이에 본 연구의 목적은 지식경영을 선도하는 일반기업들이 활용하고 있는 균형성과표(Balanced Scorecard)를 기반으로 중소병원에서 경영의사결정에 활용할 수 있는 균형성과지표를 도출하고, 이를 근거로 중소병원에서 미래 환경변화를 예측하고 경영성과를 시뮬레이션 하는데 활용할 수 있도록 시스템다이내믹스 모델(System Dynamics Model)을 개발하여 활용가능성을 검증하는데 있다.
연구의 진행은 병원경영성과와 관련된 선행연구들과 기존의 문헌을 고찰하고, BSC의 중소병원 활용에 대한 연구와 전문가들의 인터뷰 결과, 기존 통계자료 등을 통해 모델개발에 필요한 변수들의 영향력과 효과에 대한 정량적 분석을 시행하여 시스템다이내믹스 모델을 개발하였다. 개발된 연구모델의 검증을 위해 경영성과 관련자료 확보가 가능한 중소병원 3곳을 선정하여 모델의 타당성을 검증하였다.
본 연구의 주요 결과는 다음과 같다.
첫째. 본 연구에서 설정된 중소병원의 주요성과지표(Key Performance Indicators)는 균형성과표에 근거해 재무관점 지표 7개, 고객관점 지표 9개, 내부 프로세스 관점 7개 그리고 학습과 성장관점 7개를 도출하였다.
둘째. 중소병원 경영성과에 대한 시스템다이내믹스 모델을 개발 한 뒤, 모델의 타당성을 검증하기 위하여 모델의 건전성 검증인 TIME STEP에 대한 민감도 분석을 시행하였고, 모델의 객관성 검증을 시행하였다.
셋째. 모델의 검증 결과 사례 A,B,C 병원 3개의 검증결과 과거 실적자료(D patients, D income, D costs)와 모델의 시뮬레이션 결과(number of patients per month, monthly income, monthly costs)와의 비교에서 목적변수인 고객, 매출 및 지출이 추세는 물론 통계적 검증결과도 (사례 A,B,C병원 평균R2 고객: R2 = 0.850, 매출: R2 = 0.865, 지출: R2 = 0.849)유이하게 나타나 중소병원경영에서 모델로 활용하는 것은 적합하다고 판단된다.

The most famous system to support decision makers for medium and small sized hospitals would be the Balanced Scorecard (BSC) system. It is true that the well designed BSC system provide decision makers with valuable information, there are skeptical opinions that the system may not be effective for medium and small sized hospitals. The key performance indices(KPI's) of the BSC system does not directly tell about the future; the KPI's are just indicators over which certain level of analysis has to be made. However, the medium and small sized hospitals does not have the relevant managerial knowledge. For these reasons, this paper developed a system dynamics model based on the BSC. The system dynamics model can be used to predict future in a quantitative manner.
The system dynamics model consists of variables regarding number of patients, incomes, and expenses. It also include variables required for forecasting of KPI's, even though large portion of which is treated as exogenously. After formulation, three hospitals are selected for case studies with their detailed informations. The case studies show that the system dynamics model can cover the three different hospitals with the same model with different data, and shows a possibility of a generalized system dynamics model which can be applied to many medium and small sized hospitals.
This work can be interpreted as an attempt of a merge of the two techniques; BSC and system dynamics. Starting with a BSC system, the development of a system dynamics model can take advantages of the BSC information. Especially the BSC system shows what are the important variables for the system dynamics model. Since the actual running data of the BSC system are not available, the system dynamics model is tested only for financial data and number of patients.
The system dynamics model developed in this work is a conceptualized one, excluding the detailed characteristics of each hospital such as competing hospitals nearby, residents' statistics, etc. Therefore, the actual applications need more modeling processes. Still, this paper shows that a system dynamics model is expected not only to help owners of the medium and small sized hospitals, but also to provide guidelines regarding medium and small sized hospitals.
