<연구목적>o 본 연구의 최종 목적은 한국 암 환자 삶의 질을 측정할 수 있는 타당성이 검증된 표준화된 도구를 개발하는데 있음.o 구체적 목적은 첫째, 국외에서 대표적으로 사용되고 있는 미국형과 유럽형 삶의 질 측정도구를 한국 암 환자에게 적용하여 두 도구가 서로 유사한 속성을 측정하는지를 파악하는 것이고 둘째, 한국 암 환자 삶의 질 고유의 속성을 도출하는 것이며 셋째, 도출된 속성을 바탕으로 한국 암 환자 삶의 질 측정도구 문항을 구성하고 이에 대한 psychometric 검증을 하는 것임<연구방법>o 제 1단계: 전향적 횡단적 관계연구 설계를 이용하여 암 환자 326명을 대상으로 동시에 미국형 삶의 질 측정도구인 FACT-G와 유럽형 EORTC QLQ-C3O을 사용하여 임상조사를 실시 함. 수집된 자료는 정준상관분석, 다중회귀분석, Cronbach''s alpha 계수를 사용하여 분석.o 제 2단계: 질적연구방법 중 근거이론적 방법을 사용하여 10명의 암 환자를 심층면담하고, 면담내용은 녹음하고 필사, 면담내용에 대한 자료 분석은 개방코딩, 축코딩, 선택코딩의 절차를 거쳐 분석.o 제 3단계: 양적연구방법 중 방법론적 연구인 psychometric 검증의 원리에 입각해 한국 6대 암 환자(위암, 간암, 폐암, 대장암, 유방암, 자궁경부암) 337명을 대상으로 예비 삶의 질 문항에 대해 임상조사연구 실시. 내용타당도, 구성타당도(요인구성타당도, MDS, MTMM), 문항 수렴 및 판별타당도, 준거타당도, known-groups 타당도, 신뢰도, 민감성을 검증하기 위해 요인분석, MDS, 상관관계분석, 분산분석, Cronbach''s alpha, t-검정을 사용하여 분석.<연구결과>o 국외에서 흔히 사용되는 암 환자 삶의 질 측정도구인 미국형 FACT-G와 유럽형 EORTC QLQ-C3O은 한국 암 환자 삶의 질에 있어 서로 다른 면들을 측정하고 있는 것으로 나타남 EORTC QLQ-C3O은 주로 삶의 질 하부구조의 신체적 영역에 치중하여 측정하고 있으며, FACT-G는 신체, 정신, 사회적 영역을 측정하고 있는 편임, 특히 두 도구의 사회적 영역의 속성은 거의 다른 것으로 나타남. 이러한 결과는 한국 암 환자를 대상으로 실시되는 연구에서 두 도구 중 어떤 도구를 사용하느냐에 따라 결과가 다를 수 있음을 의미함. 또한 이러한 도구들을 한국 암 환자에게 사용하기에는 문화적 차이가 있음이 규명됨.o 한국 암 환자 고유의 삶의 질에 대한 핵심범주로는 ''안녕상태 유지하기''로 나타남. 즉, 질병예후나 치료와 관련된 부작용 정도에 따라 자신의 존재에 대한 의미를 인식하게 되고, 병을 극복하기 위해 대처방법을 찾으며, 그 결과 신체적 상태, 정서 상태, 대인관계, 사회적 활동 및 기능상태 변화를 가져옴.o 본 연구의 최종 결과물인 한국 암 특이형 삶의 질 측정도구는 총 21개 라이커트 유형의 문항으로 구성되었음. 이를 C-QOL (Cancer Specific Quality of Life)로 명명 함. 하부척도는 신체적 상태(6문항), 정서적 상태 (6문항), 사회적 기능(3문항), 대처기능(4문항), 걱정상태(2문항)로 구성되었음. C-QOL은 내용타당도, 구성(요인구성, MDS, 수렴, 판별) 타당도, 준거 타당도, 문항의 수렴 및 판별타당도, known-groups 타당도, 내적일관성 신뢰도, 민감성이 수립되었음. 따라서 앞으로 한국 암 환자 삶의 질 측정을 위한 연구나 실무에서 사용가능.<기대효과>o 무작위 임상시험에서 한국인 암 치료 효과를 평가하는 표준화된 지표로 사용.o 임상에서 치료과정에 따라 암 환자 삶의 질 변화를 모니터링 할 수 있는 측정도구로 사용.o 임상에서 암 환자 삶의 질 증진 중재효과 측정에 사용함으로, 과연 중재가 효과 있었는지를 체계적으로 평가 가능o 보건복지부 국가암관리사업 중 재가암환자 관리사업에 활용한다면, 전국적 재가암환자 삶의 질 DB를 구축할 수 있을 것으로 기대됨.

<PURPOSE>o The final purpose of the present study was to develop a valid instrument measuring quality of life for Korean patients with cancer.o The specific purpose was 1) to compare the two representative instruments, the FACT-G commonly used in the USA and the EORTC QLQ-C30 used in Europe, with Korean patients with cancer, 2) to derive unique attributes of quality of life from Korean patients with cancer, and 3) to develop and evaluate psychometric properties of a Korean cancer patients-specific quality of life scale(C-QOL).<METHODS>o Phase I: A prospective-and-cross sectional research design was used. A total of 326 cancer patients were participated. The FACT-G and EORTC QLQ-C30 were administered to the participants. The obtained data were analyzed using canonical correlation, multiple regression, and Cronbach''s alpha.o Phase II: Of qualitative methods, a grounded theory approach was used. Ten patients with cancer were interviewed. The contents of the interview were audio taped and transcribed verbatim. The interviewed data were analyzed using open coding, axial coding, and selective coding.o Phase III: Psychometric method was used. To 337 patients with stomach, liver, lung, colon, breast, or cervix cancer, a preliminary developed quality of life instrument were administered for the tests of content validity, construct validity (factorial validity, MDS, MTMM), item convergent and discriminant validity, criterion validity, known-groups validity, reliability, and sensitivity. For the tests, a factor analysis, MDS, correlation, analysis of variance, Cronbach''s alpha, and t-test were used.<RESULTS>o The FACT-G and EORTC QLQ-C30 commonly used as the quality of life instruments in the USA and Europe measured different aspects of the quality of life of Korean patients with cancer. The EORTC QLQ-C30 was mainly measured physical aspects of quality of life. The FACT-G measured physical, emotional, and social aspects. Especially, the two instrument measured almost different aspects regarding to the social aspects of the quality of life in Korean patients. In other way, different results may be led depend upon which instruments are used in a study with Korean patients with cancer. Also, there were cultural differences in the use of the two instruments with Korean cancer patients.o From the qualitative study using a grounded theory approach, ''keeping a wellbeing status'' was identified as a core category of quality of life in Korean patients with cancer. With the severity of the disease or side effects of treatments, patients perceived the meaning of their existence, and sought to coping ways to overcome the disease. As the results, physical, emotional, social, and functional aspects of their quality of life were changed.o The developed cancer-specific quality of life questionnaire for Korean patients with cancer, C-QOL, consists of 21 items with a 5-pont Likert-type scale. The C-QOL included five subdomains: physical status (6 items), emotional status (6 items), social function (3 items), and coping function (4 items). The C-QOL established content validity, construct validity, convergent validity, discriminant validity, known-groups validity, reliability, and sensitivity. Therefore, the reliable and valid C-QOL can be used in practice and for a study with Korean patients with cancer<EXPECTED EFFECTS>o The C-QOL can be used as a outcome indicator in a randomized clinical trial study on the evaluation of a treatment effect.o The C-QOL can be used for monitoring the change of quality of life along the processes of treatments for cancer.o The C-QOL can be used as a outcome indicator for the evaluation of an intervention program for the quality of life of patients with cancer.o The C-QOL can contribute into a database construction for the quality of life of community-dwelling cancer patients, if the instrument is applied for the care management program for cancer patients in a community, which is supported by the Ministry of Health and Welfare.
