연구목적 :
∘ 국내의 현실에 적합한 가정완화의료 및 완화케어팀의 모형 개발

연구방법 :

∘ 우리나라와 임종과 관련된 문화 및 의료보험 제도가 비슷하지만 앞서서 호스피스∙ 완화의료를 실시하고 있는 일본에 대한 현지조사를 통해 가정완화의료 및 완화케어팀 서비스의 제공 현황을 파악하고, 제도와 수가 운영의 제한점 및 개선방향을 탐색함.
∘ 43개 완화의료 전문기관과 144개 완화의료 비전문 기관을 대상으로 전화 설문조사를 통해 가정완화의료 서비스 제공 여부를 확인함. 가정완화의료를 제공하고 있다고 응답한 34개 완화의료 전문기관과 63개 완화의료 비전문기관을 대상으로 서면 설문조사를 시행하여 서비스 내용과 운영 실태를 파악함.
∘ 가정완화의료 수요조사를 위해서 완화의료 전문기관 중 환자 수 상위 7개 기관과 국립암센터 및 지역 암센터 12개 기관에서 재가 또는 입원 중인 말기 및 진행성 암환자 470명에게 서면동의를 취득한 후 가정완화의료 서비스에 대한 요구도 설문조사를 시행함.
∘ 암전문의들의 가정완화의료에 대한 요구도 및 가정완화의료 의뢰 가능성을 파악하기 위해 대한암학회에 등록된 암전문의 940명을 대상으로 이메일로 설문조사를 시행함.
∘ 보건복지부 지정 완화의료전문기관 43개 기관과 암환자 등록사업에 참여하고 있는 기관 중 지정된 완화의료전문기관을 제외한 148개 기관을 대상으로 완화케어팀 서비스 제공의 실태를 조사함.
∘ 가정완화의료 표준지침 마련을 위해 가정완화의료를 시행하고 있는 1,2,3차 병원을 포함한 7개 기관을 선정하여 가정완화의료 실무담당자에게 가정완화의료 서비스 운영과 제공 과정에 관한 반구조화된 조사지를 작성하게 한 후 포커스그룹 인터뷰를 시행함. 포커스그룹 인터뷰에서 드러난 가정완화의료 서비스를 위해 필요한 사항과 운영상의 문제점 및 개선방안을 기초로 미국, 대만, 일본, 캐나다, 싱가포르에서 제시하고 있는 가정완화의료 서비스 표준을 검토한 후 가정완화의료 표준 안을 작성하고, 3차에 걸친 전문가 델파이 설문을 통해 서비스 표준안을 개발함.
∘ 수도권의 주요 암환자 진료병원 7개와 전국의 국립 및 지역암센터 11개를 선정하여 해당 병원에서 일반병동의 말기암환자에 대한 완화의료를 제공하고 있는 담당자들을 대상으로 심층면접조사를 진행하였으며 각 병원에서의 완화케어팀 활동의 현황을 조사하고 완화의료팀 활동의 필요성과 제도화 방향에 대한 의견 청취함.
∘ 개발된 가정완화의료 서비스 표준에 근거하여 시범사업에 참가를 희망한 3개 기관에서 3개월 간 가정완화의료 시범사업을 시행함. 시범사업을 통해 서비스 내용, 빈도, 소요시간과 비용을 조사하고, 서비스 표준의 실현 가능성, 서비스 효과와 효율성을 평가함.
∘ 완화케어팀 서비스 실현가능성 평가 및 수가개발 근거자료 수집을 위해 공모에 의해 선정된 5개 기관에서 시범사업을 시행하였으며 서비스 표준지침의 필수항목 이행수준 확인, 서비스 제공 내역 확인에 필요한 제출 양식의 효과 평가, 제공된 서비스에 대한 이용자와 제공자의 만족도 및 실현 가능성 평가를 수행함.
연구결과 :
∘ 일본은 가정완화의료를 위한 방문수가 체계와 인프라를 갖추고 있음에도 가정완화 의료 이용은 높지 않았음. 그 원인으로는 1) 지역에 있는 의사 및 간호사의 완화의료 마약성 진통제 처방에 대한 지식 및 경험 부족, 2) 환자와 보호자의 인식 부족과 집에서의 간병 문제, 3) 지역 암 제휴 거점 병원과 연계 및 의사소통의 어려움이 있었음.
∘ 일본 현지조사를 통해 완화케어팀 제도의 조기 정착을 위해서는 수가의 가산 외에도 “암 진료 제휴 거점 병원”의 지정요건으로 완화케어팀 설치를 의무화한 바와 같이 비수가정책도 병행하여야만 함을 확인할 수 있었음.
∘ 암전문의들을 대상으로 시행한 가정완화의료 요구도 조사에서 4.2%의 응답률에 불과하였으나, 응답자의 97.4%가 가정완화의료 서비스가 제도화된다면 이용하겠다고 응답함. 따라서 가정완화의료 제도화와 함께 암전문의들에게 가정완화의료에 대한 홍보를 통해 인식을 높이는 것이 필요함.
∘ 환자들을 대상으로 시행한 가정완화의료 요구도 조사에서 가정에서 지내기를 원하는 경우는 75.9%였으며, 전체 89.1%가 가정완화의료 서비스를 이용할 의향이 있다고 응답하여 가정완화의료에 대한 요구도가 높은 것으로 파악됨.
∘ 현재 국내에서 PCU가 설치되지 않은 의료기관에서 완화의료를 담당하는 주요 인력은 간호사였으며 환자와 보호자에 대한 상담과 전원에 대한 중재가 주된 업무였으며 완화의료 제공에 전담하여 참여하는 사회복지사는 없었음. 완화케어팀 업무에만 종사하는 의사가 배치된 기관은 없었으며 기관 내에서 다른 직무와 병행하여 겸임하고 있었으나 참여도는 대부분의 기관에서 낮았음.
∘ 가정완화의료 시범사업에서 직종별 방문 횟수는 전담인력인 간호사가 평균 5.6회로 가장 많았으며, 의사는 평균 0.4회, 사회복지사는 평균 0.7회로 나타남. 개별 방문마다 39.0%에서 약물 처방의 변경이 발생하였으며, 32.5%에서 자원 활용과 지원이 이루어짐으로써, 의사나 사회복지사는 방문보다는 팀 회의 및 전화 연락을 통해 돌봄 계획 수립과 처방 변경 및 자원 연결에 참여하는 것으로 나타남.
∘ 방문 당 서비스제공 시간은 방문의 목적에 따라서 최소 20분에서 최대 210분으로 다양하였으며, 평균 서비스제공 시간은 71.9분으로 가정간호에 비해 길었음. 가정완화의료 방문 시 많은 시간이 소요되는 것은 환자, 가족, 환경에 대한 평가와 환자/가족의 상담과 교육에 더 오랜 시간이 요구되기 때문인 것으로 파악됨.
∘ 시범사업에서 가정완화의료 서비스를 제공받은 환자와 가족들은 사전평가와 사후 평가를 비교하였을 때 모든 서비스 항목에 대해 높은 만족도의 향상을 나타냄.
∘ 43개 완화의료전문기관을 대상으로 시행한 조사에서 한 시점에서 호스피스∙완화의료 병동에 입원한 환자 중 퇴원이 가능한 환자의 비율은 28.3%로 파악되었고, 퇴원이 불가능한 이유는 자가 돌봄 불가능 63.8%, 수액공급 필요 59.7%, 거동 불가능 51.0%, 돌봄 장비 없음 49.7%, 돌봄제공자 없음 36.2%, 사보험금 지급이 있음 18.8%, 퇴원 후 불안감 6.0% 이었음. 24시간 주7일 연속적으로 신체적, 심리적, 사회적, 영적으로 완화적 돌봄을 제공하는 가정완화의료 서비스를 통해서 말기암환자의 퇴원율을 높일 수 있으므로 의료비용 절감의 효과가 있을 것으로 기대됨.
∘ 완화케어팀의 시범사업을 통해 확인한 환자별 서비스 제공기간은 평균 11.27일이었으며, 총 서비스 제공시간은 간호사가 평균 237.44분이었고, 의사는 89.81분, 사회복지사는 83.39분이었음.
∘ 완화케어팀 서비스를 이용한 환자 중 가정으로 퇴원한 환자 중 55%와 계속 입원하거나 전원된 환자 중 62%가 완화의료서비스 체계(완화의료병동, 외래 완화의료 클리닉, 가정간호를 이용한 완화의료) 내로 지속적인 연계를 보였으며 이는 일반적인 의료이용 경로와 비교 시 매우 높은 비율로 추정할 수 있음.
∘ 시범사업을 통해 완화케어팀의 서비스에 대한 서비스 이용자(환자 및 보호자)와 의뢰의사의 공통된 높은 만족도를 확인할 수 있었음.
∘ 완화의료병동이 설치된 의료기관에서의 완화케어팀 활동은 환자들의 다양한 요구에 좀 더 부응할 수 있는 기회가 될 수 있으나 전담간호사 및 사회복지사의 확보가 선행되지 않을 경우 두 영역의 서비스의 질이 모두 저하될 위험이 있음.
∘ 시범사업을 통해 향후 완화케어팀 제도의 도입과 보급을 위해서는 환자를 의뢰할 의사들의 완화케어팀 제도에 대한 충분한 이해를 증진시킬 수 있는 교육과 함께 기관 내에서 완화케어팀의 노력이 필요하며 암진료 의사의 완화케어팀 활동에 대한 신뢰부족이 완화케어팀 제도의 확산에 가장 중요한 장애요인이 될 수 있음이 확인됨.
∘ 완화의료전문가인 완화의료전문의료기관 종사자들에 대한 의견 조회 결과 인력기준 및 수가가산의 방식, 완화의료팀 설치 의무화 등 주요 사안에 대해 전반적인 동의가 확인됨.
기대효과 :
∘ 가정완화의료와 자문형 완화케어팀 서비스 제공의 모형과 서비스 표준안의 개발을 통해 가정완화의료와 완화케어팀 제도의 도입이 말기암환자 및 암환자 진료의사의 완화의료에 대한 수용성을 증대시키고, 완화의료 전문기관의 효율적 운영 및 의료기관에서 완화의료 서비스 제공을 확대시킬 수 있는 체계를 마련함.

The main objectives of this study are as follows:
- To develop a model of home hospice and palliative care team which will be appropriate for Korean situation
- To confirm the feasibility of the developed service model and to develop the service standard through the implementation of the pilot project
Study Methods
∘ Studied the current state of Home Hospice and Palliative Care Team service in Japan and explored relevant regulations, restrictions of medical charge and improvements, through the filed work in Japan which has similar dying culture and health insurance system but implemented HPCS system earlier.
∘ Confirmed whether home hospice provided, through the telephone poll of 43 HPCS centers and 144 non-HPCS centers. Studied the service contents and process of home hospice through the written survey of 34 HPCS centers and 63 non-HPCS center who confirmed home hospice at the first telephone poll.
∘ Conducted the survey of 470 advanced or terminal cancer in- or out-patients, who agreed to the survey in writing, from top 7 hospitals and 12 regional cancer centers, in order to research the needs of home hospice.
∘ Conducted the email survey of 940 oncologists registered with Korean Cancer Association, in order to study the needs of home hospice and their possibility of request home hospice.
∘ Conducted surveys to determine current status of palliative care team.(148 medical centers joining cancer patient registry project and 43 medical centers designated as Palliative Care Specialist Agencies for Terminal Cancer Patients)
∘ Interviewed a focus group after we had the persons in charge to draw up the half-structured questionnaire on the home hospice team and process of home hospice , who came from 7 medical centers selected among 1st, 2nd, 3rd level hospitals that run home hospice program for standard development.
Developed home hospice standard through 3-time Delphi surveys, after reviewing home hospice standards of US, Taiwan, Japan, Canada and Singapore, based on requirement, operational problems and improvement plan which were found out during the focus group interview.
∘ Performed in-depth interviews with managers in charge of palliative care service department for terminal cancer patients in the general ward of 7 major cancer hospitals in Seoul metropolitan area and 11 national and regional cancer centers, studied PCT's current activities in the hospitals and collected their opinions on the necessity of institutionalization of PCT.
∘ Implemented in three medical centers pilot projects based on the home hospice standard which it took three months to develop. Researched the contents, frequency, service period, and cost of home hospice through such pilot projects and evaluated the feasibility of the service standard and the service’s effect and efficiency.
∘ Implemented pilot projects in 5 medical centers selected through the public participation for PCT’s feasibility and data collection for medical charge calculation, checked out the level of the implementation of essential items in the service standard, and evaluated the effect of the service proof template, the satisfaction level of service providers and users and the service’s feasibility.
Study Results
∘ Japan’s use rate of home hospice was not high, even though the medical charge on the visit and infra structure of human resource and facility were set. The reasons were the following: 1) regional doctors and nurses were lack of experience and knowledge on the opioid usage, 2) as patients and guardians were lack of understanding on home hospice, 3) it was difficult to communicate and to be connected to regional cancer centers.
∘ Confirmed the necessity of running parallel with non-medical charge policy, that is, the obligation of PCT set-up as a designation condition of “allied cancer central hospital”, as well as additional medical charge, in order to settle down the PCT system earlier.
∘ 97.4% of responded cancer specialists agreed to the use of home hospice in case that home hospice is institutionalized, even though the total response rate was as low as 4.2%. Therefore, it is needed to increase the understanding on home hospice through the promotion on home hospice system to cancer specialists, as well as the system institutionalization.
∘ 75.9% of surveyed patients wanted to stay at home and 89.1% of patients showed interests in the use of home hospice, which proves there is high demand on the service.
∘ With respect to the visit rates per occupation in the pilot projects, the nurse showed the highest rate because they were dedicated. The rates are average 5.6 times per nurse, average 0.4 per doctor, and average 0.8 per social worker. 39.0% of prescription change per visit happened and as resources and supports were provided in 32.5% of visits, doctors and social workers participated in care planning, prescription change and introduction to resources, by team meeting and telephone communication rather than visiting.
∘ The service time per visit varied from minimum 20 min to maximum 120 min, depending on the visit purpose and the average time was 71.9 min, which was longer than home care service. Home hospice visits took longer time because more time was spent in evaluating patients, their families and environment as well as counseling and educating patients and their families.
∘ Patients and their families who received home hospice in the pilot projects showed high satisfaction in all service areas, compared to the early evaluation.
∘ At that time of the survey on 43 HPCS centers, 28.3% of patients in PCU could be discharged. The reasons not to be discharged were the following:
63.8% impossible self-care, 59.7% fluid needed, 51.0% impossible movement, 49.7% no care equipment, 36.2% no care provider, 18.8% private insurance payment, and 6.0% anxiety after discharge. Expected to save medical expenditure because the discharging rate of terminal cancer patients will be increased by continuously providing physical, psychological, social, and spiritual palliative care for 24 hours and 7 days.
∘ 55% of discharged patients who used PCT service and 62% of continuously hospitalized patients or transferred patients were continuously linked to hospice & palliative care system such as PCU, outpatient palliative care clinic, and home care via visiting nurses, which would be very high rate, compared to the conventional medical system use courses.
∘ Confirmed the commonly high satisfaction level from service users(patients and their guardians) and doctors who requested HPCS, through the pilot project.
∘ Given the pilot projects, in order to introduce and expand PCT system in the future, education program promoting physicians' positive attitudes toward collaboration with PCTs is necessary. Continual collaboration between physicians and palliative care teams will represent a very important contributor.
∘ Confirmed general consensus on manpower criteria, additional medical charge calculation, obligation of PCT set-up, and so on, as a result of survey of stakeholders’ opinions.
Expected Effect
∘ To prepare for a system to increase HPCS acceptance of terminal cancer patients and cancer doctors, to run efficiently HPCS centers and to expand HPCS in medical centers, through developing a home HPC, counseling HPCS Team service model and service standard.
