Résumé Alors que la dysfonction érectile (DE) touche en moyenne un homme sur cinq dans le monde, on estime habituellement qu’elle est 4,5 fois plus fréquente chez le diabétique, et qu’elle survient chez lui en moyenne 10 à 15 ans plus tôt. La DE revêt chez le diabétique une valeur hautement séméiologique car elle marque, par son installation, une étape significative de l’évolution du diabète et de son retentissement viscéral. Elle apparaît aujourd’hui comme l’indicateur le plus fiable des complications microangiopathiques du diabète et le marqueur privilégié d’une possible ischémie silencieuse. La dysfonction érectile, lors de son apparition, s’accompagne chez tous les hommes de perturbations émotionnelles et relationnelles majeures. Chez le diabétique, les conséquences en sont encore plus marquées et responsables d’une altération importante de tous les domaines de la qualité de vie, ainsi que d’une moindre tolérance aux contraintes du diabète, pouvant en compromettre l’observance thérapeutique et l’équilibre métabolique. Les médecins généralistes ont encore beaucoup de mal à prendre en considération la DE du diabétique et à la dépister systématiquement, tout autant qu’à la prendre en charge. Il est cependant possible de faire évoluer nos pratiques pour mieux intégrer la prise en compte de la pathologie sexuelle chez nos patients, tout particulièrement diabétiques, d’aller au-devant de leur demande et de proposer des solutions thérapeutiques efficaces adaptées à leur attente.
