◆ 연구내용 및 방법
A. 위암의 가족력이 있는 위암환자에서 헬리코박터 감염의 역할 - 환자 대조군 연구
1) 연구대상
① 선정기준(case)
- 위선암으로 확진된 환자 중 위암의 가족력(1st degree & 2nd degree)이 있는 환자
② 선정기준(control)
- 위암의 가족력(1st degree & 2nd degree)이 있으나 위암이 없는 대상
2) 연구방법
① 환자대조군 연구
② 혈액채취: Immunoblot (Helicoblot 2.1 kit) 사용하여 H. pylori검사와 CagA, VacA항체검사, 유전자 검사( IL-1β, IL-1RN, IL-8, MUC, NAT, GST 등)
③ 통계분석: Multiple logistic regression method-Odds ratio
B. 젊은 위암환자의 가족에서 헬리코박터 감염률과 전암성 병변의 유병률- 환자 대조군 연구
1) 연구대상
① 선정기준(case)
- 조직학적으로 위선암으로 확진된 40세 미만 위암환자의 40세 미만 형제 자매
- 위내시경 검사에서 위암이 발견되지 않은 자
② 선정기준(control)
- 건강검진으로 내시경검사를 받는 자 (40세 미만)
- 위내시경 검사로 위암이 발견되지 않은 자
2) 연구방법
① 환자대조군 연구
② 내시경 검사에서 H. pylori 감염 여부 평가
③ H. pylori 감염진단 및 조직검사: Updated Sydney system에 의하여 평가
④ 혈액채취: Immunoblot (Helicoblot 2.1 kit) 사용하여 H. pylori검사와 CagA, VacA항체검사, 유전자 검사( IL-1β, IL-1RN, IL-8, MUC, NAT, GST 등)
⑤ 통계분석: Multiple logistic regression method-Odds ratio for H. pylori infection
C. 위암환자의 가족에서 헬리코박터 제균을 통한 위암 예방효과- 전향적무작위대조군연구
1) 연구대상
① 선정기준
- 위선암으로 확진된 환자의 형제 자매, 자녀 중 40세 이상 65세 미만
2) 연구방법
① 전향적무작위대조군 연구
② 참여하기로 동의한 경우에 내시경 검사, 채혈, 설문, 식습관 조사를 시행
③ 내시경 검사- 위암 존재 여부 평가하고 H. pylori 감염 여부 검사
④ H. pylori 감염진단 및 조직검사(H&E stain, Wright Giemsa stain, CLO test)
: Updated Sydney system에 의하여 평가
⑤ 혈액채취: Immunoblot (Helicoblot 2.1 kit) 사용하여 H. pylori검사와 CagA, VacA항체검사, 유전자 검사( IL-1β, IL-1RN, IL-8, MUC, NAT, GST 등)
⑥ 연구대상자의 무작위 배정 및 추적
- 치료군(active treatment group): H. pylori 박멸을 위한 3제 요법
- 위약군(placebe group): 위약 복용
- 추적 관찰: 매 2년마다 6년째까지 내시경 추적검사: 위암발생 여부 검사

1. Objectives
The primary objective of this study is to evaluate whether Helicobacter pylori infection increases gastric cancer risk in subjects with family history of gastric cancer
2. Eligibility
1) Case - Gastric adenocarcinoma patient confirmed by EGD and biopsy
- Family history of gastric cancer in 1st or 2nd degree relatives
2) Control - Health screening subject
- Gastric cancer is not found at endoscopy
- Family history of gastric cancer in 1st or 2nd degree relatives
3. Plan
1) Case-control study (Age, Sex, SES matching)
2) H. pylori infection analysis
- Hp IgG Antibody by serology and virulence factors (CagA, VacA) by Immunoblot
- Gene in relation to H. pylori infection(IL-1B, IL-8, IL-10, TNF-A, TGF-B, etc.) and gastric cancer incidence risk(MUC, NAT, GST, CYP, E-cadherin, etc.)

Helicobacter pylori Infection Rate and Prevalence of Preneoplastic Conditions in Siblings of Young Gastric Cancer Patients: A Case-Control Study
1. Objectives
The primary objective of this study is to evaluate whether Helicobacter pylori infection rate and prevalence of preneoplastic conditions are increased in siblings of young gastric cancer patients.
2. Eligibility
1) Case - Sibling of patient less than 40 years with gastrric adenocarcinom confirmed EGD and biopsy (Age: over 20 years-less than 40 years)
- Gastric cancer is not found at endoscopy and biopsy
2) Control - Health screening subject (Age: over 20 years-less than 40 years)
- Gastric cancer is not found at endoscopy
- No family history of gastric cancer within 2nd degree relatives
3. Plan
1) Case-control study (Age, Sex, SES matching)
2) H. pylori infection analysis
- Endoscopy and biopsy(Antrum 2 pieces, Body 2 pieces)
- Hp IgG Antibody by serology and virulence factors (CagA, VacA) by Immunoblot
- Gene in relation to H. pylori infection(IL-1B, IL-8, IL-10,, TNF-A, TGF-B, etc.) and gastric cancer incidence risk(MUC, NAT, GST, CYP, E-cadherin, etc.)

Helicobacter pylori Eradication to Prevent Gastric Cancer in Subjects with Family History of Gastric Cancer: A Randomized Controlled Study
1. Objectives
The primary objective of this study is to evaluate whether Helicobacter pylori infection rate and prevalence of preneoplastic conditions are increased in siblings of young gastric cancer patients.
2. Eligibility
Sibling or offspring of more than 40 years patients with gastric adenocarcinoma confirmed by EGD and biopsy (Age: 40-65 years)
3. Plan
1) Study design: Prospective, randomized, placebo-controlled double blind study
2) H. pylori infection analysis
- Endoscopy and biopsy (2 at antrum LC, 2 at upper body LC for histology and 1 for CLO test at upper body GC side)
- Hp IgG Antibody by serology and virulence factors (CagA, VacA) by Immunoblot
- Gene in relation to H. pylori infection(IL-1B, IL-8, IL-10, TNF-A,TGF-B, etc.) and gastric cancer incidence risk(MUC, NAT, GST, CYP, E-cadherin, etc.)
3) Questionnaire after informed consent
4) Randomization and follow up
① Treatment arm : Proton pump inhibitor (PPI) + two antibiotics for 1 week
② Placebo arm : Placebo for PPI and antibiotics for 1 week
③ Follow up : EGD and biopsy every 2 years (2, 4, 6, 8th year)
5) Statistical consideration
① Sample size calculation: 905 patients at each arm (3100 screening)
② Outcom measure
• Primary -Incidence of gastric cancer between active treatment and placebo groups
• Secondary -Incidence of gastric cancer in patients with or without precancerous lesions, compared between the two groups
③ Accumulative gastric cancer incidence : Kaplan-Meyer method and log-rank test
Factor of gastric cancer incidence : Cox regression analysis
◆ Results
1. Although, H. pylori infection rate was not increased in the family members of gastric cancer patients, it is clearly increased in the gastric cancer patients with family member.
2. H. pylori and resultant mucosal changes are associated with gastric cancer in young adults. Young siblings also have increased H. pylori infection and intestinal metaplasia.
3. TNF-α 1030 C/T polymorphism among cytokine SNPs is associated with a significantly decreased risk of gastric cancer with family history and this association was further confirmed among H. pylori infected subjects.
4. Helicobacter pylori Eradication to Prevent Gastric Cancer in Subjects with Family History of Gastric Cancer: A Randomized Controlled Study
Screening: 3100/3000 (103.3%) Randomization: 1837/1810 (101.4%)
◆ Conclusions
In the family members of gastric cancer patients, H. pylori infection and advanced histological changes are risk factors for gastric cancer. This can be applied for the young gastric cancer patients. Gene-enviroment interaction in gastric carcinogenesis of H. pylori is suggested.
