간호 조직문화는 간호 서비스의 질적, 양적인 면을 결정하고 전문적인 간호 서비스에 대한 소비자들의 요구를 충족시켜줌으로써 궁극적으로 간호조직 전체의 효율성 증대에 지대한 역할을 한다. 이에 본 연구는 조직문화 특성과 간호 조직문화 유형을 구성하는 문화적 특성에 관한 문헌고찰과 선행연구 결과를 토대로 개념적 기틀을 구성하고, 공유가치, 관리전략, 공식화, 의사소통, 의사결정, 자율성, 동기화 및 리더십을 간호 조직문화 특성으로 선정하여, 직무만족, 조직몰입 및 자발적 행동의 질적 지표와 총자본 순이익률 및 의료수익 증가율의 양적 지표가 포함된 조직 유효성과의 관계를 규명하고자 시도되었다. 본 연구의 대상자는 서울 및 경기지역에 소재 한 500병상 이상의 8개 종합병원에 근무하고 있는 경력 1년 이상의 간호사(일반 간호사, 책임 간호사 및 수간호사) 717명이었다. 본 연구의 자료수집 기간은 2002년 10월 21일부터 12월 7일까지이며, 조직유효성의 질적 지표는 자가 보고식 설문지를 통하여 자료수집 하였고, 조직 유효성의 양적 지표는 병원표준화 심사결과 보고서 2001, 2002년 자료를 통해 수집되었다. 자료 분석은 SAS 및 SPSS Program을 이용하여 서술적 통계, 신뢰도, 군집 분석과 정준 판별분석, ANOVA 및 Scheffe test등의 통계기법을 이용하였다. 본 연구의 결과는 다음과 같다. 1) 간호 조직문화 특성을 분류한 결과, 3개의 군집으로 분류되었다. 즉, 제1군집은 관리전략과 공식화 정도가 높게 나타나 구조와 업무체계 지향의 조직문화 특성이 있는 군집으로, A, E, G 3개의 병원이 포함되었다. 제2군집은 공유가치와 의사소통, 의사결정, 자율성 및 동기화 정도가 높게 나타나 비구조적 측면의 인간, 관계지향의 조직문화 특성이 있는 군집으로, D, F, H 3개의 병원이 포함되었다. 제 3군집은 리더십 발휘 정도가 높게 나타나 리더십 지향의 조직문화 특성이 있는 군집으로, B, C 2개의 병원이 포함되었다. 2) 간호 조직문화 특성에 따라 분류된 세 군집과 조직 유효성의 질적 지표를 총합한 점수와의 차이를 분석한 결과, 제 2군집이 조직 유효성 질적 지표 총합이 평균 3.18로 가장 높았고, 세 군집간에도 통계적으로 유의한 차이를 보였다(F=5.422, p=.005). 즉, 간호 조직문화 특성에 따라 분류된 세 군집과 직무만족, 조직몰입, 자발적 행동과의 차이를 보면, 제 2군집이 제 1군집과 제3군집에 비해 조직몰입(F=3.924, p=.020)과 자발적 행동(F=5.643, p=.004) 점수가 더 높았으며, 세 군집간에도 통계적으로 유의한 차이를 보였다. 3) 간호 조직문화 특성에 따라 분류된 세 군집과 조직 유효성의 양적 지표 즉, 총자본 순이익률(F=1.578, p=.294)과 의료수익 증가율(F=.149, p=.865)에 대한 차이를 분석한 결과, 세 군집간에 통계적으로 유의한 차이가 없는 것으로 나타났다. 본 연구결과, 공유가치와 의사소통, 의사결정, 자율성 및 동기화의 간호 조직문화 특성이 조직 유효성에 유의한 영향을 미치는 것으로 생각된다. 따라서 간호 조직 유효성을 증진시키기 위해 조직 구성원 모두가 공유가치를 창출할 수 있는 교육 프로그램을 도입하거나, 간호사들의 자율성을 보장하기 위한 경력개발프로그램과 임파워먼트 전략 등을 도입하여 환자간호의 질을 향상시키고, 간호사의 전문적 성장의 기회를 제공하는 등 간호관리 실무에서 적극 활용하는 것이 바람직하다고 생각한다.

This study aims to classify the character of the nursing organizational culture and to examine the relationship between the character of the nursing organizational culture and the organizational effectiveness. The data were gathered from the Self-reported Questionnaires of 717 ordinary, chief and head nurses who work for eight different general hospitals located around Seoul and Kyounggi province. The quantitative indicators of the organizational effectiveness are selected from the Evaluation Report on the Standardization Result of Hospital of 2001 and 2002. The period of data collection was from October 21 to December 7, 2002. The tools applied to this study were modified or moderated to adapt to the nursing organization. Belows are details on them. The Modified Measuring Scales for the shared values, Lee Myung-ha(1998)'s Managerial Strategic Method Scale, Won Sung-yeon(1989)'s Formalization Method Scale, Woo Chisung(1989)'s Method of Intraorganizational Communication Scale, Hage & Aiken(1967)'s Decision-making Participation in Organization Scale, Schutzenhofer(1983)'s Professional Nursing Autonomy Scale, Van de ven & his colleague's Job Motivation Scale, Bass(1985)'s Multifactor Leadership Questionnaire(MLQ), Paula(1978)'s Job Satisfaction Scale, Mowday's(1979) Organizational Commitment Questionnaire, Netemyer, Boles, McKee & McMurrian(1997)'s Organizational Citizenship Behavior Scale. All data were analyzed by SAS & SPSS Program. Samples' general characteristics and research variables were depicted by descriptive statistics. All measuring scales' internal coherence and reliability were tested by Cronbach's α. Clustering Analysis and Canonical Discriminant Analysis were used to classify the character of the nursing organizational culture. ANOVA and Scheffe's test were applied to examine the relationship between the character of the nursing organizational culture and the organizational effectiveness. Followings are the result of this study. 1) In this study, the character of the nursing organizational culture was classified as three different clusters. The first cluster highly recognized the managerial strategy and formalization. The second cluster focused upon the shared values, communication, decision-making, autonomy and job motivation. The third cluster highly recognized the leadership. 2) The total qualitative indicators on the organizational effectiveness among the classified clusters were analyzed that the second cluster's degree of the organizational effectiveness was much higher than those of the other two clusters'(F=5.422, p=.005), And the qualitative indicators on the organizational effectiveness among the classified clusters were analyzed that the second cluster's degrees of the organizational commitment(F=3.924, p=.020) & the organizational citizenship behavior(F=5.643, p=.004) were much higher than those of other two clusters', and among three clusters did show significantly meanings. 3) The quantitative indicators on the organizational effectiveness among the classified clusters were analyzed that the difference of the profit rate on total capital and the increasing rate of medical revenue among three clusters did not show any statistically significant meanings. From the above results, it is logically inferred that the second cluster's culture which characterized its components as the shared values, communication, decision-making, and autonomy and job motivation is very implicative to enhance the organizational effectiveness. This is because the second cluster's character showed highest score not only on organizational effectiveness but also organizational committment and organizational citizenship behavior. Belows are suggestions based upon the result of this study. 1) This study concluded that the most effective culture to the nursing organization is the one whose managerial ways, like the second group of this study, are having shared value, satisfactory communication not only vertically but also horizontally, high job motivation through participatory decision-making and strengthened autonomy. 2) Now that even the same cultural factors impact differently on the nursing organizational culture as its size, comparative study on various size of hospitals is recommended. 3) It is much desirable if a qualitative study is tried to further know the nursing organizational culture to the subject of study. 4) It is suggested that the scientific and objective research tools be developed to measure the variables which explained the character of the nursing organizational culture in this study.
