1. 보건소별 건강증진사업 조사
가. 건강증진사업별 보건소 분포 분석
1개 핵심사업만 수행하는 보건소는 흡연 예방 및 금연 사업 17개소 17.9%, 운동사업 13개소 13.7%, 영양사업 8개소 8.4%, 절주사업 3개소 3.2%였다. 2개 중복사업 보건소는 27개(28.4%), 3개 중복은 5개(5.3%), 4개 중복사업 대상은 8개(8.4%)였다.
나. 건강증진사업별 규모
흡연 예방 및 금연 사업은 총 19개 사업, 운동사업은 10개, 영양사업은 29개, 절주사업은 10개 사업이었다.<

1. 보건소별 건강증진사업 조사
가. 건강증진사업별 보건소 분포 분석
1개 핵심사업만 수행하는 보건소는 흡연 예방 및 금연 사업 17개소 17.9%, 운동사업 13개소 13.7%, 영양사업 8개소 8.4%, 절주사업 3개소 3.2%였다. 2개 중복사업 보건소는 27개(28.4%), 3개 중복은 5개(5.3%), 4개 중복사업 대상은 8개(8.4%)였다.
나. 건강증진사업별 규모
흡연 예방 및 금연 사업은 총 19개 사업, 운동사업은 10개, 영양사업은 29개, 절주사업은 10개 사업이었다.
다. 건강증진사업별 주요내용 분석
흡연예방 및 금연사업수행은 흡연실태조사와 홍보, 각종대회, 캠페인등이 각각 100.0%이었고, 금연교실 운영 등이 64.7%, 교육사업58.8%, 흡연학교지정이 52.9%의 순이었다.
운동사업수행을 보면 기초체력 검사가 53.9%, 교육사업이 53.9%, 운동 프로그램 수행이 46.2%, 운동실천율 조사가 38.5%, 운동상담실 운영이 30.8%였다. 영양사업 수행을 보면 영양실태 조사, 영양교육 사업, 홍보, 각종대회 및 캠페인, 상담실 운영 사업을 100.0%로 수행하고 있었고, 절주사업 수행을 하고 있는 보건소는 음주실태 조사, 절주교육, 상담실운영, 그리고 각종 홍보사업과 캠페인 등 사업을 100.0% 수행하고 있었다.
2. 설문조사
가. 보건소 건강증진사업에 대한 의견
1) 보건소에서 가장 우선적으로 개선해야 할 사항으로는 '전문인력의 보강’이 51.5%로 가장 높았으며, 그 다음이 ‘시설장비 보강’ 21.2%의 순이었다.
2) 현재 보건소에서 이루어지고 있는 건강증사업의 업무 과중도는 ‘보통’정도라는 응답률이 48.5%였으며, 보건소의 건강증진사업에 대한 만족도는 ‘만족’한다는 응답률이 42.4%로 만족하고 있었으며, 업무담당기간별로는 ‘7-10년 미만’ 근무자들의 만족도가 높았다.
3) 건강증진사업에 대한 해당 지방자치단체장의 관심도에서는 ‘보통’이라는 응답률이 45.5%였고, 직렬별로는 ‘보건직’에서 가장 높은 응답률을 보였고, 직급별로는 ‘5급’에서 58.3%로 관심도가 타 직군 보다 높았다.
4) 건강증진 주요사업에 대한 각각의 사업별 교육의 필요성에서는 ‘필요함’이 54.5%로 가장 높았고, ‘반드시 필요함'이 39.4%로 대다수의 응답자가 교육의 필요성을 인식하고 있었으며 연령별로는 ‘40대’가 50.0%로 가장 높은 응답률을 보였다.
5) 건강증진사업에 대한 예산에 대한 견해에서는 ‘보통’이 48.5%로 가장 높았고, ‘만족’이 42.4%로 대체적으로 응답자들은 예산에 대하여 만족하고 있었으며, 현 업무담당기간별로는 ‘3-5년 미만’의 응답자들이 가장 많이 만족하고 있었다.
6) 효율적인 보건사업 수행을 위한 “모든 보건요원의 보건교육사화”에 대한 견해에서는 ‘좋은 생각이다’가 42.4%로 가장 높았고, ‘매우 좋은 생각이다’가 36.4%로 대부분이 찬성하고 있었으며, 직렬별로는 ‘행정직’에서 “보건교육사화”는 좋은 생각이라고 인식하고 있었다.
7) 건강증진사업 수행 인력에 대한 의견에서 ‘불만’이라는 응답률은 39.4%였고, 연령별로는 ‘30대‘가, 직급에서는 ’4급‘이, 현재 업무 담당 기간이 ’1년 미만‘인 응답자에서 각각 불만족하다는 의견이 많았다.
8) 현재 수행중인 건강증진 사업 중 가장 중점을 두고 있는 사업에서는 ‘금연’이 42.4%로 가장 높았고, 복지업무와 보건업무를 연계 또는 공동추진하는 것에 대한 의견은 ‘필요하다‘는 응답률이 60.6%로 가장 높았다.
9) 건강증진사업의 평가에 대한 견해에서는 ‘반드시 필요하다’가 57.6%로 가장 높았다.
나. 건강증진관련 보건 요원 교육에 대한 의견
1) 지난 2년간 건강증진 사업 관련 교육을 받은 횟수에서는 ‘1회’가 60.0%로 가장 높았고, 직렬별로는 ‘간호직’이 직급에서는 ‘6급’이, 보건소 총 근무기간별로는 ‘20년 이상’이 교육을 1회 정도 받았다.
2) 건강증진사업 관련 교육받은 시간은 ‘5-6시간’이 42.4%로 가장 많았고, 직렬별로는 ‘의무직’, 현 업무 담당기간별로는 ‘5-7년 미만’이 높은 응답률을 보였다.
3) 건강증진사업 관련 교육받은 기간의 적절성은 ‘적절하지 않다’는 응답률이 48.5%로 가장 높았으며, 사업수행에 필요한 적당한 교육기간은 ‘1일’이 39.4%로 가장 높았다. 건강증진사업 관련 교육의 내용의 업무수행 적절성은 ‘적절하다’가 39.2%, ‘보통이다’라는 응답률은 45.5%였으며, 교육자료의 적절성도 ‘보통이다’라는 응답률이 63.6%로 가장 높았으며 ‘적절하다’는 30.3%였다.
4) 보건교육이 가장 요구되어지는 분야 1순위에서는 ‘금연’이 42.4%, 2순위는 ‘운동분야’로 18.2%였다. 생애주기별 건강증진사업에서 보건교육이 요구되어지는 보건분야 1순위에서는 ‘청소년 보건’이 27.3%로 가장 높았고, 2순위는 ‘모자 보건’, 3순위는 ‘노인 보건’과 ‘영유아 보건’이었다.
5) 사업별 보건교육 지침서나 프로그램개발의 필요성에서는 ‘필요하다’가 66.4%, 특성별로 보면 직급에서는 ‘6급’에서, 현 업무 담당기간별로는 ‘7-10년 미만’자에서 필요하다는 응답률이 높았다.
6) 건강증진사업 수행시 가장 부족하다고 생각되는 교육자료는 ‘절주관련자료’가 39.4%로 가장 높았으며, 홍보효과가 높다고 생각되는 자료는‘비디오 테이프’가 57.6%로 가장 높은 응답률을 보였다.
7) 건강증진사업에서 보건교육 업무 수행시 가장 부족하다고 생각하는 것은 ‘보건교육 기획능력’이 33.3%로 가장 높았고, 다음이 ‘보건교육 전략기술’과 ‘강의 요령’이었다.
8) 건강증진사업 수행시 우선적으로 개선해야 할 사항 1순위에서는 ‘담당업무 인력부족’이 66.7%로 가장 높았으며, ‘시설장비부족’, ‘교육자료 부족’, ‘관련단체 비협조’의 순이었다. 보건교육 효과가 높다고 생각되는 교육보조 매체에서는 ‘2가지 복합 활용’이 57.6%로 가장 높았고, ‘빔 프로젝트’가 다음이었다.
다. 지난 2년간(2002~2003년)흡연예방 및 금연사업 설문조사 결과
1) 흡연예방 및 금연사업을 수행하고 있는 보건소는 조사대상 보건소 66개 중에서 64개소로 97.0%로 대부분의 보건소가 흡연예방 및 금연사업을 시행하고 있었다.
2) 흡연예방 및 금연사업의 내용을 보면, ‘도시’의 경우는 ‘전문가 초청강연’(83.3%), ‘흡연자에 대한 금연사업’(83.3%)을 가장 많이 시행하고 있었으며, ‘홍보자료 제작과 배부’를 66.7%의 보건소에서 시행하고 있었다.
3) 예산투입 규모 순위별 사업을 보면 ‘도시’의 경우 1순위는 ‘홍보자료 제작 및 배부’였고, 2순위는 ‘전문가 초청강연’과 ‘흡연자 금연사업’이었다. ‘농촌’의 경우 1순위는 ‘흡연율 조사’였고, 2순위는 ‘흡연자 금연사업’이었다.
4) 건강증진사업의 필요성 순위를 보면 ‘도시’의 경우 1순위는 ‘흡연자금연사업’이었고 2순위는 ‘흡연예방 및 금연교실’과 ‘금연 지도자 교육’이었다. ‘농촌’의 경우 1순위는 ‘청소년 금연캠프’였고, 2순위는 ‘흡연예방 및 금연교실’이었다.
5) 흡연예방 및 금연사업 관련 장비보유 현황을 보면 ‘도시’의 경우 ‘노트북’과 ‘교육용모형’을 보유한 보건소가 각각 8개소(66.7%)로 가장 높았으며, ‘농촌’의 경우 ‘노트북’과 ‘OHP’를 보유한 보건소가 각각 48개소(81.5%)로 가장 높았다.
6) 흡연예방 및 금연교육을 위한 교재자료 현황을 보면 ‘도시’의 경우 ‘보드(판넬)’와 ‘팜플렛’, ‘비디오 테이프’를 보유한 보건소가 각각 10개소(83.3%)로 가장 많았으며 ‘농촌’의 경우 ‘보드(판넬)’와 ‘팜플렛’을 보유한 보건소가 각각 10개소(88.9%)로 가장 높았다.
7) 흡연예방 및 금연교육대상을 보면 ‘도시’의 경우 초등학생을 대상으로 100% 실시하였으며 노인과 임산부를 대상으로 각각 83.3%에서 실시하였다. ‘농촌’의 경우 초등학생을 대상으로 100% 실시하였으며 흡연학생을 대상으로 88.9%에서 실시하였다.
8) 흡연예방 및 금연사업평가 현황을 보면 ‘도시’의 경우 6개소로 50%에서 평가를 실시하였다. ‘농촌’의 경우 28개소로 51.9%에서 평가를 실시하였으며, 내부 및 외부 평가 시행 비율을 살펴보면 ‘도시’의 경우 ‘내부평가’와 ‘외부평가’를 받은 보건소는 각각 12개소로 100%였다. ‘농촌’의 경우 ‘내부평가’와 ‘외부평가’를 받은 보건소는 각각 52개소로 96.3%였다.
라. 지난 2년간(2002~2003년) 영양사업 설문조사 결과
1) 보건소에서 수행했거나 수행하고 있는 영양사업 수행 분야로 ‘식단 전시회 및 시식회’ 60.6%, ‘영양상담’ 57.6%, ‘성인 영양교육’ 57.6%, ‘만성퇴행성질환자에 대한 영양진단 및 상담’ 51.5%이었다.
2) 수행을 위해 보유하고 있는 장비는 ‘노트북’ 72.2%, ‘빔 프로젝트’63.6%, ‘OHP’ 60.6%, ‘교육용 모형’ 60.6%이었다.
3) 영양사업의 예산 중 1순위로 소요한 사업은 ‘전시회 및 시식회’33.3%, ‘유아 영양교육교재 개발비’ 15.2%, ‘영양캠프’ 15.2%이었다.
4) 영양사업의 대상으로 ‘노인’ 인구층이 전체 보건소에서 57.6%로 가장 높게 실시했으며 ‘취학 전 아동’ 51.5%, ‘성인(장년층)’ 48.5% 순이었다. 그러나 ‘도시형’ 보건소에서는 ‘성인(장년층)’대상이 83.3% 로 가장 높아 ‘농촌형’ 보건소 40.5%에 비해 유의하게 높았다.
5) 영양사업에 대한 홍보활동방법으로 ‘지역신문’ 48.5%, ‘리플렛’ 48.5%,‘현수막’ 39.4%, ‘팜플렛’ 39.4%, ‘홈페이지 활용’ 36.4%이었다.
6) 영양사업에 참여하는 보건소 내 전담인력 ‘1명’을 둔 보건소는 48.5%이었고, 전담인력이 없는 보건소도 30.3%이었다.
7) 영양사업에 대한 내부적 평가는 ‘1회 평가’ 45.5%이었고, 평가를 하지 않은 보건소도 21.2%이었다.
마. 지난 2년간(2002~2003년) 운동사업 설문조사 결과
1) ‘도시형’에서는 ‘운동상담실’을 운영하는 경우가 가장 많았고 ‘체력단련실 운영’과 ‘생활체육교실 운영’이 그 다음 순이었으며 ‘자가 건강 측정 코너 운영’이 42.9%, ‘체력 진단실 운영’이 40.0%순이였다.
2) 건강증진담당자에게 꼭 필요한 운동사업 내용을 조사한 결과 ‘체력진단실 운영’이 18.9%가 필요하다고 하여 가장 높았고 그 다음으로 ‘운동처방’ 15.6%, ‘건강운동교실’과 ‘체력단련실 운영’이 12.2% , ‘운동부하 검사’가 11.1% 순이였다. ‘자가건강측정코너를 설치 운영’하자는 의견이 7.8%, ‘운동상담실’ 운영이 6.7%, ‘체조교실’과 ‘어린이운동교실 운영’이 각각 5.6% 순이였다.
3) 운동사업의 교육실시 대상자로 ‘지역주민전체’를 대상으로 한 경우가 51.5%로 가장 높았고, ‘노인’을 대상으로 한 경우가 33.3%였으며 ‘비만중년’과 ‘고위험군’이 각각 27.3%와 24.2%, 그 다음으로 ‘비만아동’과 ‘초등학생교육’ 및 ‘건강관리 회원’, 기타 ‘임산부’ 순이였다.
4) ‘생애주기 별로 구분하여 운동사업’을 실시하는 것이 좋다는 견해가 69.7%였고, ‘관련 질병별 운동사업을 실시’하는 견해에서는 ‘관절염과 관련 운동’을 실시하여야 한다는 견해가 36.4%로 가장 높았고, ‘고혈압, 당뇨 및 비만자 운동 프로그램’, ‘기타 임신부를 위한 라마즈 체조교실운영’, ‘요통 극복을 위한 운동사업’의 순이였다.
5) 각 보건소에서 보유 중이거나 사용 중인 운동교육자료는 ‘비디오 테이프’과 ‘리플렛’이 각각 51.5%와 48.5%로 가장 높았고 그 다음으로 ‘보드’와 ‘팜플렛’ 순이였다. 지역주민들에게 운동사업에 대한 홍보방법으로 ‘홈페이지’를 활용하고 있다는 응답률이 51.5%로 가장 높았고 그 다음으로 ‘지역신문’, ‘팜플렛’, ‘건강소식지’, ‘프랜카드’, ‘시정 소식지’ 순이였다.
6) 각 보건소에서 운동관련 행사내용은 42.2%가 ‘건강걷기 대회를 개최’하였다고 응답하였으며, ‘건강축제와 체육대회’, 기타 ‘단축마라톤 대회’ 등의 순이였다.
7) 운동을 위한 교육실시 대상자로 ‘특정질병자’가 39.4%로 가장 높았고, 그 다음으로 ‘비만군’, ‘노인’, ‘일반인’, ‘가정주부’, ‘학생 및 임산부’ 순이였다.
8) 운동사업에 대한 평가 여부조사에서 내부평가를 ‘한 적이 없다’는 경우가 63.6%였고, 33.4%는 ‘내부평가’가 이루어졌으며, ‘외부 평가’는 75.7%가 이루어지지 않았고 24.3%만이 평가를 하였다.
바. 지난 2년간(2002~2003년) 절주사업 설문조사 결과
1) 절주사업 시행 현황을 보면 ‘도시’의 경우 2개소로 16.7%에서 ‘절주사업’을 시행하였고 ‘농촌’의 경우 4개소로 7.7%에서 ‘절주사업’을 시행하였다.
2) 사업의 필요성 순위를 보면 ‘도시’의 경우 1순위는 ‘비디오 상영’과 ‘전문가 초청강의’였고 2순위는 ‘음주예방’, ‘절주교실’과 ‘절주지도자 교육’이었다. ‘농촌’의 경우 1순위는 ‘음주율 조사’였고 2순위는 ‘음주예방’과 ‘절주교실’이었다

1. Survey of health promotion projects according to each public health center.
1) Analysis of distribution of public health center according to each health promotion projects.
The public health center of performing only one core projects is smoking prevention and cessation at 17.9%, exercise service at13.7%, nutrition service at 8.4%, reducing alcohol intake service at 3.2%. The number of overlapping project of performing in public health center is two projects at 28.4%, three projects at 5.3%, four projects at 8.4%.
2) The number of type according to each health promotion projects.
The number of type of health promotion projects of performing public health center is smoking prevention and cessation at 19, exercise service at 10, nutrition service at 29, reducing alcohol intake service at 10.
2. Opinion of health promotion service of public health center
1) Most respondents at 51.5% reported that the first thing to improve in public health centers was to supplement professional staff, followed by supplement facilities and equipments at 21.2%.
2) The type of health promotion projects in public health centers is "average" at 48.5%. There were more "men" at 64.3%, compared with "women". According to the period of performing current work, most was "less than 1 year".
3) The level of interest by heads of local government on health promotion projects was "average" at 45.5%. According to positions, the highest response came from "public health positions". According to the level of position, "grade 5 government employees" had the most interest at 58.3%.
4) On the need of education according to each public healthprojects on health promotion, most at 54.5% felt the need for health promotion projects at 54.5%, followed by "absolute need" at 39.4%, showing the most respondents felt the need for education. According to age groups, those respondents in their 40's showed the highest rate of response at 50.0%.
5) On the view of making public health training public to effectively perform public health projects by all public health workers, most at 42.4% responded that it was "a good idea", followed by "a very good idea" at 36.4%. According to positions, "administrative workers" recognized felt that making public health training public a good idea.
6) Most respondents at 42.4% felt that the most important project among currently held public health projects was on "quitting smoking". According to positions, 66.7% of nurses preferred projects on quitting smoking, followed by "grade 6 government employees". According to the period of conducting current work, those who worked in their jobs for less than a year preferred projects over quitting smoking.
7) On the view of evaluating health promotion projects, most at 57.6% felt that the evaluation is absolutely needed. According to genders, "men" had the stronger view.
3. Opinion of education about health promotion personnel
1) The number of receiving education related with health promotion projects was "once" in most cases at 60.0%. According to position, "nurses" received the education/trainingonce. According to the level of position, "grade 6 government employees" received the education/training. According to the period of work, those worked for "more than 20 years' received the training/education once.
2) The number of hours of receiving educationon health promotion projects was "between 5-6 hours in most cases at 42.4%. According to position, "medical personnel" showed a high response rate. According to the period of work, those who worked for "between 5-7 years" showed a high response rate.
3) On the appropriateness of education materials on health promotion projects, most responded that the materials were "average".
4) On the need of developing guidelines or programs on public health education, most at 36.4% responded that it is "absolutely needed".
5) On the need for public health equation on health promotion according to age groups, most at 27.3% felt the need for education in "adolescent period".
6) On the public health materials lacking when performing health promotion projects, most at 39.4% felt that these materials are lacking on "reducing alcohol intake".
7) On the materials effective for publicity, most at 57.6% felt that "videotapes" are most effective.
8) On the area of ability of workers lacking when performing health promotion projects, most felt that these workers lack "planning ability for public health education". According to age groups, those people in "their 40's" showed a high response rate. According to the period of work, those who worked "between 7-10 years" showed a high response rate. According to the total period of working at public health centers, those who worked "between 15-20 years" showed a high response rate.
9) On the number item of improving when performing health promotion projects, most responded that "lack of ability on executing work by workers".
10) According to age groups, "those in their 50's" showed a high response rate. According to the period of work, those who worked for "less than a year" showed a high response rate.
4. Smoking services
1) Among 66 public health centers surveyed, most (64 centers) at 97.0% performed the projects on smoking prevention and cessation for the past 2 years (2002-2003).
2) As for the content of these projects for the past 2 years (2002-2003), city public health centers invited professional speakers (83.3%) and performed smoking cessation projects (83.3%) in most cases. Publicity materials were prepared and distributed by public health centers at 66.7%.
3) As for the size of budget for each project, the number one project involved the "preparation and distribution of public publicity materials", followed by giving "speeches by inviting professional speakers" and "smoking cessation projects" in the case of city public health centers. In the case of rural public health centers, the number one project involved the "investigation of smoking rate", followed by "smoking cessation projects".
4) As for the status of owing equipments related with smoking prevention, most city public health centers at 66.7% had notebook computers and educational models(8 centers each). Most rural public health centers at 81.5% were equipped with notebook computers and OHP (48 centers each).
5) As for the status of education materials on smoking prevention and cessation, most city public health centers at 83.3% were equipped with boards (panel), pamphlets, and videotapes (10 centers each). Most rural public health centers at 88.9% were equipped with boards (panel) and pamphlets (10 centers each).
6) As for the status of evaluating smoking prevention and cessation programs for the past 2 years, 6 (50%) of the city public health centers and 28 (51.9%) of rural public health centers performed evaluation.
5. Nutrition Service
1) As for the projects on nutrition had held or were being by public health centers for the past 2 years (2002-2003), the number one project was displaying menus and taste testings at 60.6%, followed by nutrition counseling at 57.6%, adult nutrition education at 57.6%, and nutrition check-up and counseling for people with chronic degenerative diseases at 51.5%.
2) As for the type of equipments that public health centers had related with performing nutrition projects, most were notebook computers at 72.2%, followed by beam projection at 63.6%, OHP at 60.6%, and educational model at 60.6%.
3) The nutrition project that costed the most for the past 2 years (2002-2003) was displaying menus and taste testings at 33.3%, followed by the cost for developing educational materials on infant/children nutrition at 15.2%, and nutrition camp at 15.2%.
4) The number one group of candidates who underwent nutrition projects for the past 2 years (2002-2003) were the elderly at 57.6%, followed by non-school age children at 51.5%, and adults (middle aged people) at 48.5%. However, the number one group of candidates in city public health centers was adults (middle aged people) at 83.3%, which was significantly high compared with 40.5% in rural public health centers.
5) The publicity on nutritional projects by public health centers for the past 2 years (2002-2003) was transmitted the most through local newspapers at 48.5%, followed by through leaflets at 48.5%, placards at 39.4%, pamphlets at 39.4%, and homepages at 36.4%.
6) The rate of public health centers that had a worker who only handled nutrition projects for the past 2 years (2002-2003) was 48.5%, whereas 30.3% had no such personnel.
6. Exercise Service
1) Most public health centers in cities operated exercise counseling centers, followed by exercise centers and daily gymnastic centers. Among the centers surveyed, 42.9% operated a self-health check-up center and 40.0% operated physical consulting room.
