□ 연구의 목적 및 내용
약제내성결핵의 실태와 추이를 파악하는 것은 국가의 결핵관리 사업의 성과를 평가하는데 있어서 가장 중요한 역학적 지표 중의 하나로써, 항결핵제 감수성 결과를 토대로 다제내성결핵(multi-drug resistant tuberculosis, MDR-TB)을 진단하고 적절한 항결핵제를 선택하게 됨. MDR-TB의 진단은 전통적으로 배지를 이용한 약제감수성 검사(drug susceptibility test, DST)를 이용하여 진단되어 왔으나 결과보고까지 시간이 오래 걸리는 것이 주요 단점임. 따라서 최근에는
□ 연구의 목적 및 내용
약제내성결핵의 실태와 추이를 파악하는 것은 국가의 결핵관리 사업의 성과를 평가하는데 있어서 가장 중요한 역학적 지표 중의 하나로써, 항결핵제 감수성 결과를 토대로 다제내성결핵(multi-drug resistant tuberculosis, MDR-TB)을 진단하고 적절한 항결핵제를 선택하게 됨. □ 연구결과의 활용계획
차세대 염기서열 분석 기법으로 결핵균 염색체의 많은 부분의 염기서열을 쉽게 분석함으로써 균주를 구별할 수 있는 새로운 구별법이 나타나고 여러 계통의 결핵균의 전체 게놈이 밝혀질 것이고, 결핵균 계통에 따라 전염력, 발병력 등의 차이가 있는지, 차이가 있다면 그에 따른 영향은 어느 정도인지를 알 수 있게 될 것이다. 따라서 분포되어 있는 균주의 계통에 알맞은 결핵관리 대책을 마련할 수 있을 것으로 판단하고 있다.
유전자형 검사는 항결핵약제의 내성에 관여하는 유전자의 특정 부위의 돌연변이를 확인하여 내성 여부를 검사하는 방법으로 객담 검체 내에 소수의 결핵균이 있어도 검출이 가능하므로 신속히 결과 확인이 가능함.

The increasing burden of multidrug resistant (MDR)-TB, defined by resistance to rifampin (RFP) and isoniazid (INH), and extensively drug resistant-TB, defined by MDR-TB with additional resistance to fluoroquinolones (FQs) and more than one second-line injectable drug, is a serious impediment to global TB control. We evaluated the feasibility of full-length gene analysis including inhA, katG, rpoB, pncA, rpsL embB, eis, and gyrA using a semiconductor NGS with the Ion AmpliSeq TB panel to directly analyse 34 sputum specimens confirmed by phenotypic DST: INH, RFP, ethambutol (EMB), pyrazinamide (PZA), amikacin, kanamycin, streptomycin (SM), FQs including ofloxacin, moxifloxacin, and levofloxacin.
□ Results
1. Distribution of molecular drug resistance profiles by Ion AmpliSeq TB
Of the samples analysed by the Ion Torrent Proton with the Ion AmpliSeq TB panel, 23/34 (68%) resulted in genotypic resistance in the coding region of katG, rpoB, pncA, embB, rpsL, or gyrA, whereas no reads aligned with the promotor region of common ihnA, eis, or pncA mutations. Six false-negative results for 5 c.-15C > T of inhA and 1 c.- 6G > A of ahpC were observed. A substitution at position 463 (p.Arg463Leu) in the katG product has been previously shown to have no effect on drug resistance was excluded (Torres et al., 2015). Of the various mutated genes, katG was the most frequent (96%, 22/23), followed by embB (43%, 10/23), rpoB (39%, 9/23), rpsL (26%, 6/23), pncA (17%, 4/23), and gyrA (9%, 2/23). Interestingly, six out of eight p.Arg463Leu katG mutations simultaneously occurred with other katG mutations such as p.Ser315Thr or p.Phe183_Gly184del. In total, we identified 56 mutations (50 previously reported and 6 novel mutations) related to possible drug resistance from 23 samples.
2. Molecular drug resistance profiles as predictors of phenotype
The molecular drug resistance profiles showed “very good” and “substantial” strength of agreement for the phenotypic DST results of RFP and EMB, PZA, SM, FQs resistance with specificities of 96%, and 88%, 97%, 100% and sensitivities of 100%, and 88%, 60%, 67%, respectively.
□ Expected Contribution
Genotypic DST using ion semiconductor NGS may be able to identify known and novel mutations that, when correlated with data from the phenotypic DST, could identify new resistant strains of MTB as well as the cumulative inhibitory effect of multiple mutations.
Our NGS methodology should be validated by larger studies in the future, using various types of samples to show different phenotypic drug susceptibilities from various geographical regions. Molecular drug resistant profiles should continue to be complemented and validated by subsequent phenotypic DST studies.
