Although "family" is the key concept of beneficiary system of the National Health Insurance(NHI) in Korea, there has been no study of family sociology on the beneficiary institution for the last 30 years. Utilizing public statistics and documents, newspaper articles and relevant literature, this study investigated norms of families implied in the NHI and the changes in the rules of beneficiary approval in the context of formal and inform institutions of family. Based on the historical institutionalism, this study figured out the path dependence on the traditional norms of patriarchal stem family in the early years of beneficiary regulations, while the path became flexible taking the bi-lateral family norms with political democratization in mid-1980s. As policy interests were focused on the financial balance by initiating the NHI covering whole population from 1989, the roles of family institutions were getting more depreciated. Rather, in recent years, the income requirements have become the focal issue for policy makers to keep the financial balance. However, the NHI have confronted strong financial pressures and people"s complaints on the equities of premium system. Only concentrating on the financial balance in lack of understanding the changes and diversities of Korean families in terms of structure and economic aspects, policy makers might have failed to establish a financial system integrating income requirements and realities of the families. To meet the challenges and needs embedded in the rapidly changing population structure and families in Korean society, the more systematic studies based on the empirical analysis on the family structures, economic realities and their contingent needs have been suggested for policy reforms in beneficiary regulations and premium collection of the NHI.
