우리나라의 노인들을 대상으로 한 코호트에서 교란변수의 영향을 통제한 후의 음주와 뇌졸중 발생 간의 관련성을 코호트내 환자 대조군 연구를 통하여 파악하여 뇌졸중의 예방대책 수립에 필요한 기초자료를 제공하고자 하였다. 부산, 울산, 경남지역에 거주하는 65세 이상의 노인들 중 공무원 및 사립학교 교직원으로 구성된 의료보험의 피보험자 및 피부양자의 자격을 갖는 사람들을 대상으로 한 한국 노인약물 역학코호트에서, 1993년부터 1998년까지 뇌졸중 발생을 입원 수진자료를 통해 확인하고, 병원 방문조사를 실시하여 뇌졸중의 진단을 확정한 사람들 중, 1996년과 2002년 두 차례에 걸쳐 시행된 설문조사 결과를 통해 음주 및 이와 관련된 교란변수를 확인할 수 있는 경우를 환자군(n=217)으로 하여, 성, 연령, 설문조사 시점을 짝짓기 변수로 하여 1:4 의 비율로 무작위 배정된 대조군(n=868)을 추출하여 코호트내 환자 대조군 연구를 수행하였다. 성별과 연령에 대하여는 짝짓기 과정에서 보정하였고, 고혈압, 당뇨, 흡연 및 신체량 지수를 교란변수로 하여 음주와 허혈성 뇌졸중 발생에 대한 조건부 로지스틱 다변량 회귀분석을 시행하였다. 전혀 음주를 하지 않았던 군에 비하여 과거 음주군에서 허혈성 뇌졸중의 위험도가 유의하게 높음이 확인되었고(aOR 1.74, 95% CI 1.11-2.73), 소량의 음주를 하였던 군에서 허혈성 뇌졸중 발생이 감소되는 경향을 확인하였으나 유의하지는 않았다(aOR 0.73, 95% CI 0.46-1.18). 술의 종류에 따른 허혈성 뇌졸중 발생의 차이를 비교하였으나 유의하지 않았다. 그 외에 당뇨, 고혈압이 허혈성 뇌졸중 발병의 위험도를 유의하게 높이는 것으로 나타났다.

The aim of this study is to assess the association between alcohol consumption and stroke in Korean elderly people by nested case-control study. Source population are members of the Korean Elderly Pharmacoepidemiologic Cohort (KEPEC). They are 65 years or more, living in Busan, Ulsan, Kyung-Nam, and beneficiaries of Korean Medical Insurance Corporation. We observe the occurrence of stroke from Jan. 1993 to Dec. 1998 and confirm the cases, whose information about alcohol consumption and its related confounders could be acquired from mailed questionnaire. Two times of survey for the alcohol consumption and its related confounders using mailed questionnaire was conducted in 1996 and 2002. The potential cases on stroke were collected from the claims data between 1993 and 1998, and the hospital survey was conducted to confirm the final diagnoses. Using sex, age and source of questionnaire as a matching variable, we allocate four controls (n=568) randomly to the confirmed each one case (n=217) and then perform the nested case-control study. After controlling the hypertension, diabetes, smoking and body mass index, adjusted Odds Ratio (aOR) of ex-drinker using non-drinker fer reference is obtained through conditional logistic regression analysis (aOR 1.74, 95% CI 1.11-2.73). And there seems to be a tendency to have low risk of ischemic stroke in light drinkers, but net statistically significant (aOR 0.73, 95% CI 0.46-1.18). There is no statistically significant association between the type of alcohol and ischemic stroke. Diabetes, hypertensions increase the stroke risk significantly.
