BMI와 혈청 간기능검사, 공복시혈당, 간초음파검사 심화도 등에서 두 군 간에 의미있는 차이를 보인 것은 BMI가 유일하였는데(지방증 25.8±2.8, NASH 28.4±3.4, p=0.025), BMI 28.9 이상이면 NASH일 가능성이 매우 높았다. Thus, the aims of our study were to identify clinical factors distinguishing NASH from steatosis in patients with nonalcoholic fatty liver and to identify clinical predictors reflecting the pathologic severity in patients with NASH. METHODS: Complete data including liver biopsy were available in thirty-nine patients with fatty liver based on the following criteria: (1) persistent abnormal liver biochemistries (> 3 months); (2) the significant steatosis (> 10%); (3) appropriate exclusion of other liver disease such as hepatitis B, hepatitis C, alcohol drinking history, others. All liver biopsies were reviewed by an experienced pathologist and graded steatosis, inflammation and fibrosis. Comparison between nonalcoholic steatosis (NAS) and NASH was done across clinical and biochemical data. RESULTS: NAS was fifteen patients and NASH was twenty-four patients with following characteristics: age 32.7 and 32.8, respectively; gender (M:F) 13:2 and 17:7, respectively, in which there is no statistical difference. Serum biochemistries including fasting blood sugar did not show difference between two groups. Only body mass index (BMI) differentiate NASH (28.4± 3.4) from NAS (25.8±2.8) (p< 0.03). The best threshold for BMI distinguishing NASH form NAS was 28.9. Serum AST/ALT ratio was reversely associated with the degree of steatosis (p<0.01). Multivariate analysis showed the relation between BMI and the degree of fibrosis (p<0.01) and the reverse-relation between serum ALT value and the degree of fibrosis (p<0.01). Ultrasonographic grade of fatty change was associated with the sum of pathologic grade (p <0.05), but not with each histologic component such as fibrosis, steatosis and inflammation of NASH. COCLUSION: Body mass index helps identify NASH patients from nonalcoholic fatty liver by threshold 28.9. In NASH patients, high BMI with low ALT value and severe fatty change of ultrasonography could suggest severe fibrosis. This is the subgroup of patients with NASH who would be expected to derive the most benefit from close follow-up including a liver biopsy.
