약물감시연구사업단은 서울대학교 의과대학 박병주 교수를 사업단장으로 하여 2009년 6월 24일 출범하였다. 약물감시연구사업단은 안전한 의약품 사용을 도모하기 위하여 국가적인 약물감시체계를 구축하는 것을 목표로 한다. 세부적으로는 자발적부작용보고를 활성화하고, 약물역학연구를 수행하며, 교육 및 홍보를 촉진하고, 약물감시분야에 있어 국제적인 협력체계를 구축하고자 하였다.
약물감시연구사업단은 네 개의 중단위와 그 아래 28개의 세부과제로 이루어져 있으며, 자문위원회, 운영위원회, 및 사무국을 별도로 운영하였다.

약물감시연구사업단은 네 개의 중단위와 그 아래 28개의 세부과제로 이루어져 있으며, 자문위원회, 운영위원회, 및 사무국을 별도로 운영하였다.
1중단위는 연세대학교 의과대학 홍천수 교수가 책임을 맡고 있으며 약물감시활성화연구를 수행하였다. 그 아래 1-1세부과제는 지역약물감시센터의 평가 및 관리방안 개발연구를 맡았으며, 나머지 세부과제들은 전국 각 지역의 지역약물감시센터들로 이루어져 있다. 2010년까지는 15개의 지역약물감시센터가 운영되었고, 2011년에 들어오면서는 5개의 지역 약물감시센터가 추가로 지정되어 총 20개의 지역약물감시센터가 운영되고 있다. 각 지역 약물감시센터에서는 원내 및 관할 지역의 약물유해사례를 모니터링하고 수집된 유해사례를 대상으로 인과관계를 평가한 후 식약청에 보고하고, 취약계층(노인, 소아)과 5개국 이상 시판금지약물에 대한 집중모니터링을 수행하는 한편, 전담인력을 두어 약물유해사례에 대한 상담을 제공하며, 지역약물감시센터 내부인력 및 외부인력을 대상으로 교육 및 홍보를 실시하였다.
2중단위는 서울대학교 의과대학 박병주 교수가 책임을 맡고 있으며, 약물역학 및 분석 연구를 수행하였다. 총 세 개의 세부과제로 구성되어 있는데 2-1세부과제는 약물역학연구 방법론 및 실마리정보 검색프로그램 개발연구를 맡았고, 2-2세부과제는 병원 EMR자료를 이용한 실마리정보 검색연구를 맡았으며, 2-3세부과제에서는 심평원자료를 이용한 실마리정보 검색방법 개발 및 적용방안을 연구하였다.
3중단위는 울산대학교 의과대학 김영식 교수가 책임을 맡았으며 교육 및 홍보 컨텐츠 개발연구를 수행하였다. 총 두 개의 세부과제로 구성되었는데 3-1세부과제는 전문가대상 교육프로그램 개발연구를 맡았고, 3-2세부과제는 일반인대상 교육프로그램 개발연구를 맡았다.
4중단위는 동국대학교 의과대학 이진호 교수가 책임을 맡았으며 국제협력 및 용어표준화 연구를 수행하였다. 총 두 개의 세부과제로 구성되었는데 4-1세부과제는 약물감시 국제네트워크 구축 및 피드백 구조 분석연구를 맡았고, 4-2세부과제는 약물감시 용어표준화 연구를 맡았다.
사업단에서는 운영위원회를 구성하여 사업단 운영일정 및 운영방안에 대해 논의하였으며, 분기마다 진도보고회를 개최하여 중단위별 발표를 통해 연구진행상황을 확인하고 중단위간 연계를 도모하였다. 또한 사무국을 두어 사업단의 운영 및 연구수행에 필요한 행정지원을 담당하도록 하였다.
또한 사업단 홈페이지를 개발하여 각 지역약물감시센터 연구자들이 홈페이지를 통해 유해사례를 보고하고 인과성을 평가할 수 있도록 하였다. 사업단 홈페이지로 보고되고 인과성 평가를 거친 유해사례 보고건은 식약청 서버로 자동 보고되는 시스템을 구축하였다. 그 밖에도 홈페이지를 통해 사업단 세부과제들의 진도관리 및 평가가 가능하며 약물감시 가이드라인 및 국외현황 등에 대한 자료를 제공하였고, 이를 통해 각 세부과제를 연계하고 정보를 공유할 수 있도록 하였다.
각 지역약물감시센터의 업무를 표준화하고 새로 추가 지정된 지역약물감시센터에서 사업단 연구를 수행하는 데 편의를 제공하기 위해 사업단홈페이지 이용길라잡이, 약물부작용 상담지침서, 지역약물감시센터 표준업무기준서 등을 개발하여 배포하였다.
두 달에 한 번씩 사업단 뉴스레터를 발간하여 식약청 소식과 약물유해반응 증례, 사업단 보고실적 및 소식, 해외동향 등을 소개하고, 이를 각 지역약물감시센터 뿐 아니라 협력병의원 및 약국들에도 배포하여 약물감시에 대한 홍보도 겸하였다.
2009년 6월 30일 약물감시연구사업단 출범 이후 유해사례 신고건수는 크게 증가하여 사업단 보고건이 2009년에 26,827건, 2010년에 36,458건에 이르렀으며, 2011년에는 52,836건이 수집되었다. 약물부작용 관련 상담실적은 2010년에 원내와 지역을 합해 3,972건이었고, 2011년에는 5,759건으로 증가하였다. 약물유해반응 감시 및 자발적 보고에 관한 교육 활동은 2009년에 총 205회, 2010년에 578회, 2011년에 762회로 역시 증가하는 것을 확인할 수 있었다.
지역약물감시센터에서 식약청으로 보고한 Ezdrug 자료는 데이터클리닝 작업을 완료하여 실마리정보를 검색하는 데 활용 가능하도록 하였다. 다빈도로 보고된 의약품 중 안전성 이슈가 있는 요오드계 조영제, 트라마돌, 세프트리악손 등을 연구대상약물로 선정하고, 중대한 약물유해반응으로 아나필락시스를 선정하여 실마리정보를 검색하였다. 이를 통해 알려지지 않았던 여러 유해사례를 파악할 수 있었다.
또한, 식약청 등에서 실마리정보를 검색할 때 전공자가 아니어도 쉽게 사용할 수 있도록 실마리정보 검색프로그램을 개발하여 특허등록을 완료하였다. 심평원자료를 이용해 composite data-mining index를 개발하여 기존의 Proportional Claiming Ratio (PCR), Claiming Odds Ratio (COR), Information Component (IC), Relative risk (RR) 등과 검색 결과를 비교해 보았다. 본 사업단에서 개발한 composite index는 민감도도 떨어지지 않으면서 특이도도 다른 지표와 견줄만 하여 가장 좋은 검색능력을 보여 주었다.
약물감시 교육 및 홍보를 위해서 전문가 약물감시교육프로그램을 개발하고 전문인력양성을 위한 워크숍과 심포지엄을 개최하였다. 일반인을 대상으로 하여서는 CF 동영상, 포스터, 배너광고 등을 개발하고 리플렛, 책갈피, 탁상달력 등을 만들어 배포하는 한편 2011년에는 UCC 공모전을 개최하여 최우수작과 우수작을 선정하여 포상함으로써 소비자들의 관심을 끌고 홍보효과도 낼 수 있게 하였다.
바람직한 의약품 안전성정보의 피드백구조를 지역사회 측면, 지역약물감시센터 측면, 식약청 측면으로 나누어 제안하였다. 유해사례 보고건의 신뢰성을 제고하기 위해 데이터가 갖추어야 할 요건을 조사하였고, 실제 지역약물감시센터에서 보고한 유해사례건의 질을 평가하였다. 국제 약물감시네트워크의 가능성을 타진하기 위해 아시아약물감시네트워크를 구성하여 기획안을 작성하고 아시아 국가들 간 공동연구를 수행하였다.
약물감시 용어를 표준화하기 위해 WHO-ART 092버전을 한글로 번역하고 MedDRA와 비교하여 호환성을 검토하였다. 번역된 WHO-ART 092버전은 활용집과 함께 각 지역 약물감시센터에 배포하였고, 알기 쉬운 부작용 용어집도 개발하였다. 이와 함께 자문창구를 통해 지속적으로 부작용 코딩 과정을 지원하고 홈페이지 및 뉴스레터 등을 통해 부작용 용어를 교육, 홍보하여 보고의 질을 높이고자 하였다.
이러한 활동들을 통해 본 약물감시연구사업단은 포괄적 국가약물감시체계 구축과 약물 감시 활성화 기반 마련에 크게 기여하였다.

PharmcoVigilance Research Network (PVNet) was established on June 24th, 2009, and Professor Byung-Joo Park at Seoul National University College of Medicine was appointed as a Chairperson of the Network. The main objective of PVNet was to construct the comprehensive national pharmacovigilance system which could ensure the safe use of drugs in Korea. In detail, this network was aimed at activating spontaneous reporting for adverse drug events, conducting researches in pharmacoepidemiology, promoting education and publicity for the issue of drug safety, and establishing an international collaborating pharmacovigilance system.
PVNet was consisted of 4 units and 28 subunits as well as the advisory committee, steering committee, and executive office.
Professor Hong, Chein-Soo (Yonsei University College of Medicine) was in charge of Unit 1, and the unit was assigned for pharmacovigilance activities. The mission of Subunit 1-1 was to develop the method for assessing and managing Regional PharmacoVigilance Centers(RPVCs). Other subunits of Unit 1 were 15 RPVCs, which were set up the system in order to monitor the pharmacovigilance programme in Korea. The number of RPVCs was increased from 15 to 20 in the year 2011. Each RPVC took part in adverse events monitoring, causality assessment between adverse events and specific drug, reporting to the KFDA, intensive monitoring for special population who were vulnerable such as children and the elderly, and for drugs that were withdrawn in more than 5 countries, providing counseling services for adverse events, and also providing education and publicity for safe drug use.
Professor Park, Byung-Joo (Seoul National University College of Medicine) was in charge of Unit 2, the unit was assigned for pharmacoepidemiology and analysis studies. Unit 2 was consisted of 3 subunits. Subunit 2-1 was in charge of developing the methodology for pharmacoepidemiology study and the signal detection software program, Subunit 2-2 and Subunit 2-3 were conducted to develop signal detection program using EMR database and HIRA claims database, respectively.
Professor Kim, Young-Sik (Ulsan University College of Medicine) was in charge of Unit 3 which was assigned for developing educational materials for pharmacovigilance. Subunit 3-1 targeted expert groups as the subjects for education and subunit 3-2 targeted general population.
Professor Lee, Jin-Ho (Dong-guk University College of Medicine) was in charge of Unit 4 which was assigned for international collaboration for drug safety and adverse event terminology standardization. In detail, the main goal of subunit 4-1 was to establish the international pharmacoepidemiology network and the feedback assessment of the network, and the objective of Subunit 4-2 was to standardize adverse drug reaction reporting terminology.
The steering committee was convened to discuss the operating schedules and plans for PVNet. And regular workshops were held once a quarter, for checking each project phases and promoting cooperation between 4 Units by presenting on their own project phase. In addition, PVNet had run for the executive office to support operations and researches in the network.
PVNet developed their own website for RPVCs to report AEs and assess causality of AE. The AEs reported to website were automatically transmitted to KFDA Ezdrug database. In addition, PVNet website enabled to manage the research of subunits and to provide pharmacovigilance guidelines and current drug safety situation in other countries, in turn, these lead to develop a collaborating system and helps to share the relevant information.
The guideline for PVNet’s website, the counseling protocol for drug side effects and the standard operating procedures(SOP) for RPVCs were developed and sent to each RPVC. The objective of the guideline was to standardize all operations in each RPV center and contribute to the convenience of newly-appointed RPV centers.
The newsletter of PVNet has been published every two months. It contained the news from KFDA, case reports for specific adverse drug reaction, PVNet’s statistics, international pharmacovigilance affairs, etc,. It was sent to not only each RPVC, but also collaborating hospitals and pharmacies for promotion.
As a result of PVNet's activities, the number of adverse drug event reports was significantly increased from 26,827 reports in 2009 to 36,458 in 2010, and 52,836 in 2011. The records of side effect counseling were increased from 3,972 in 2010 to 5,759 in 2011. The frequency of education for pharmacovigilance and spontaneous reporting was also increased from 205 times in 2009 to 578 in 2010, and 762 in 2011.
After collecting the Ezdrug data from RPVCs, we conducted data cleaning. Consequently, the data could become available for signal detection. Among frequently reported drugs, iodine contrast media, tramadol, ceftriaxone, etc. were chosen as target drugs for data mining. In addition to those drugs, anaphylaxis as a target serious adverse drug reaction was also selected. The data mining for these drugs and adverse drug reaction enabled to detect signals and to identify unexpected adverse events.
And also, we developed the signal detection program which could be handled by not only experts but also non-experts. In addition to using HIRA databases, we developed the composite data-mining index and compared the result with other signal indices such as Proportional Claiming Ratio(PCR), Claiming Odds Ratio(COR), Information Component(IC), and Relative risk(RR). The composite index has been proven as having not only better sensitivity, but also comparable specificity.
For the purpose of education and publicity on pharmacovigilance and pharmacoepidemiology, we developed educational programs for healthcare professionals, and held workshops and symposia. For the public, CF, posters, and internet banners were developed and leaflets, bookmarks, and table calendar were also develpoed and distributed. In addition to that, we held the UCC contest in 2011 for enhancing the advertising effect.
We developed the feedback system for drug safety in three prospectives, local society, RPVCs and KFDA. We investigated the data requisite to promote reliability of reports and evaluated the quality of reports from RPVCs. For realizing the international pharmacovigilance network, we organized the Asian Pharmacoepidemiology Network(AsPEN) and conducted collaborative researches among participating countries.
In order to standardize the pharmacovigilance terminology, WHO-ART 092 version was translated into Korean and it was compared with MedDRA. The translated WHO-ART 092 version was distributed to each RPVC with the layout, and we also developed easy Korean glossary for adverse drug reaction. In addition to that, through an e-mail consulting program, we have been supporting to solve the problems occurring during the coding process for adverse events.
In conclusion, PVNet has been contributed to establish comprehensive national pharmacovigilance system in Korea.
