연구배경 : 결핵성 흉막염은 우리나라에서 흉막 삼출액의 가장 흔한 원인이다. 진단 방법으로 객담, 흉수 등의 결핵균 배양 검사는 민감도가 낮으며, adenosine deaminase (ADA) 증가 소견은 민감도는 높으나 악성 흉수 등에서도 증가할 수 있으므로 정확한 진단을 위하여서는 침습적인 흉막 조직 검사가 필요하다. 그러나 조직 검사에서 육아종성 병변이 관찰되지 않는 경우 진단에 어려움이 있다. 본 연구는 결핵성 흉막염의 진단에 미생물학 검사, 조직 검사와 비교하여 흉수 및 흉막 조직을 대상으로 한 결핵균 PCR의 진단적 유용성을 알아보고자 시행되었다. 방법 : 전향적으로 2002년 1월부터 2003년 12월까지 2년간 흉수 검사에서 림프구 우세 삼출액 소견을 보여 조직 검사를 시행한 환자를 대상으로 하였다. 결핵성 흉막염의 진단 기준은 (1) 객담 또는 흉수에서 결핵균 배양 양성인 경우 (2) 흉막 조직검사에서 육아종성 병변이 관찰된 경우, (3) 육아종성 병변이 관찰되지 않으나 림프구 우세 삼출액 소견을 보이며 항결핵 치료 후 호전되어 결핵에 합당한 임상상을 보인 경우로 정의하였다. 객담 및 흉수로 항산균 도말 및 배양을 시행하였고 흉수의 염증세포 감별계산, 단백, 포도당, 알부민, ADA 등 생화학적 검사와 흉막 조직 검사를 시행하였다. 흉수 및 흉막 조직으로 IS6110 DNA 분절을 목표로 한 결핵균 PCR을 시행하였다. 결과 : 림프구 우세 흉막 삼출액으로 흉막 조직 검사가 시행되었던 총 121명에서 진단이 불명확한 8명을 제외한 113명 중 결핵성 흉막염은 96명이었으며 악성 흉막염은 17명이었다. 가. 결핵의 진단 결핵성 흉막염 96명의 환자 중 객담 또는 흉수에서 결핵균 배양 양성인 경우가 48%(46명), 흉막 조직검사에서 육아종성 병변이 관찰된 경우가 40%(38명), 육아종성 병변이 관찰되지 않으나 림프구 우세 삼출액 소견을 보이며 항결핵 치료 후 호전되어 결핵에 합당한 임상상을 보인 경우가 12%(12명)이었다. 나. ADA 활성도 ADA는 악성 흉막염보다 결핵성 흉막염에서 유의하게 높았다 (24.5±17.7 vs 95.7±59.1 U/L, p < 0.001). ADA >40 U/L를 기준값으로 하였을 때 결핵성 흉막염 진단의 민감도 96.9%, 특이도 94.1%, 양성예측률 98.9%, 음성예측률 84.2%, 정확도 96.5%였다. 다. 항산균 도말 및 배양 검사 결핵성 흉막염 96명 중 객담의 항산균 도말은 7%, 배양은 46.5%에서 양성이었다. 흉수의 항산균 도말은 3%, 배양은 19.1%에서 양성이었다. 라. 흉막 조직검사상 육아종이 보여 결핵으로 진단된 예는 40%(38/96)이었다. 마. PCR 결과 및 진단적 유용성 흉수 결핵균 PCR은 17.4%에서 양성이었으며 흉막 조직 결핵균 PCR은 47.9%에서 양성이었다. 악성 흉막염에서 흉수와 흉막 조직의 PCR은 전부 음성이었다. 결핵균 배양과 PCR을 합한 경우 76.0%(73/96)에서 원인적 진단이 가능하였으며 PCR은 결핵균 배양 음성이었던 환자 중 27명에 양성으로 진단율을 30.2% 향상시킬 수 있었다. 결론 : 결핵성 흉막염의 진단에서 흉수 및 흉막 조직의 결핵균 PCR은 세균학적 검사에 ADA를 추가하는 경우와 비교하여 진단적 유용성은 떨어진다. 그러나 흉수 PCR은 흉수의 항산균 도말 양성율이 낮은 점을 고려할 때 흉막 조직검사가 어려운 환자군에서 보조적 진단법으로 사용될 수 있을 것으로 사료된다. 흉막 조직 PCR은 림프종과 같이 위양성으로 ADA가 증가할 수 있는 악성 흉막염이 의심되거나 육아종이 관찰되지 않는 경우 보조적 진단법으로 사용될 수 있을 것으로 사료된다.

Acid-fast staining of pleural fluid is a rapid, inexpensive method, but sensitivity is low. The culture of pleural fluid for Mycobacterium tuberculosis permits firm diagnosis, but sensitivity is low and results can take several weeks. Biochemical analysis for adenosine deaminase (ADA), interferon-γ are useful in the diagnosis of tuberculous pleuritis. However, different authors have reported various cutoff levels for the pleural fluid ADA with variable sensitivity and specificity. The aim of this study was to evaluate the utility of PCR of pleural fluid and tissue in the diagnosis of pleural tuberculosis, and comparing it with conventional methods; microbiologic technique, ADA and histology. Methods : One hundred and twenty one patients with lymphocyte- dominant exudative pleural effusion were enrolled. Acid-fast staining of sputum, pleural fluid, pleural biopsy and M. tuberculosis culture of sputum, pleural fluid were performed. Pleural fluid analysis for cell count, biochemical profile, ADA activity were tested. PCR for M. tuberculosis were performed with the pleural fluid and formalin-fixed biopsy specimen. Results : 1) Of the 121 patients , 96 patients were tuberculous pleuritis, 17 patients were malignant pleural effusion, and 8 patients were undetermined. 2) ADA activity was significantly elevated in tuberculous pleuritis than malignant effusion (95.7±59.1 vs 24.5±17.7 U/L, p<0.001). 3) Sensitivity of each diagnostic method was 48.0%(25/52) for sputum culture, 22.2%(16/72) for pleural fluid culture, 77.1%(74/96) for granuloma, 17.4%(15/86) for pleural fluid PCR, and 47.9%(46/96) for pleural biopsy PCR. 4) PCR was positive in 19%(4/21) of the group that was negative in AFB stain and granuloma. Conclusion : The sensitivity of M. tuberculosis PCR in pleural fluid and biopsy were 17.4% and 47.9%, respectively. The sensitivity was low compared with ADA activity, PCR may be useful in the etiologic diagnosis and in differential diagnosis with malignant effusion like lymphoma.
