본 연구는 2019년에 전국 산욕기 수유부 1,054명을 대상으로 실시한 요오드 섭취 실태조사 연구에 대한 추적조사로 산욕기의 과도한 요오드섭취가 산모의 이후 갑상선질환 발생 및 자녀의 성장발달에 미치는 영향에 대해 알아보고자 실시되었다. 추적조사를 위하여 휴대폰과 웹에서 활용 가능한 'untact' 설문조사 시스템을 개발하였다. 조사내용에는 갑상선질환 진단 여부를 포함한 일반설문조사, 식품섭취빈도조사(엄마), 3일 비연속 식이조사(엄마, 자녀). 엄마 혈액의 갑상선호르몬(T3, TSH, Free T4) 측정, 자녀의 성장 및 발달 상태 평가 등이 포함되었다. ’19년 실태조사 이후 갑상선질환 의사진단을 받았는지에 대해 조사된 대상자의 수는 684명(추적률 64.9%)이었다. 이 중 FFQ와 3일 식이기록은 550명. 혈액 갑상선호르몬 측정은 535명이 완료하였다. 자녀의 성장 및 발달 상태 평가는 540명이 응답하였다. 3년의 추적조사 결과, 엄마의 갑상선질환 발생은 684명 중 23명(3.4%)으로 나타났다. 실태조사시 식이기록으로 조사된 엄마의 평균 요오드 섭취량은 2.927㎍/day이었으나, 현재 엄마의 평균 요오드 섭취량은 365.48/day이었다. 실태조사 요오드 섭취량을 한국인 성인의 요오드 상한 섭취량인 2,400㎍/day로 나누어 엄마의 갑상선질환 발생 비율, 현재 갑상선 호르몬 상태, 자녀의 현재 성장·발달과의 관련성을 분석한 결과 유의적인 차이를 보이지 않았다. 수유부의 요오드 섭취량에 따른 갑상선 질환 발생 위험도를 평가하기 위하여 실태조사시 요오드 섭취량을 50㎍ 단위로 cut-off를 설정하여 cut-off 이하 및 초과로 섭취한 사람들의 갑상선질환 발생 위험도(Hazard Ratio)를 다변량 보정하여 분석한 결과, 요오드 섭취와 갑상선 질환 발생과의 통계적으로 유의적인 연관성을 보이지 않았다. 결론적으로, 전국에 거주하는 산모를 대상으로 한 실태조사와 추적조사 결과 산욕기의 과도한 요오드 섭취는 갑상선질환 발생과 관련성을 보이지 않았으며, 자녀의 성장·발달에도 영향을 미치지 않는 것으로 나타났다. 따라서 수유부의 구체적인 요오드 적정 섭취량을 제안하기 위해서는 더 많은 다각적인 연구가 필요하며, 특히 잘 디자인된 대규모 장기 코호트 연구가 필요한 것으로 보인다.

This study was conducted to investigate the effect of excessive iodine intake during childbirth on the incidence of thyroid disease of mother and the growth of children as a follow-up study of an representative nationwide iodine intake survey conducted on 1,054 lactating mothers in Korea in 2019. For the follow-up study, an ‘untack’ survey system that can be used on mobile phones and the web was developed. The questionnaire included thyroid disease history, FFQ(mother), non-consecutive 3-day dietary records(mother and child), mother’s blood thyroid hormones(T3, TSH, Free T4) concentration, child’s growth and development status. Among 1,054 mothers, 684 (64.9%) responded whether or not they were diagnosed with thyroid disease. 550 mothers completed FFQ and a three-day dietary record, and 535 mothers completed blood thyroid hormone function tests. Also, we collected data on the growth and development of 540 children. As a result of three year follow-up, the incidence of maternal thyroid disease was 23 out of 684(3.4%). In the baseline study, the average iodine intake of 1,054 lactating mother was 2.927㎍/day, and the current maternal iodine intake was 358 ㎍/day. As a result of analyzing the relationship between maternal thyroid disease incidence, current thyroid hormone status, and child's current growth development by dividing iodine intake by 2,400 μg/day, there was no significant difference.
In order to evaluate the risk of thyroid disease according to the iodine intake of lactating mother, a cut-off was set for the iodine intake in units of 50㎍ of baseline study, and the multivariable adjusted hazard ratio of thyroid disease among those who consumed less than or more than the cut-off was calculated.. There was no statistical relationship between iodine intake and thyroid disease.
In conclusion, as a result of a baseline survey and follow-up survey of 1,054 mothers, it was found that excessive iodine intake after childbirth was not related to thyroid disease and did not affect the growth and development of their children.
Therefore, more multilateral studies are needed to suggest a upper limit of iodine intake for lactating mothers, especially well-designed large long-term cohort studies.
