본 연구팀은 국내.외 연구현황과 선행연구 결과 도출된 자료 및 식약청 주관부서와의 협의를 바탕으로 보건건복지부 코드 311번으로 분류되어 있는 대사성 질환에 허가초과로 사용되는 의약품 중 안전성, 의료상 필요성을 감안하고 의료현장에서 사용되고 있는 대사성 의약품 중에서 임상연구를 통한 안전성 및 유효성을 평가하기 위한 의약품 1종을 선정하기 위해 국내.외 문헌을 검토하였고, 심평원에서 제시하는 대사성 의약품 중 의료현장에서 사용되나 아직 안전성 및 유효성의 근거가 미약하여 임상연구가 필요한 calcitriol을 선정하였다. Calcitriol은 vitamin D로서 임상연구 등에서 RAS system을 억제하고 면역기전에 관여함으로써 단백뇨를 완화시키는 효과를 보였으며, 단백뇨의 감소는 곧 만성콩팥병으로의 이환을 지연시키므로 신보호 효과가 있는 것으로 보고되고 있다.
선정된 의약품에 대하여 비당뇨 만성콩팥병 환자에서 calcitriol을 사용함으로써 단백뇨의 감소와 신보호 효과를 평가하여 허가초과 사용의 안전성 및 유효성을 확보하고자 임상시험 계획을 수립하였다. 본 연구는 임상시험의 과학적 근거 수준을 높이기 위해 무작위배정, 이중맹검, 위약-대조, 평행 연구를 설계하였고 약사법령에 의거하여 임상시험 계획을 수립하였으며, 수립된 임상시험 계획을 토대로 임상시험 계획서를 작성하여 식약청과 임상기관의 승인 절차를 거쳐 임상시험을 위한 기반을 마련하고, 승인된 계획서에 따라 임상시험대상자 모집 등의 임상시험을 수행하였다. 시험약은 calcitriol이고 위약은 주성분인 calcitriol을 제외하고 fractionated coconut oil로 충진하였으며, 시험약과 위약 모두 6개월간 투여하였다. 일차 유효성 평가변수는 random urine에서 단백의 변화량이고, 이차 유효성 평가변수는 신기능의 변화, 소변 중 신손상 지표의 변화를 평가하고자 하였다. 안전성 평가변수는 약물이상반응의 발생을 평가하였다.
연구결과, 총 126명의 임상시험대상자를 모집하였고, 그 중 23명이 중도 탈락되어 최종적으로 103명이 연구를 완료하였다. 탈락된 임상시험대상자 중 2명은 경미한 두통과 부종을 호소하였으나 증상이 특이적이지 않아 약물관련 이상반응으로 고려되지 않았다. 소변으로의 단백배설량은 시험군에서 유의하게 감소하였으나 대조군에서는 유의한 차이가 없었다. 단백배설량의 백분율 변화는 시험군에서 더 크게 감소하는 경향을 보였고, 신기능의 변화 또한 시험군에서는 신기능 지표인 크레아티닌 청소율이 향상되는 결과를 보였다. 또한 신손상 지표인 MCP-1의 소변 중 배설량도 시험군에서 유의하게 감소하는 것으로 나타나 calcitriol의 신보호효과를 입증할 수 있었다. 하위집단 분석으로 만성콩팥병 3단계 임상시험대상자들을 대상으로 분석한 결과, 1, 2단계 만성콩팥병 임상시험대상자들에 비해 calcitriol의 사용으로 단백이 유의하게 감소하였고, 신기능 또한 유의하게 향상된 것을 볼 수 있었다. Calcitriol에 대한 안전성 평가로서 고칼슘혈증, 두통, 부종 등을 포함한 이상반응을 평가했을 때, 중증의 이상반응은 보고되지 않았고, 두 군간에도 이상반응 발생빈도의 유의한 차이가 없는 것으로 보여, 안전함이 밝혀졌다.
본 연구에서 또한 허가초과 사용의 재분류를 위해 각국의 허가사항, 국내외 문헌과 임상지침, 임상연구문헌 등을 분석하였고, 이를 토대로 calcitriol의 허가초과 사용에 대한 안전성 및 유효성 평가를 실시하였다. 임상연구 결과와 문헌분석 자료를 종합하여 평가한 결과에 따라 허가초과 의약품 등급을 분류했을 때 calcitriol은 ClassIIaa였으며, 등급분류 후 후속조치 기준에 따라 지속적 사용이 가능함을 제시하였다.
본 연구의 결과로 제시되는 calcitriol의 안정성 및 유효성 평가의 결과를 바탕으로 대사성 의약품 중 국내에서 임상 근거가 미미하여 허가초과로 사용되지 않는 의약품이나 기존의 허가사항으로 인정받지 못하여 허가초과로 실제 임상 현장에서 사용되는 약제들의 허가사항 반영의 근거의 예시 또는 부적절한 근거로 허가초과로 사용되는 의약품의 사용 중단의 선행사례로 활용되어, 추후 허가초과 의약품을 대상으로 한 임상연구를 활성화하고 임상시험의 평가기준을 마련하는 단초가 될 수 있을 것이다. 또한 본 연구의 결과로 대사성 의약품의 허가초과 사용에 대한 임상 시험의 지침을 개발함으로써 다른 의약품의 허가사항 초과 사용 시 안전성과 유효성을 평가하는 데 기본방향을 제시함으로써 허가초과 의약품의 평가 정책 수립에 기여할 수 있을 것으로 사료된다.

Recent studies have demonstrated the broad-ranging activities of vitamin D including the regulation of renal and cardiovascular functions, as well as the modulation of RAS system and immune responses. Previous reported experimental data have indicated that vitamin D analogues lead to a decrease in albuminuria and slow the progression of kidney injury through activation of the vitamin D receptor in several animal models of kidney disease. Therefore, the objectives of this study was to evaluate the safety and efficacy of the off-label use of calcitriol to ensure safe drug use for the public and to promote new drug development in the pharmaceutical industry.
This study was a double-blinded, randomized, placebo-controlled, parallel trial of calcitriol in patients with non-diabetic kidney disease. The study protocol was obtained permission from the Commissioner of the Korea Food and Drug Administration accordance with the Pharmaceutical Affairs Act (2012) and approved by three independent ethics committees at Seoul National University Hospital, Seoul National University Bundang Hospital, and Boramae Medical Center. The randomization sequence was computer generated by an independent statistician and preserved in a sealed envelope and Patients were assigned (1:1) to receive calcitriol 0.5 mcg/day or placebo for 24 weeks. Patients were recalled every 3 months for clinical evaluation and measurement of serum intact parathyroid hormone (iPTH), calcium, phosphorus, creatinine, and proteinuria. The difference in change in proteinuria between baseline measurement and the last follow-up after taking calcitriol (24 weeks in study completers) between the two groups was the predefined primary end point. Secondary efficacy measures were the mean changes in renal function and various renal damage markers from baseline to the last measurement during treatment. All adverse events were recorded throughout the study. Analysis was by ITT. This trial is registered with ClinicalTrials.gov, number NCT01512862.
Between December 2012, and October 2013, 126 patients were enrolled and assigned to receive calcitriol (n=64) or placebo (62), and 23 patients have dropped out. UACR was significantly decreased only in calcitriol group (calcitriol vs. placebo, -0.154 [95% CI, -0.257 to -0.052] vs. -0.083 [95% CI, -0.207 to 0.041]). Percent changes in both UPACR and UACR were tended to decrease in calcitriol group, but those were increased in placebo group (calcitriol vs. placebo, UPCR and UACR,–13.16% vs. 0.621%, -13.88% vs. 6.19%, respectively). Serum creatinine and creatinine clearance were improved in calcitriol group in contrast to the control, however, there was no statistically significant difference between two groups (p=0.13). No serious adverse events were reported in study period. And incidence of hypercalcaemia, adverse events such as headache, mild leg edema was similar between groups receiving calcitriol and placebo. Addition of 0.5 mcg/day calcitriol to general treatment with or without RAS inhibition safely lowers residual albuminuria in patients with non-diabetic nephropathy, and could be a novel approach to protect renal function in CKD patients.
To provide objective, systematic, and accurate evaluations of off-label use, we searched and selected relevant information from tertiary databases, including standard and wide-accepted textbooks amongst healthcare providers, secondary databases including pharmacopoeia from eight countries and guidelines from related academic databases such as AHFS/Martindale/Micromedex, and primary literatures from Pubmed, EMBASE, and Clinicaltrials.gov. We collected and rearranged all relevant and detailed information throughout the databases stated above to provide correct information.
Another goal of this study was to establish ‘A clinical trial guideline for the off-label use of vitamin D agent in reducing proteinuria' and we expect this guideline to promote clinical trials of off-label drug use and the result of this study to be a basis of improving public health and robust medical practice for practitioners, advancement of healthcare policies, and promoting clinical trials and new drug development in the pharmaceutical industry.
