Drug-induced liver injury (DILI)는 약물 퇴출 등의 주요 원인이 되는 약물 부작용 중 하나이다. Non-steroidal anti-imflammatory drugs (NSAIDs)는 세계에서 가장 많이 처방되는 약물군에 속하기 때문에 약물에 의한 간독성의 10%가 NSAIDs와 관련되어져 있다고 알려져 있다.

Drug-induced liver injury (DILI)는 약물 퇴출 등의 주요 원인이 되는 약물 부작용 중 하나이다. Non-steroidal anti-imflammatory drugs (NSAIDs)는 세계에서 가장 많이 처방되는 약물군에 속하기 때문에 약물에 의한 간독성의 10%가 NSAIDs와 관련되어져 있다고 알려져 있다.
본 연구에서는 NSAIDs 특이적 간독성 지표를 발굴하기 위해 유전자 발현 분석을 수행하였다. 6개의 NSAIDs (sulindac, diclofenac, piroxicam, ketoprofen, indoprofen, flufenamic acid)를 각 약물의 Cmax 농도에 해당하는 양으로 사람일차간세포에 처리하였다. NSAIDs 처리에 따른 간세포의 세포 반응을 알아보기 위해 먼저 MTT, CellTiterBlue, LDH and LIVE/DEAD staining assay 등의 in vitro assay를 수행하였다. Diclofenac 및 sulindac의 세포 독성이 나머지 약물 4종보다 높은 것을 확인하였다. 이러한 결과는 NSAIDS 유발 간독성 발생 빈도에 대한 미국 FDA의 임상 보고와 일치하였다.
사람일차간세포에 상기 NSAIDS 약물 6종을 8시간 및 24시간동안 처리 후 real-time PCR을 수행하였다. 총 29개의 유전자가 8시간 처리의 diclofenac 및 sulindac 처리군에서만 유의하게 발현 변화되었다. 그리고 24시간처리에서는 총 19개의 유전자가 diclofenac 및 sulindac 처리군에서만 유의하게 발현변화되었다. NSAIDs 특이적 간독성 지표를 발굴하기 위해 비NSAIDs 간독성 약물인 acetaminophen, valproic acid 및 flutamide를 사람일차간세포에 처리하고 real-time PCR을 수행하였다. 8개 유전자(8H;GADD45A, HYOU1, SLC2A3, 24H; DNAJC3, GCLC, CD68, ABCC2, MLXIPL)의 발현이 sulindac 및 diclofenac이 처리된 간세포에서만 유의하게 변화되는 것을 확인하였다. 그리고 이들 8개 유전자의 단백질 발현도 sulindac 및 diclofenac이 처리된 간세포에서만 변화되는 것을 확인하였다.
사람일차간세포 및 랫드 일차간세포에 diclofenac 및 sulindac 24시간 처리에 따른 19개 유전자의 유전자 발현 양상을 비교한 결과, 두 종간의 유전자의 발현이 거의 반대 양상을 나타내는 것을 확인할 수 있었으며, 이러한 결과는 종간 extrapolation에 문제점이 있다는 기존의 보고를 증명하는 귀중한 자료인 것으로 사료된다. 또한 본 연구는 사람일차간세포를 이용하여 NSAIDs 특이적 간독성 생체지표를 최종 제시함으로써 이를 신약 개발 및 안전성 평가시 활용할 수 있는 중요한 자료로 사료된다.

Drug-induced liver injury (DILI) is the most common adverse event causing drug non-approvals and drug withdrawals. Non-steroidal anti-imflammatory drugs (NSAIDs) constitute a family of drugs which represents one of the most frequently prescribed around the world. Thus, NSAIDs list on the top for causes of DILI. Indeed, roughly 10% of total drug-induced hepatotoxicity is NSAIDs related.
In this study, we performed gene expression analyses to find NSAIDs-induced hepatotoxic markers. Human hepatocytes were treated with 6 NSAIDs (sulindac, diclofenac, piroxicam, ketoprofen, indoprofen, flufenamic acid) at a 100-fold therapeutically active plasma maximum concentration (Cmax). To examine the cellular responses under the drug treatments, we conducted cell viability test such as MTT, CellTiterBlue, LDH and LIVE/DEAD staining assay. The hepatocytes treated with diclofenac and sulindac showed lower viability than other drugs. These results were consistent with previous reports on the incidence of NSAIDS-induced liver injury.
We performed real-time PCR at 8 and 24 hour after drug treatments. A total of 29 genes were significantly modulated in both diclofenac and sulindac at 8h, but not changed in other NSAIDs. Additionally, a total of 19 genes were significantly modulated in both diclofenac and sulindac at 24h, but not changed in other NSAIDs. We also treated with a representative non-NSAIDs hepatotoxic drug such as acetaminophen, valproic acid and flutamide, on human primary hepatocytes and performed real-time PCR to select NSAIDs-specific hepatotoxic markers. The expressions of 8 genes (8H; GADD45A, HYOU1, SLC2A3, 24H; DNAJC3, GCLC, CD68, ABCC2, MLXIPL) showed significant modulation in both sulindac and diclofenac-treated hepatocytes. Proteins expression of these selected markers also showed modulation in both sulindac and diclofenac-treated hepatocytes.
Our findings provided gene level information that may serve for elucidation of the molecular mechanisms underlined beneath NSAIDS-induced hepatotoxicity and potential NSAIDs-specific hepatotoxic marker that may use in hepatotoxicity screening of new drug development
