Background: Quality of life (QoL) is one of the most important issues in caring the hospice patients because the goal of the palliative care is the achievement of the best quality of life for patients and their families. This study was conducted to develop and validate the QoL questionnaire in hospice patients in Korea. Methods: The hospice cancer patients who were over 20 years old and who had the advanced cancers, which were not responsive to curative treatments, were included. The QoL scale was developed through the following steps; 1) item generation through the in-depth interview and review of existing scales; 2) item reduction through the expert panel and the Delphi methods 3) the reliability & validity test. Reliability was measured by the test-retest reliability and by internal consistency. Also, the construct validitywas evaluated by multi-trait scaling analysis and the concurrent validitywas done using EORTC QLQ C-30 and McMaster QLS. Results: The final Hospice Quality of Life Scale (HQLS) consisted of 40 questions within 13 domains. Its internal consistency was 0.41~0.93 and test-retest reliability was 0.29~0.83. For construct validity, the QoL scores were significantly different according to theirECOG performance status (known-group validity). Convergent success rate was optimal in 9 subscales and discriminant success rate was in 4 subscales. Total and related subscale scores were significantly correlated with the subscales in EORTC QLQ C-30 and McMaster QLS, which supported the criterion validity. Conclusions: HQLS was approved as an efficient tool in assessing the quality of life of the hospice cancer patients in Korea. It can be used to evaluate the outcome of the palliative medical intervention.
