IV.

To control the symptoms of chronic arthritis, it is very important to exercise continuously for the rest of life. So for 902 patients with chronic arthritis, it has been surveyed 1) whether they exercised after being diagnosed or not, and how their functions are, 2) what kinds of exercise they did, how often they exercised, how long they exercised, how much they enjoyed, and how hard they felt, 3) how long they continued to exercise, 4) what made them adhere exercise, 5) what made them start exercise, and 6) what made them perform exercise, 7) what are different between exerciser and non-exercisers. The affecting variables are selected by Pender's health promotion model and empirical studies. Finally, based upon these results it is aimed to develop the model to predict exercise commencement, maintenance, and performance.
Over 50% of subjects respond that they had never exercised before. Even though the functions are more improved than when they are diagnosed, they still have problems in doing activities of daily life. And the function of RA patients is lower than that of OA patients.
Walking is found to be the most frequent performed exercise. But most of the exercises are performed by less than 10% of subjects. Usually twice a week and less than 30 minutes were spent for excercise. And they feel more difficult than enjoyable.
It is age, education level, and job to differentiate exercisers and non-exercisers. The functional status is not different. And exercisers took part in self-help and water-exercise program more than non-exercisers. Exercise specific cognition and affect (perceived exercise benefit, perceived exercise barrier, perceived exercise self-efficacy, and enjoyment) are different between these two groups. Social support as an interpersonal influence is also different.
It is the financial status, job, and exercise self-efficacy to predict significantly the intent of exercise adherence. And 41% of the variance of exercise maintenance were explained by these variables. Age, education level, exercise benefit and exercise self-efficacy is significant variables to predict the intent of exercise commencement. And exercise barrier, self-efficacy, social support significantly predict exercise performance. Based upon these results, models to predict exercise commencement, maintenance, and performance were developed.
These results will be used to modify the programs administered by Korean Rheumatism Health Professionals. Also these will be used in the education program for instructors who care patients with chronic arthritis. Finally, theses are supposed to be used as the theoretical framework in the exercise program for the patients with chronic disease.
