이 연구는 유전-환경 통합 위험점수(Gene-Environmental Risk Score, GERS)를 개발하고 이를 활용하여 대장암 고위험군 기준을 개발하고, 환경 유전 상호작용을 분자생물학적인 방법으로 분석하여 이를 세계적으로도 선도적인 방법론을 제시함과 동시에 국가 암 관리 정책 구현에 이바지하고자 함.

이를 위해 본 연구는 한국인 일반인구 약 18만 명을 대상으로 후향적 및 전향적 추적으로 확인된 대장암을 대상으로 환경적/유전적 위험요인을 규명하고자 하였음. 대장암과 관련성이 있는 유전위험요인 발굴 및 비만으로 인한 대장암 발생의 유전-환경 상호작용을 위한 후보 유전자군을 선정하기 위해서 코호트 연구를 통해 확보한 대장암 발생 350건과 대조군 994건의 GWAS 자료를 활용, Affymatrix exome array chip을 이용하여 분석, 이를 바탕으로 선정한 대장암 관련 SNP을 선정하여 SNP sequencing을 시행함. 또한 이전 48개 연구논문에서 발표된 대장암 관련 SNP 23개를 선정하여 본 코호트 샘플에 적용하여 분석하였음. 대장암 발생의 유전-환경 상호작용을 규명하여 유전-환경 위험점수(GERS) 방법론을 개발과 모형 평가. 유전-환경 위험점수(GERS)를 한국인의 질병예측모형 시범화를 진행하여 대장암 고위험 기준 개발하고자 하였음.
이러한 내용을 실험적으로 증명하기 위해 discovery study와 replication study set을 통해 case-control study를 시행함. 또한 target 유전자의 RNA levels과 protein levels에서의 발현 차이 분석을 위해 대장암 시료 수집하였고, Colorectal cancer cell line 및 Normal colon cell line 구축하였음.
구체적인 연구결과 내용은 다음과 같음.
○ 한국인의 대장암 발생 환경위험점수(Environmental Risk Score, ERS) 개발
- 기존연구에서 제시한 대장암 환경위험요인의 타당성을 KCPS (Korean Cancer Prevention Study) I, KCPS II를 통해 타당성 재확인하여 Healthy Lifestyle Score (HLS)에 포함된 대장암 발생 위험요인별 가중치를 부여하여 한국인에 적합한 환경위험점수(ERS) 개발. 본 연구에서는 최종 ERS 모형에 연령, 성, 공복시 혈당, 흡연, 운동, 대장암 가족력을 포함함.
- 2015년 발표한 대장암 위험 모형에서는 흡연, 운동 등 생활환경습관과 구분하기 위해 전통적 위험점수(traditional risk score)라고 명명하여 표기하였음.
○ 한국인의 대장암 발생 유전위험점수(Genetic Risk Score, GRS) 개발
- 문헌조사, GWAS study, Imputation method, Axiom Exome Sequencing, SNP sequencing을 통해 유의한 SNP 선정.
- 국제 공동 연구(Asian Genetic Epidemiology Network, AGEN)를 통해 replication 진행.
- 이러한 방법을 통해 총 48개의 SNP을 선정하고 이중 유의한 결과를 보인 8개의 SNP(rs6659923, rs3802842, rs4444235, rs1447295, rs12613322, rs10795668, rs9929218, rs4939827)을 최종 선정하여 GRS를 개발함.

○ 대장암 발생 GERS를 활용하여 대장암 발생 고위험 기준 개발
- 최종 개발한 대장암의 유전-환경 위험점수를 이용하여 대장암 고위험군 선별 가능.
- 개발된 모형은 대장암 73%, 결장암 75%, 직장암 74%의 예측율을 보였음.

This study aims to investigate the environmental/genetic risk for colorectal cancer development using approximately 180,000 participants who developed colorectal cancer in prospective and retrospective cohort study models. About 350 subjects diagnosed with colorectal cancer and 994 controls obtained from the cohort and previous studies on GWAS were analyzed by Affymatrix exome array chip, and SNPs were further selected for sequencing. The selection of 23 SNPs was based on 48 previous studies which reported their relativeness with colorectal cancer. Using interactions between gene-environmental factors and colorectal cancer, the prediction model for colroectal cancer could be evaluated using a developed GERS method and could set criteria for the high risk group.
Through discovery study and replication study set, a case-control study was conducted in order to provide experimental evidence of this study. In addition, colorectal cancer cell line as well as normal colon cell line were constructed for analysis finding differences in target gene expressions at RNA and protein levels.
○ Environmental Risk Score (ERS) Development for Colorectal Cancer in Koreans
- The ERS applicable to Koreans was developed by reconfirming ERS model via KCPS I and KCPS II. The risk factors for colorectal cancer included in Healthy Lifestlye Score (HLS) were weighted for ERS development. The final ERS model includes age, sex, FBS, smoking status, exercise status, and family history of colorectal cancer.
- In this study, the colorectal cancer risk model reported in 2015 was designated as "traditional risk score" to differentiate from factors such smoking and exercise status.
○ Genetic Risk Score (GRS) Development for Colorectal Cancer in Koreans
- SNPs relevant to colorectal cancer were selected based on literature search, GWAS study, imputation method, Axiom Exome Sequencing, SNP sequencing, etc.
- Replication was conducted via Asian Genetic Epidemiology Network (AGEN).
