This study was undertaken to develop a horticultural therapy (HT) program based on suitable treatment techniques and treatment components for dementia; to investigate effect of a developed HT program on the improvement of cognitive function and BPSD (Behavioral and psychological symptoms of dementia) in dementia; to examine regional 18FDG (F-18 fluorodeoxyglucose) uptake differences in both Alzheimer's disease (AD) and vascular dementia (VD) patients before and after HT. With 164 published papers related to dementia, the characteristics of dementia, non-pharmacological therapies, and treatment techniques were summarized. After summarizing and verifying previous studies, reminiscence was chosen as the best treatment technique for professional horticulture therapy. Injured performance components in early and mild stages of dementia were chosen as the treatment component from previous studies related to occupational therapy. The pertinent HT program sessions and times of each session were decided to maximize the therapeutic effect on dementia according to a previous study on meta-analysis in horticultural therapy. HT programs were planned around 40 minute sessions twice per week for 8 weeks. Finally, HT programs from previous research were divided into 4 categories: planting and growing, flower arrangement, craft, and other. After this division, the HT programs were counted from high to low frequency in each category. According to the developmental stages of lifetime in reminiscence, the subjects and activities of reminiscence, and horticultural activities related to the subjects of reminiscence were detailed. Sixteen horticultural activities were selected in turn for high frequency. Each horticultural activity selected was divided into sensory and behavioral experiences, and was contained treatment components in each HT program as much as possible through an analysis of the activity. Effect of a developed horticultural therapy program on the improvement of cognitive function and BPSD (Behavioral and psychological symptoms of dementia) in dementia was investigated. In this study, 15 dementia patients participated at a municipal hospital and were assigned to two groups: 10 patients participated in a horticultural therapy program with drug intervention as a drug and horticultural therapy treatment (DHT) group, and 5 patients were chosen for a drug-only treatment (DT) group. The DHT group attended the developed horticultural therapy program 2 times per week for 8 weeks. To evaluate the effect of the horticultural therapy program, the Seoul Neuropsychological Screening Battery (SNSB) was applied, and anti-dementia and BPSD-related medications were investigated at 2 different times; at the first and the second test with 8 week interval for both the DHT and DT groups. Also, behavioral analysis via video recording was used to verify cognitive function and BPSD trends according to the progress of the horticultural therapy sessions for the DHT group only. When a comparison was made between the first and second tests in both the DT and DHT groups, the DT group displayed a significant increase in the SVLT free recall part of memory category and depression of SNSB, with a significant decrease in the visuospatial function category of SNSB. Doses of anti-dementia medication were increased between the first and second test in the DT group. However, the DHT group showed an increase in almost all categories of SNSB and significant improvement in the visuospatial function category, SVLT free recall and the rey copy recognition of memory category of SNSB, and the total score of the SNSB-D. In a behavioral analysis via video recording, the DHT group showed gradual improvement in attention, language and related functions, memory, other cognitive functions, and BPSD according to the progress of the horticultural therapy sessions although the orientation category did not show any change. Behavioral changes also displayed similar trends of improvement in cognitive functions with SNSB between the first and second test in the DHT group. The doses of anti-dementia medication were not changed and the BPSD-related medication was similar or decreased between the first and second test in the DHT group. Regional 18FDG (F-18 fluorodeoxyglucose) uptake differences in both Alzheimer's disease (AD) and vascular dementia (VD) patients before and after horticultural therapy were examined in this study. Horticultural therapy programs were performed every weekday for 14 days. An 18FDG-PET image of the AD patient revealed a reduced regional FDG uptake in the left parieto-temporal area as usual in traditional AD patients before horticultural therapy. A comparison of statistical parametric mapping (SPM) analyses of the 18FDG-PET images before and after horticultural therapy in the AD patient was done. SPM analysis showed no significant change despite an increasing tendency in the left parieto-temporal area. On the contrary, the VD patient displayed reduced regional 18FDG uptake in the bilateral multiple fronto-temporal-parietal area before horticultural therapy. While 18FDG uptake displayed an increasing tendency in the bilateral multiple fronto-temporal-parietal area after horticultural therapy in the VD patient, SPM analysis of the brain PET images showed no significant change despite an increasing tendency in the bilateral multiple fronto-temporal-parietal area after horticultural therapy. 18FDG uptake was gradually decreased according to the progress of dementia. The AD and VD patients in this study, however, showed an increasing tendency of glucose metabolism in the reduced 18FDG uptake regions after horticultural therapy.
