약물반응의 차이는 나이, 성별, 환경 및 유전적 결정인자 등 수많은 요인에 의해 발생되는데 약물수송담체 및 약물대사효소나 약물표적부위(예, 수용체)의 유전자 변형으로 인한 차이에 기인한다는 것이 밝혀지면서, 환자 개개인의 유전적 다양성에 대한 관심이 커지고, 약물요법에 있어서 유전적 다양성의 중요성이 크게 부각되기 시작하였다. 특히, 이와 같은 약물유전체의 다양성에 관한 임상적 특성연구는 개개인에 대한 약물 부작용을 최소화 하고 최적의 약물요법을 수립하여 제공하는 맞춤 약물요법의 개발이라는 측면에서도 매우 중요하다. 국내에서는 이미 CYP2C19이나 CYP2D6의 한국인에 있어서의 유전적 특성과 약물대사와의 상관성에 관한 연구가 몇몇 연구자들에 의해서 수행되었으나, CYP2C9의 유전형과 약물대사특성을 대사동태 전체적인 측면에서 연구한 결과는 없었다. 따라서, 글리메피리드를 모델약물로 하여 한국인의 CYP2C9 유전적 다형성에 따른 약물대사특성과 약물동태의 변화양상을 평가하고, 약동학적 특성을 파악함으로써 한국인에 있어서의 대사효소의 유전형과 표현형과의 기초적인 상관성 자료를 구축하고 기초방법론을 개발하는 것이 본 연구의 목적이다. 본 연구에서는 먼저 CYP2C9 유전형을 검색하여 건강한 한국인 성인 남성 428명을 CYP2C9*1/*1과 *1/*3 군으로 구분한 데이터를 바탕으로 각 군에서 지원자를 선별하여 글리메피리드의 hydroxyglimepiride (M1) 및 carboxyglimepiride(M2)으로의 대사동태를 실험함으로써 CYP2C9의 유전형과 표현형과의 상관성을 밝히고자 하였다. 이러한 CYP2C9 표현형 데이터를 얻고자 혈청 중 글리메피리드와 hydroxyglimepiride(M1) 및 carboxyglimepiride(M2)의 HPLC-UV 검출기를 이용한 동시 정량법을확립하였고 그 분석법을 검증하였으며 건강한 한국인 성인을 대상으로 한 아마릴 정 2 ㎎ 단회 경구투약 시험을 수행하였고 혈청 중 글리메피리드와 hydroxyglimepiride(M1) 및 carboxyglimepiride(M2)의 농도를 분석하였으며 이를 WinNonlin 프로그램을 이용하여 약물속도론적 파라미터를 분석하였다. 그 결과 CYP2C9*1/*1과 *1/*3 군으로 구분된 유전형에 따라 글리메피리드의 hydroxyglimepiride(M1)로의 대사동태는 통계적으로 유의한 차이가 있음을 알 수 있었다.

Importantly, genetic makeup determines inherent pharmacokinetics, giving rise to interperson differences in drug absorption, distribution, metabolism, and excrerion. There were a few investigations about the relationship of CYP2C19 or CYP2D6 and its probe drugs in Korean population. However, there were few investigations about the relationship of CYP2C9 genotype and full pharmacokinetic patterns of its probe drugs as phenotype in Korean population. The aim of this study was to evaluate the relationship between the genetic polymorphisms of CYP2C9 and glimepiride in healthy Korean subjects. 428 subjects were genotyped for the CYP2C9*3 allele. Simultaneous analytical method of glimepiride, hydroxyglimepiride(M1) and carboxyglimepiride(M2) for bioavailability test was developed and validated. After the validation, bioavailability study was conducted by administering $Amaryl^{\circledR}$ tablet (2 ㎎ glimepiride) orally once and pharmacokinetic parameters were calculated.Pharmacokinetic parameters of glimepiride and metabolic ratio to hydroxyglimepiride (M1) were significantly different between the *1/*1 and *1/*3.
