과거에는 전문가나 지식인에게 독점되었던 의료정보가 현대사회의 다양한 대중매체를 통해 대중에게 전해지고 있다. 이에 따라 의료소비자는 치료뿐만 아니라 진료환경에도 관심을 갖게 되었고, 특히 진료실 내의 감염관리에 많은 관심을 갖게 되었다. 이와 더불어 감염관리에 관한 인식이 변화하였고 자기안전에 대한 기대와 요구수준이 높아져 이를 적극적으로 평가하기에 이르렀다. 따라서 의료기관은 일방적으로 행하는 감염관리 수준에서 벗어나, 의료소비자인 환자의 기대에 부응하는 감염관리에 대한 노력이 필요하다. 따라서 본 연구의 목적은 치과에 내원하는 환자를 대상으로 치과감염관리에 관한 인식과 요구정도를 조사하여 환자가 이를 평가하는 기준을 제시함으로써, 의료소비자인 환자가 만족하고 신뢰할 수 있는 감염관리를 행하는 한편, 치과의료진에게 치과경영의 홍보 전략으로 내세울 수 있는 기초자료를 제공하고자 시도하였다. 이에 본 연구는 2008년 9월 18부터 2008년 11월 8일까지 서울, 경기, 충청지역에 소재한 치과의원 13곳에서 진행하였으며, 대상자는 내원한 환자 중에서 설문에 동의한 자로 18세 이상의 성인 714명이다. 연구도구는 구조화된 설문지이고 조사는 자기기입식 또는 면접조사로 수행하였으며, 연구결과를 다음과 같이 요약하였다. 1. 응답자의 성별은 남자가 43.0%, 여자는 57.0%이었고 연령은 20대 이하 32.1%, 30대 26.8%, 40대 22.3%, 50대 12.3%, 60대 이상은 6.6%이었다. 거주지별 분포는 특별 및 광역시 36.8%, 중소도시 48.0%, 농촌지역 15.1%이었고 결혼여부는 기혼자가 62.3%, 미혼자는 37.7%이었다. 학력은 대졸이상과 고졸이 각각 62.3%, 32.5%이었고 중졸이하는 5.2%이었다. 월평균소득은 200만원 이하가 33.2%로 가장 높은 비율을 보였고, 직업은 전문직 및 기술직이 21.6%로 가장 높은 비율이었다. 2. 환자가 치과를 선택할 때 고려하는 사항들을 비교한 결과, ‘감염관리(청결 및 위생관리)(3.74점)’, ‘친절하고 설명을 잘하는 의료진(3.61점)’, ‘현대적 시설 및 장비(3.55점)’, ‘치과의료진의 경력 및 인지도(3.52점)’ 순으로 나타나 감염관리가 치과선택에 중요한 변수임을 알 수 있었다. 3. 치과를 선택할 때, 남자보다 여자가 감염관리를 더 중요시 하는 것으로 나타났으며(p<0.001), 또한 1~2회 방문한 환자의 경우, 감염관리를 더 중요시하는 경향이 있었다. 4. 치과의 감염관리 책임소재에 관하여 응답자의 81.2%가 ‘모든 치과의료진’(치과의사, 치과위생사, 간호조무사 등)의 책임이라고 인식하였다. 5. 응답자에게 신뢰를 주는 진료복의 종류는 ‘상·하의 유니폼(52.2%)’, ‘흰색의사가운(28.3%)’, ‘상의유니폼·하의평상복(12.1%) 순이었고, 6.6%는 진료복의 종류와 상관없다고 하였다. 반면 이를 연령에 따라 분석한 결과, 60대 이상은 ‘흰색의사가운(44.7%)’, ‘상·하의 유니폼(31.9%)’ 순으로 응답하여 전체적인 선호도와 차이가 있음을 알 수 있었다(p<0.04). 6. 81.2%의 응답자가 명찰을 착용한 치과의료진에게 신뢰감이 더 생긴다고 하였고, 또한 94.1%는 의료진의 단정한 차림새(청결한 진료복, 머리모양 등)가 신뢰감 형성에 영향을 준다고 하였다. 7. 치과의료진이 보호장구를 착용하는 이유에 대하여 ‘치과의료진 및 환자를 보호하기위한 것(77.6%)’, ‘환자를 보호하기위한 것(13.4%)’, ‘치과의료진을 보호하기위한 것(9.0%)’ 순으로 인식하고 있었다. 8. 보호장구의 필요성에 대한 환자의 인식을 조사한 결과 전반적으로 마스크는 응답자의 93.6%가, 장갑은 88.1%, 보안경은 47.1%가 치료 시, 꼭 착용해야 한다고 하여 마스크와 장갑의 필요성을 높이 인식하고 있었다. 또한 장갑의 경우, 연령에 따라 차이가 있었는데 20대 이하가 50대 또는 60대 이상에 비하여 장갑의 필요성을 높게 인식하고 있었다(p<0.001). 9. 응답자의 41.7%가 치과의료진이 진료를 시작하기 전에 손을 씻는지 살펴본다고 하였다. 이를 일반적 특성에 따라 분석한 결과, 남자보다 여자가 더 많이 살펴본다고 하였고(p<0.001), 연령에서는 60대 이상이 가장 많이 살펴본다고 하였으며(p<0.01), 직업에서는 주부가 가장 많이 살펴본다고 하였다(p<0.001). 9, 사용한 장갑의 교체여부에 관한 환자의 인식을 조사한 결과, 응답자의 83.1%가 진료를 시작하기 전, 매번 장갑을 교체해야 한다고 인식하였다. 11, 응답자의 47.5%가 치과감염관리에 관한 정보를 접한 적이 있고, 그 중에서 78.2%가 공중파 방송을 통해서 정보를 접하였다. 또한 정보를 접한 응답자 중, 54.3%가 기구의 소독상태를 예전보다 유심히 살핀다고 하였고, 35.7%는 치과를 선택할 때, 감염관리를 고려한다고 하였다. 결론적으로, 감염관리는 치과선택 시 대부분의 환자에게 중요한 변수로 작용한다. 따라서 감염관리를 잘 시행한다는 치과의 이미지를 부각시키고 환자가 만족하는 감염관리 절차를 제공할 때, 환자는 그 치과를 신뢰하고 다시 내원할 가능성이 많아질 것이다. 또한 치과감염관리에 관한 환자의 인식이 전반적으로 높게 나타난 만큼 치과의료진의 관심정도와 실천 또한 변화해야 하는데 이를 변화시키기 위한 효과적인 방법은 환자의 인식과 요구정도를 조사하고 평가하여 이를 실무에 반영하는 것이라고 여겨진다. 따라서 치과감염관리에 관한 환자의 인식을 알아보기 위한 지속적인 연구가 필요할 것이며 이는 궁극적으로 감염관리에 대한 치과의료진의 관심과 실천을 증대시킬 것이다.

Accordingly, medical consumer’s awareness and perception about infection control performed in the clinic is heightened. They expect and demand more for self-protection and come to actively assess the quality of infection control. In this sense, patients visiting dental office were surveyed of their perception and demand about infection control in the dental health-care settings in this study. This purpose of this study is to investigate information of patient’s perception about infection control in dental settings. By providing this information, dental personnel can offer more satisfying quality of infection control to patients and it can be used as a marketing strategy. Survey was performed on 714 subjects aged 18 years or older who visited 13 dental offices located in Seoul, Gyoung-gi, and Choong-chung area from Sep.18 to Nov.8 in 2008. Research too l was a structured questionnaire and it was filled-in by respondents or interviewed and followings were the results. 1. Respondents consist of 43.0% of men and 57.0% of women. 32.1% were aged 20 years or younger, 26.8% were in their 30s, 22.3% were in their 40s, and 12.3% were in their 50s. Concerning residential locations, 36.8% were from Seoul or metropolitan cities,48.0% were from mid and small-sized city, and 15.1% were from rural areas. Married persons were 62.3% and unmarried were 37.7%. Regarding education, high school and college graduates were 62.3% and 32.5%, respectively, and those with less than middle school education were 5.2%. Average monthly income of 2,000,000 won or less took the largest proportion with 33.2%. Professionals or tech workers were more abundant than any other occupations with 21.6%. 2. When factors affecting patient’s selection of dental offices were compared, ‘infection control(cleanness and hygiene) of the office’ was 3.74 points, ‘caring health-care personnel who kindly explains’ was 3.61 points, ‘modern facility and equipment’ was 3.55 points, and ‘dentists’ experience’ was 3.52 points. 3. When choosing a dental clinic, women and patients with one or two previous visits tended to care more about infection control status than men(p<0.001). 4. 81.2% of respondents replied that ‘dentists and dental health-care personnel’ were responsible for the infection control. 5. With regard to respondent's preference to dental personnel's uniform, 52.2% preferred to 'uniforms of both upper garment and pants or skirts' followed by 'long white jacket over clothing(28.3%)’, and ‘upper uniform with ordinary pants or skirts(12.1%) and 6.6% didn’t care about type of clothing. Concerning relationship between age and the preference, patients in their 60s and older preferred to 'long white jacket over clothing(44.7%)' followed by 'uniforms of both upper garment and pants or skirts(31.9%)' showing different preference(p<0.04). 6. 81.2% of respondents more trusted dental personnel with name tag and 94.1% conceived dental personnel's appearance clean and confident when their appearance(clean uniform, hairdo) was neat. 7. With respect to reasons of wearing personal protective equipment by dental health-care personnel, ‘to protect health-care personnel and patients(77.6%)’, ‘to protect patients(13.4%)’, ‘to protect dental health-care personnel(9.0%)’ were the responses. 8. With regard to necessity of patient’s perception about protective equipment, they replied that mask(93.6%), gloves(88.1%), and protective eye-wear(47.1%) should be worn during the treatment. Mask and gloves were generally recognized as a mandatory hygienic equipment to be worn. And with respect to gloves, perception was different depending on the age. Respondents in their 20s and younger cared more about dental personnel's wearing of gloves than those in their 50s and older(p<0.001). 9. 41.7% of respondents replied that they checked whether dental health-care personnel wash their hands before starting the treatment. Analysis of relationship between this result with general characteristics showed that women (p<0.001), respondents in their 60s and older (p<0.01), and housewives(p<0.001) cared more. 10. Regarding patients’ perception of changing used gloves, 83.1% replied that it should be change whenever they see a new patients. 11. 47.5% of the respondents heard about information of infection control in dental office and 78.2% of them encountered by airwaves. 54.3% of respondents who heard about the information more carefully watched sterilization status of the equipment. 35.7% considered the status of infection control when choosing the dental office. In conclusion, infection control is an important factor for patient when selecting dental office. Therefore, promoting the image of the dental office with the emphasis on strictly abiding by infection control protocol and putting it into practice, possibility that patients will trust and visit the office again will increase. As patient's concern of infection control in dental health care-settings found to be grave, dental health-care personnel's perception and behavior need to be changed. Surveying and assessing patient's perception and demands and reflecting the result to the clinic is considered to be an efficient way to change this. Further researches are required to continuously investigate patient's perception of infection control in dental office and these will eventually increase dental health-care personnel's interest and action about infection control.
