The purpose of this study was to compare the therapeutic results between the two groups of patients who were treated either by 3-year Okayama regimen which comprised induction with aclacinomycin, cytosine arabinoside, and prednisolone, and repetitive cyclic consolidation and maintenance, or by 2-year KSBRM regimen which consisted of induction with BH-AC, idarubicin, and 6-thioguanine, and consolidation and maintenance. Materials and Methods: The subjects were 33 AML patients among 42 newly diagnosed AML at Chonnam University Hospital from Apr. 1991 to Jun. 1998, after excluding 9 cases of Down syndrome or secondary leukemia. Until April, 1994 10 patients were treated by the Okayama regimen while 23 patients received KSBRM regimen thereafter. The median follow up period was 22.3 months in the Okayama group, and 17.4 months in the KSBRM group. The remission induction rate, relapse rate, and survival rate were compared retrospectively between the two groups. Results: 1) The remission induction rate by the first line treatment for all subjects was 69.7% (23/33cases), without significant difference between the two groups. The overall remission rate, added after second line treatment, was 81.8% (27/33): Okayama group, 80.0% (8/10); KSBRM group, 82.6% (19/23). 2) The relapse was observed in 7 out of 25 patients (25.9%) who achieved an initial remission. The relapse rate in the Okayama group was 37.5% (3/8) while that in the KSBRM group was 21.1% (4/19). Relapse rate was not different between the two groups. 3) Deaths were encountered in 13 patients (39.4%): 60.0% (6/10) of Okayama group vs 30.4% (7/23) of KSBRM group (P=0.43). 4) Kaplan-Meier 3-year disease free survival (DFS) for all of the patients was 48%.
