1) 해외 현황 조사
○ 영국, 미국, 일본, 대만, 싱가포르의 소아완화의료 현황을 문헌 고찰 및 방문을 통해 조사함.
○ 소아완화의료 서비스의 대상을 진단명, 기대여명으로 제한하지 않음.
○ 어린이병원에서 질병 치료와 함께 자문 형태의 완화의료를 통합적으로 운영.

2) 국내 현황 조사
A. 중증소아환자의 역학적 특성 및 의료 이용
- 영국 등지에서 소아완화의료의 대상 규모를 분석하는 데 사용되는 LLC(life-limiting conditions) 진단 코드를 기준으로 국민건강보험청구자료를 분석하여 국내 중증소아환자의 유병과 사망 현황을 파악함.
- 만 24세 이하 중증 질환의 연간 유병 인구는 약 13만 명으로 암환자는 23.6%, 비암 환자는 76.4%임.
유병인구 및 지역 분포는 인구비례를 따르며 소득 수준 및 거주 지역에 따른 차이는 뚜렷하지 않음. 장애진단이 없는 경우가 85.4%에 해당함. 유병 인구의 26.6%가 연간 1회 이상 입원 서비스를 이용하며 평균재원 일수는 26.2일임.

B. 중증소아환자 가족의 돌봄 경험과 완화의료 요구도 조사
- [질적] 13명의 돌봄 및 사별가족을 개별심층면접법을 통해 조사하였고 치료 과정에서 상당한 심리적 스트레스가 있고 의사결정에 대한 지원, 정서적인 공감과 안정의 욕구가 있음이 나타남. 소아청소년 완화 의료에 대한 태도는 긍정적이었으며 부문별로 다양한 니즈가 있었으며 개별적이고 정서적인 지원 욕구가 큼.
- [양적] 상급종합병원 3개소에서 300명의 보호자에게 구조화된 설문지를 통해 조사함. 암환자와 비암 환자에서 보호자가 인식하는 중증도와 신체적・심리적 어려움의 정도에 차이가 있었으며 돌봄을 제공하고 있는 보호자의 삶의 질은 상당히 낮은 것으로 나타남. 의료진과의 소통 및 정보 제공, 간병 등에 대한 니즈가 매우 높았으며, 소아청소년 완화의료 서비스에 대해서는 긍정적인 기대가 있는 것으로 나타남.
C. 소아청소년 분야 전문의 및 완화의료 전문기관의 소아완화의료 현황 및 인식
- 전문의 141명이 응답하였으며 소아청소년과도 사전의료계획 논의가 필요하다는 응답이 많았으며 소아 완화의료에 대한 자신감은 낮고 필요성에 대한 인식이 높았음. 소아청소년 질환의 특성을 고려하여 법에 규정된 질환 외에 신경, 유전, 선천 기형 등 다양한 질환이 추가되는 것이 적절하다고 보고함. 형태에 따라서는 자문, 입원, 가정 서비스에 대한 요구가 모두 높은 것으로 나타남.
- 완화의료 전문기관 61개소에서 응답하였으며 소아청소년 환자에 대해서는 현재 제공하는 기관이 적었으며 향후 계획이 있는 기관도 거의 없었음. 장애요인으로는 훈련된 전문 인력 부족, 완화의료에 대한 부정적 인식, 심리적 어려움 등이 나타남.
3) 소아완화의료 구축 로드맵 및 시범사업 제안
○ 소아청소년 질환의 구성과 특성을 고려하였을 때 소아완화의료의 대상과 개입 시기를 법률에 규정된 4개 질환과 말기로 제한하는 것은 현실적으로 타당하지 않음. 타 국가 및 세계보건기구 기준에서 소아완화의료는 질병의 종류 및 시기에 제한이 없이 제공하도록 정의하고 있으며 국내 환자 가족의 의견 및 전문가 의견도 이와 일치하였음.
○ 대상 질환으로 제한을 두지 않는 것이 적절하며, 질병 단계의 경우에도 의료진 또는 환자・가족이 필요하다고 판단하는 경우에 제공할 수 있도록 하는 것이 필요함. 또한 소아완화의료의 활성화를 위해서는 임종 서비스라는 인식을 탈피하는 것이 중요하므로 용어 사용에 신중함이 요구됨.
○ 소아완화의료의 제공은 다학제 전문가로 구성된 팀이 하도록 하며 상급종합병원, 지역사회 의료기관, 가정에서의 서비스 제공이 체계적으로 연계될 수 있어야 함. 권역별 소아완화의료센터, 완화의료전문기관, 요양기관 등으로 발전 체계를 갖추어 나갈 수 있으며 모형 개발 및 보완을 위해 소수 기관에서 시작하되 소아 완화의료 전문가 교육 개발과 더불어 확산시켜가는 전략이 필요함. 대상 환자의 수가 적어 건강보험수가만으로 유지되기 어려운 부분이 있으므로 모형에 따른 수가 개발을 하되 지원금에 대한 충분한 고려가 필요함.

3. Results
1) Investigation of Current status of Pediatric Palliative Care in Foreign Countries
○ All children with palliative needs deserve pediatric palliative care regardless of diagnosis and/or life expectancy. Pediatric palliative care services are provided children’s hospital based and offered as a consultative services, enabling integrative approach. Community-based services are emphasized considering the family dynamics of children with serious illness.
○ In the early years, private donations, in addition to government subsidies, are a big part, but they are being changed into tax or insurance subsidies for sustainable and stable financial operations.
2) Identifying needs of pediatric palliative care in Korea
A. Epidemiology and health care utilization of children with serious illness
- LLC(life-limiting conditions) codes developed to analyze the epidemiologic data of children who need pediatric palliative care were used. We analyzed national health insurance data in order to how many children had LLC codes and died with those.
- The number of patients with serious illness under 24 years old is about 130,000, of which 23.6% are cancer patients. The regional distribution of the patients was proportional to the population and the differences according to the income level and residence area were not significant. Twenty-seven percent of LLC patients were hospitalized at least once a year and the average length of hospital stay was 26.2 days.
- Every year, approximately 1,300 patients under the age of 24 die with LLC. Of the deaths, 33.8% were cancer patients and 66.2% died because of non-cancer diagnosis.
Fifty-three percent of deceased patients were admitted to a hospital located in Seoul or Gyeonggi province last before death. Ninety-three percent of deceased patients with LLC were hospitalized at least once a year and the average length of hospital stay was 99.2 days.
B. Familial experiences and needs for pediatric palliative care
- Individual in-depth interviews were conducted with 13 family members and 300 parents answered structured questionnaire. Participants reported that both patients and family members experienced significant psychological stress during the treatment and requested support for decision making and emotional distress.
C. Perspectives of pediatric specialists and palliative care providers
- Pediatric specialists recognized it is necessary to discuss treatment plan with children with poor prognosis and they agreed with the need of palliative care. They reported children with various diseases need palliative care even though the law limits the disease category for palliative care.
- In the palliative care institution survey, a few respondents answered they are providing service to children, and the obstacles are the lack of trained workforce, negative perception of palliative care, and psychological difficulties.
3) Recommendation for pediatric palliative pilot project and a road map for implementing pediatric palliative care in Korea
○ Considering the cases of other countries, the criteria of the WHO, the opinions of experts and families, it is problematic to limit the subject and timing of palliative care provision for children according to the law.
○ Palliative care should be provided for all children and their family who suffer from physical, psychological, spiritual distress. Care should be taken in using terminology to reduce negative perceptions of palliative care.
○ Pediatric palliative care should be provided by a team of multi-disciplinary specialists. It should be possible to continuously link the services provided by tertiary hospital, palliative care institution, and community. In order to develop the model suitable for Korea, it is inevitable to start the pilot project with a small number of institutions, but it should be accompanied by development of education and training professionals. Because the number of patients for palliative care is small, financial subsidies for operating expenses in addition to the national health insurance support.
