■ 연구 개발 목표
기후 변화 및 야외활동 증가로 인해 국내 뿐 만아니라 세계적으로 진드기 매개 감염병 발생이 증가하고 감염실태 또한 다양화되고 있음. 진드기 매개 질환의 각 질환별 증상 구분이 어려워 진단이 지연되어 치료가 적절히 이루어지지 않으면 사망에까지 이르게 되는 문제점이 있음.
따라서 본 연구는 진드기와 진드기 매개 질환의 환자에서 원인 병원체를 확인하고 각 질환별 연구용 자원 및 임상정보 확보하여 매개체-병원체-사람 간 상호작용에 관한 폭넓은 연구를 수행하기 위한 연구용 자원 확보하는 것임.

■ 연구 개발 목표

따라서 본 연구는 진드기와 진드기 매개 질환의 환자에서 원인 병원체를 확인하고 각 질환별 연구용 자원 및 임상정보 확보하여 매개체-병원체-사람 간 상호작용에 관한 폭넓은 연구를 수행하기 위한 연구용 자원 확보하는 것임.
■ 연구 방법 및 내용
► 야외활동 감염병 환자에서 원인 병원체 확인 연구를 위한 임상시험 심의위원회(IRB) 승인서 확보 및 18개 병원간 협력 네트워크 구성하였음. 진드기 및 진드기 매개질환 환자의 검체에서 genomic DNA 추출하여 원인 병원체 유전자를 PCR 법으로 진단하였으며 항체진단 검사를 동시에 수행함. 원인 병원체 10종 이상을 대상으로 2018~2019년 2년 동안 총 533명의 환자의 전혈 및 buffy coat 등의 환자 검체 및 60마리 진드기를 확보하여 매개체 감염병 연구를 수행하였음
■ 연구 결과
► 쯔쯔가무시증 의심 또는 야외활동력이 있는 발열 환자 검체 192건을 대상으로 수행한 쯔쯔가무시증 진단을 위한 nested PCR결과 77개가 양성으로 나타나 40.1%(77/192)의 PCR 양성률을 보였으며, O. tsutsugamushi strain Boryong, Karp, Ikeda, Kawasaki, Kanda, Pa-joo, Taguchi 그리고 Nishino 종들이 확인되었음
► 아나플라즈마증 의심 또는 야외활동력이 있는 발열환자 및 tick bite 환자 검체 400건을 대상으로 수행한 아나플라즈마증 진단을 위한 nested PCR결과 31개가 양성으로 나타나 7.8%(31/400)의 PCR 양성률을 보였으며, 모두 A. phagocytophilum으로 확인되었음
► Q-열 의심 또는 야외활동력이 있는 발열환자 및 tick bite 환자 검체 347건을 대상으로 수행한 Q-열 진단을 위한 nested PCR결과 20개가 양성으로 나타나 5.76%(20/347)의 PCR 양성률을 보였으며, 모두 C. burnetii로 확인되었음
► 라임병 의심 또는 야외활동력이 있는 발열환자 및 tick bite 환자의 전혈 검체 332건을 대상으로 수행한 pyrG nested PCR 결과 1개가 양성으로 나타나 0.3%(1/332)의 PCR 양성률을 보였으며, B. yangtzensis로 확인되었음
► 렙토스피라증 의심 환자 검체 82건을 대상으로 수행한 nested PCR 결과 모두 음성으로 확인됨.
그러나 본 연구결과에서 MAT 진단법을 이용하여 국내 환자에서 L. interrogans serovar Australis 혈청형의 존재를 최초로 확인하였음.
► 바토넬라 의심 검체 대상으로 바토넬라 PCR 진단을 수행하여 0.3%(1/333)의 PCR 양성률을 보였으며, B. henselase로 확인되었음. 또한 2명의 cat bite 환자에서 항체가 상승 확인하였음
► 리케치아증 의심 또는 야외활동력이 있는 발열환자 및 tick bite 환자 검체 263건을 대상으로 수행한 리케치아증 진단을 위한 nested PCR결과 모두 음성으로 확인되었음. 그러나 항체가를 조사한 결과 항체가 4배이상 상승한 환자 8명을 확인하였음.
► 2018~2019년 진드기에 물려 내원한 임상환자로부터 얻은 참진드기는 총 60마리였으며, 이들 모두 종, 성별, 발달단계를 확인함. 또한 Rickettsia, O. tsutsugamushi, Anaplasma, Borrelia, Coxiella, Bartonella, Francisella 및 Yersinia) 확인을 위한 PCR을 수행하였으며 그 결과 리케치아 양성 30건, 쯔쯔가무시 1건, 아나플라즈마 2건, 보렐리아 2건, 콕시엘라 10건, 바토넬라 2건이 확인되었음.
■ 연구 결론
예상외로 다양한 매개체 감염병이 한국에 존재함을 확인하였으며, 본 연구를 통해 매개체-병원체-사람 간 상호작용에 관한 폭 넓은 연구가 수행될 수 있는 검체 확보하였으며, 진드기 매개질환에 대한 효과적인 진단 및 치료법 개발을 위한 자원 확보 및 발판을 마련한 의미 있는 연구로 사료됨

■ Purpose

Due to the global climate change and increment in outdoor activities, the incidence of tick-bone infection is increasing not only in Korea but also around the world, with more diversified infectious conditions. The most common symptoms of tick-borne infectious disease are fever, myalgia and gastrointestinal disorders such as diarrhea and vomiting. Moreover, it is difficult to differenciate the symptoms associated with each tick-borne disease. Therefore, this study aimed to identify pathogens of tick borne diseases in both ticks and patients besides securing of these samples for each disease as research resources.
■ Method
This study was approved by Institutional Review Board (IRB) for identification of causative pathogens in patients with infectious diseases associated with outdoor activities. Total 18 hospitals participated in this cooperative network. The genomic DNA was extracted from patients’ sample with tick and tick-borne diseases; afterward diagnosis was made by PCR of different targets.
Subsequent samples were also analyzed with antibody titer. We obtained samples of whole blood and buffy coat from a total of 533 patients over a period of 2 years from 2018 to 2019.
■ Result
► After performing PCR for the diagnosis of O. tsutsugamushi from 192 suspected cases of scrub typhus or with outdoor activity, 77 cases were positive. The positive PCR product sequence was determined in both directions. As a result, O. tsutsugamushi strains namley Boryong, Karp, Ikeda, Kawasaki, Kanda, Pa-joo, and Taguchi species were identified. The clinical characteristics of scrub typhus positive patients were confirmed by positive tsutsugamushi-specific 56 kDa nested PCR or four-fold or more rise of IFA antibody titers.
► 400 suspected cases of anaplasmosis or with outdoor activity were tested by PCR for diagnosis of anaplasmosis. As a result, 31 out of 400 cases were positive. Positive cases were identified as A. phagocytophilum. The clinical characteristics of anaplasmosis positive patients were confirmed by positive anaplsma-specific groEL and ankA nested PCR or more than four-fold rise of IFA antibody titers.
► 263, 216 and 82 suspected cases of rickettsioses, bartonellosis and leptospirosis respectively, were found negative by performing target PCR for each of them. However, we confirmed eight patient with more than four-fold of IFA titer for diagnosis of rickettsiosis except one case of B.henselae.
Moreover, we confirmed the antibody of L. interrogans serovar Australis in serum of the patient by MAT for the first time in Korea.
► As a result of performing PCR for the diagnosis of Q-fever from 347 suspected cases of Q-fever disease or with outdoor activity, 20 cases were positive. The positive PCR product sequences were identified as C.burnetii.
► As a result of performing PCR for diagnosis of lyme disease from 333 suspected cases of lyme disease or with outdoor activity, one case was identified as B.yangtzensis.
► A total of 60 ticks were obtained from patients with positive history of tick bite, all of which were identified in accordance with species, sex, and stage of development. In addition, PCR was performed to confirm Rickettsia, O.tsutsugamushi, Anaplasma, Borrelia, Coxiella, Bartonella, Francisella, and Yersinia. As a result, 20 cases of Rickettsia, 2 cases of Anaplasma, 2 cases of Borrelia, 10 cases of Coxiella and 2 cases of Bartonella were identified.
■ Conclusion
This study revealed a wide range of vector-pathogen-human interaction, and it is considered to be a valuable study that has significant resources for development of effective diagnosis and treatment of tick-borne diseases.
