Objective:To identify protective effect for perineal laceration and its mechanism of new fetal head delivery method during vaginal delivery at term, and influence of restrictive episiotomy. Methods:In this prospective clinical trial of 434 patients with singleton pregnancy more than 37 weeks' gestation, pregnant women were randomly divided into three groups;classical delievery method with episiotomy group (145 cases), new delivery method with episiotomy group (146 cases), new delivery method w5thout episiotomy group (143 cases) from September 1999 to April 2002 at Chonnam National University Hospital. At the HaHaHa method, maternal pushing was stopped just before the delivery of the largest diameter of fetal head and frequent deep breathing was applied. The frequency and degree of vaginal and perineal lacerations during vaginal delivery were compared among three groups. Surface EMG was used to assess the electrical activity of pelvic floor muscles in 15 patients to whom the new method was applied. The incidence of urinary incontinence, anal incontinence, dyspareunia, and parineal pain in three groups, and the difference of vaginal pressure between group 2 and 3 were invetigated at three months after delivery. Results:The frequency of severe perineal lacerations was 13.8% in group 1 and 4.0% in group 2 (P<0.01), Severe perineal laceration was reported by six women (4%) in group 2 and one woman (0.7%) in group 3 (P<0.056). The rates of vaginal wall laceration and anterior perineal laceration were highest in group 3 (P=0.005, P<0.001), but most lacerative lesions were mild degree and had no clinical significance. Surface EMG during vaginal delivery in patients used new delievery method revealed significant relaxation of pelvic floor muscles during HaHaHa method (15 cases). The incidences of urinary incontinence, anal incontinence, dyspareunia, and perineal pain were no statistical difference among three groups at three months after vaginal delivery.
