1. The goal and necessity of this study
Many candidate genes and genetic polymorphisms of ADHD have been identified. But to this date, the next step after identifying associated genes has shown little progress, and knowledge about how the identified polymorphisms affect brain structure and neuropsychologic profile of ADHD patients is limited. Recently, a series of studies suggesting that environmental toxins, especially endocrine disruptors(ED) are involved in the pathophysiology of ADHD have been published. The harm of EDs that affect the development of the neurotransmitter system, such as phthalate and bisphenol-A (BPA) are being emphasized, but there are no studies investigating the effect of these substances on the pathophysiology of ADHD. The aim of this study is to identify how ADHD associated genes and EDs affect the brain imaging findings and treatment response/prognosis of ADHD patients, by using a longitudinal follow-up design and brain imaging, neurophysiologic, and neuropsychological methodology.
Ⅱ. The contents and range of the study
1. Genetic polymorphisms association study 2. Endocrine disruptors effect study 3. Imaging genetics and brain imaging study about endocrine disruptors 4. Genetic-neurophysiological study 5. Gene-environment interaction study 6.Treatment response and prognosis predictive indicators
Ⅲ. Results of the study
As a result of genetics association studies using the ADHD family trio data, genes including dopamine and norepinephrine system genes, glutamate receptor and transporter genes, neurotrophic factor genes were associated with ADHD. We collected neuropsychological, brain imaging, and medication response data from the ADHD patients, and investigated the difference in DTI according the NE genotype, cognitive function and emotional characteristics according to glutamate genotype, treatment response according to NE genotype. We analyzed the correlation between the urine phthalate concentration and behavioral, neuropsychological characteristics, and also urine phthalate concentration and cortical thickness.
Ⅳ. Application of the study results
MRI/DTI results can be used as endophenotypes in further imaging genetic studies. Neuropsychological test-brain imaging-genetic polymorphisms can be used to develop biomarkers in other psychiatric disorder studies. The methodology using brain imaging and neuropsychological tests to identify how endocrine disruptors affect ADHD pathophysiology can be applied to other neurodevelopmental disorders and act as a novel study model.
