에 부정적인 영향을 미쳤고 일부는 수면문제를 통하여 정신건강 수준에 부정적인 영향을 미친 것으로 나타났다.
001) 부분매개 효과가 있었다. 매개효과의 유의성 검증을 위해 10,000회의 부트스트래핑 기법을 활용하여 분석한 결과 B=-0.37, 95% CI=-0.58~-0.18으로 95%의 신뢰구간이 0을 포함하지 않아 수면문제의 매개효과가 통계적으로 유의한 것으로 나타나 유의한 간접효과가 있음을 확인하였다(Table 5).

후속연구

따라서 추후 연구에서는 근골격계 증상과 업무 관련성을 명확하게 규명할 수 있는 방법을 적용하여 연구하는 것이 필요하다. 둘째, 본 연구에서는 돌봄 노동자의 근무조건으로 월 소득수준, 경력, 주당 근무시간 변수만이 연구에 투입되어 돌봄 노동자의 근무조건의 특성을 구체적으로 반영하지 못하였다. 특히, 방문돌봄 노동자는 1:1 서비스로 인하여 인력대체가 어려워 휴가사용 및 근무 중 휴식이 어렵고, 시급제에 기반한 호출근로로 인하여 고용상태의 안정성이 떨어진다는 특징을 가지고 있다.
첫째, 근골격계 자각증상을 부위별 증상 경험 유무로 측정함으로써 증상의 강도나 빈도 등을 정밀하게 측정하지 못하였고, 경력이 1년 이내인 연구대상자의 경우 현재 호소하는 근골격계 자각증상이 업무로 인한 것인지 다른 이유로 인한 것인지 명확하지 않다. 따라서 추후 연구에서는 근골격계 증상과 업무 관련성을 명확하게 규명할 수 있는 방법을 적용하여 연구하는 것이 필요하다. 둘째, 본 연구에서는 돌봄 노동자의 근무조건으로 월 소득수준, 경력, 주당 근무시간 변수만이 연구에 투입되어 돌봄 노동자의 근무조건의 특성을 구체적으로 반영하지 못하였다.
특히, 방문돌봄 노동자는 1:1 서비스로 인하여 인력대체가 어려워 휴가사용 및 근무 중 휴식이 어렵고, 시급제에 기반한 호출근로로 인하여 고용상태의 안정성이 떨어진다는 특징을 가지고 있다. 추후 연구에서는 이러한 고용의 질과 관련한 다양한 측면을 반영하여 심층적으로 분석하는 것이 필요하다. 그럼에도 불구하고 본 연구는 방문 돌봄 서비스를 제공하는 여러 돌봄 직종을 대상으로 연구를 함으로써 돌봄 노동자 직종 간의 건강수준의 차이를 비교하는 결과를 제시하였고 돌봄 노동자의 여러 가지 건강문제 간 상호 관련성을 분석하였다는 측면에서 돌봄 노동자의 건강수준을 개선하기 위한 전략개발에 유용한 근거를 제시하였다는 의의를 가진다.
