본 연구는 심폐소생술의 전문성을 향상시키고자 2006년 4월 7일부터 9월 8일까지 제조분야가 아닌 의료분야에 6시그마 기법을 적용하여 과학적이고 통계적인 시각에서 고객의 의견을 중심으로 문제를 해결하기 위하여 단계별로 분석하고 단계별 개선사항을 토대로 심폐소생업무 심실세동프로토콜을 개발하고 프로토콜을 근거로 동영상을 제작하여 내용타당도와 임상타당도를 검증한 후 최종 심폐소생업무 심실세동 프로토콜과 동영상을 확정하고 적절성을 평가하는 단계를 거치는 액션 연구이다. 수집된 자료는 Minitap 15 PC+ 1개월 평가판 프로그램을 이용하여 분석하였고, 서술 자료는 친화도 기법을 이용하여 분석하였다. 본 연구의 연구진행 과정은 5단계로 다음과 같다. 첫 번째, 6시그마기법 정의단계로 6시그마 과제 선정 배경과 당위성을 기술하여 6시그마 과제를 선정, 과제를 추진할 개선팀 구성, 과제 실행계획서 작성 및 QI팀에 등록하여 간호부의 승인을 얻는 과정으로 간호사 대상 심폐소생술 상황시 문제점에 대한 특성요인도(Fish Bone Diagram) 작성과 간호사 및 전공의 · 교수 대상 설문조사를 실시하였다. 특성요인도 분석 결과 주요항목을 확인하였으며, 확인된 주요 항목을 주요 문제로 확정하기 위해 간호사 대상 설문 분석 및 전공의 · 교수 대상 설문 분석 및 통합 결과 총 7개 항목의 주요 문제를 확인하였다. 7개 항목의 주요 문제 중 시급도와 중요도가 높은 4개 주요 문제를 선정하고 선정된 4개 주요 문제에 대하여 간호사의 핵심요구사항인 주요품질특성으로 전환하였다. 그리고 4개 항목의 주요품질특성을 근거로 제시함으로써 심폐소생업무 심실세동 프로토콜 개발을 6시그마 과제로 승인을 거쳤다. 두 번째, 6시그마 측정단계로 정의단계에서 도출된 주요 문제를 명확히 정의하기 위해서 측정 가능한 주요 문제를 확인하고, 확인된 주요 문제에 대하여 DPMO에 의한 6시그마 현 수준 측정과 주요품질특성에 영향을 줄 수 있는 잠재원인변수들을 도출하여 우선순위화 및 집중적으로 관리해야 할 항목을 결정하는 단계이다. 측정 가능한 주요문제를 확인하기 위하여 CPR전자의무기록 수집 · 분석 및 6시그마 현 수준 측정, 간호사 대상 지식 · 수행능력 설문조사 및 분석, 수기 작성된 병동 CPR 간호기록지 수집 · 분석 및 6시그마 현 수준 측정, 병동 응급약물 및 응급물품 점검표 파악 · 분석 및 6시그마 현 수준 측정, 현행 심폐소생업무 프로세스를 파악하였다. 그리고 선정된 4개 항목의 주요품질특성에 영향을 줄 수 있는 14개의 잠재원인변수들을 도출하여 우선순위화 및 9개 항목을 집중적으로 관리하는 것이 필요함을 알 수 있었다. 세 번째, 6시그마 분석단계로 측정단계에서 도출된 9개 항목의 잠재원인변수 중에서 주요품질특성에 가장 영향을 미치는 핵심원인변수를 선별하기 위하여 현장실사, 브레인스토밍, 문헌고찰, 설문조사에 의해 분석하였다. 분석결과 5개 항목의 핵심원인변수를 도출하였다. 네 번째, 6시그마기법 개선단계로 주요품질특성이 목표를 달성할 수 있도록 선정된 핵심원인변수의 최적 개선안을 도출하였다. 도출된 최적 개선안은 간호사 2인 기준 심폐소생업무 심실세동 프로토콜 개발, 개발된 심실세동 프로토콜을 근거로 교육용 동영상 제작, 수기 작성 병동 CPR 간호기록지와 점검표의 표준화를 개선안으로 수립 및 실행하였다. 도출된 개선안에 대하여 전문가 검증을 거쳐 수정 및 보완하였다. 전문가 검증을 거쳐 최종 심실세동 프로토콜을 확정하였으며, 대상 병원의 병동 간호사들에 의해 심실세동 프로토콜의 적절성이 평가되었다. 최종 심실세동 프로토콜을 토대로 시나리오 작성 및 동영상을 촬영하였으며, 전문가 타당도 검증 및 수정을 거쳐 최종 교육용 동영상을 제작하였다. 그리고 문헌고찰을 통해 수기 작성 병동 CPR 간호기록지와 병동 응급약물 및 응급물품 점검표를 표준화하였으며, 전문가 타당도 검증 및 수정을 거쳐 표준화하였다. 최종 확정된 개선결과를 검증하기 위하여 파일럿 테스트 실행범위를 설정한 결과 교육용 동영상을 이용하여 간호사 대상 교육을 통해 간호사의 역할 및 업무분담의 수행, 간호사의 지식 · 수행능력, 간호기록지 활용 3개 항목의 개선안에 대하여 파일럿 테스트 전 · 후 효과성을 파악하기로 연구자와 개선팀이 합의하였다. 교육용 동영상을 이용하여 2006년 9월 1일에 간호사 16명을 대상으로 교육을 실시하여 개선결과를 검증하였다. 다섯 번째, 6시그마기법 관리단계로 최종 병원차원의 승인을 얻기 위해 최종 확정된 프로토콜과 동영상의 효율적인 활용을 위해 관리 지표, 관리 한계, 관리 담당자, 관리 주기, 관리방법 및 이상 발생시 조치사항에 관한 관리계획서를 작성하였으며, 6시그마 기법 5단계 과정에서 이루어진 모든 데이터를 정리하였고, 내용, 알려야 할 대상과 방법이 포함된 문서작성과 개선팀의 개인 변화에 대한 설문조사를 실시한 후 6시그마 과제 진행을 종료하였다. 이상의 연구결과를 종합하면 다음과 같은 결론에 이른다. 본 연구에서 개발한 6시그마 기법을 이용한 심폐소생업무 심실세동 프로토콜과 동영상은 일관성 있는 교육 효과는 물론 간호사들에게 명확한 역할과 업무분담을 제시함으로써 업무의 중복을 피하고 실무의 표준을 향상시킬 수 있으리라 본다. 그리고 표준화한 수기 작성 병동 CPR 간호기록지는 심폐소생술 과정에서의 절차 및 처치에 대한 효율적인 기록과 심폐소생술 후 객관적인 성적 보고서로 활용될 수 있으며, 표준화한 병동 응급약물 및 응급물품 점검표는 정기적 점검으로 재고량 및 불량률을 사전에 예방하여 환자의 생존율 향상에 이바지 할 수 있다고 본다. 이와 같은 연구결과는 향후 의료분야에서의 6시그마 기법을 확대 적용하는 데 기여할 것으로 본다.

This study is action research aimed at solving problems of CPR focusing on customers’ opinions on scientific and statistical perspectives by applying the 6 Sigma Technique not to the manufacturing industry but to the medical industry from April 7 · September 8, 2007 with a view to enhancing the specialty of CPR. Therefore, the study conducted a phased analysis of the problems, developed a CPR task ventricular fibrillation protocol based on phased items to be improved, prepared a video material based on the protocol, and verified the validity of the contents and clinical validity. Then, the study confirmed the final CPR task ventricular fibrillation protocol and the video material, and evaluated their validity. The study analyzed collected data by using the Minitap 15 PC+ 1 month evaluation version program and descriptive data by using an affinity technique. The progress of the study included the following five steps. First, as a 6 Sigma Technique defining step, the study described the background and reasonable need of the 6 Sigma project selection, organized a QI quality improvement team to pursue the project, prepared a written project action plan, and registered the QI team. As a procedure of obtaining approval from the nurse department, the study prepared a Fish Bone Diagram for problems in CPR situations that was aimed at nurses, and conducted survey research aimed at nurses, residents, and professors. As a result of the Fish Bone Diagram, the study confirmed main items, and in order to fix the main items as main problems, the study confirmed the main problems in a total of 7 items as a result of analyzing surveys aimed at nurses and aimed at residents and professors, and summarizing the results. Out of the main problems in the 7 items, the study selected 4 problems with higher contingency and importance, and converted the 4 problems into the main quality features which are a core demand of nurses. Then, the study went through the approval for CPR task ventricular fibrillation protocol development as a 6 Sigma project by offering the main quality features of the 4 items as a basis. Second, as a 6 Sigma measurement step, the study confirmed measurable main problems in order to clearly define main problems withdrawn in the definition step, deduced potential cause variables which may affect the current 6 Sigma level measurement by DPMO and main quality features for the confirmed main problems, and determined items to give priority and manage intensively. In order to confirm measurable main problems, the study collected and analyzed CPR-related electronic mandatory records, and measured its current level of 6 Sigma, conducted survey research into and an analysis of knowledge and job performance ability of nurses, collected and analyzed handwritten CPR nursing charts and measured its current 6 Sigma level, identified and analyzed first aid medication and goods in a ward and measured its current 6 Sigma level, and identified the current CPR task process. Also, the study identified the need of deducing 14 potential cause variables which may be influential to main quality features of the selected 4 items, and prioritizing them, and intensively managing 9 items. Third, as a 6 Sigma analysis step, in order to select core cause variables most influential to main quality features out of potential cause variables of 9 items, deduced at the measurement step, the study conducted an analysis by due diligence, brainstorming, literature study, and survey research. As an analysis result, the study withdrew core cause variables of 5 items. Fourth, as a 6 Sigma Technique improvement step, the study withdrew the optimal improvement plans of core cause variables which were selected for main quality features to attain a goal. With the deduced optimal improvement plans, the study established and performed the development of the standard CPR task ventricular fibrillation protocol for 2 nurses, preparation of educational video material based on the developed protocol, and the standardization of a handwritten CPR nursing chart in a ward and a checklist, as improvement plans. The deduced improvement plans went through the verification, modification, and supplementation of professionals. The study confirmed the final CPR task ventricular fibrillation protocol through the professional review, and the validity of the protocol was assessed by nurses of a target hospital. The study prepared a scenario and videotaped it based on the final CPR task ventricular fibrillation protocol, and prepared the final educational video material through a validity test and the modification of professionals. Then, the study standardized the handwritten CPR nursing charts and the first-aid medication and goods checklist through literature study as well as through a validity test and the modification of the profession. In order to verify the final improvement results, the study established the scope of a pilot test. As a result, researchers and the QI team agreed to identify the efficiency before and after the pilot test for the improvement plans of 3 items, including the role and role sharing performance of nurses through nurse education with the use of an educational video material, the knowledge and job performance ability of nurses, and the utilization of a nursing chart. The study verified improvement results by offering education to 16 nurses on September 1, 2006 by using an educational video material. Fifth, as a 6 Sigma Technique management step, in order to obtain final approval from the hospital for the final protocol and the video material, the study prepared a written management plan for a management index, a management limit, a person in charge of management, a management cycle, a management method, and the solutions for an occurrence of abnormality for the efficient utilization of the protocol and the video material. Then, the study organized all data produced in the 6 Sigma Technique’s five steps, conducted survey research into the preparation of a document mentioning contents, the target to inform and the method, and the personal change of the QI team, and closed the 6 Sigma project. The study reached the following conclusions by summarizing the research results. The CPR task ventricular fibrillation protocol and the video material using the 6 Sigma Technique developed in this research are supposed to help nurses avoid duplicated jobs and improve the standard of fieldwork by proposing a clear role and role sharing to them as well as a consistently educational effect. Also, the standardized handwritten CPR nursing chart may be utilized in the ward as an objective performance report after CPR as well as an efficient record of the procedure and treatment of CPR, and the regular check of the standardized first-aid medication and good checklist may be a contributor to the enhancement of the survival rate of patients by preventing unnecessary inventories and a defection ratio. These results will contribute to performing an extended application of the 6 Sigma Technique in the field of medicine.
