This study is a descriptive survey aiming to identify postpartum depression and maternal identity of high risk postpartum women who gave birth to their child even from high risk pregnancy, and determine potential associations between postpartum depression and maternal identity in those women. This study used a questionnaire form to survey total 59 postpartum women for data analysis. They were hospitalized on the 1st day to 7th day after giving birth to their child even from high risk pregnancy at G General Hospital and other neighboring local 2 maternity clinics from Oct. 13 to Nov. 21, 2008. For survey tools, this study adopted an adapted version of BDI (Beck Depression Inventory; by Lee & Song, 1991) that was originally designed by Beck (1967) to measure postpartum depression. In order to measure maternal identity, this study adopted total 17 question items to measure general meanings of role identity used by Kim(1996). Collected data were processed using SPSS WINDOWS 12.0, a statistic program, and were also analyzed using real number, percentage, mean and standard deviation, analysis of variance(ANOVA), t-test, post-test (Scheffe's test), Pearson's correlation coefficient and Cronbach's alpha coefficient. The results of this study can be outlined as follows: 1. According to diagnosis of diseases associated with high risk pregnancy of our high risk postpartum women, 21 women(35.6%) were 35 years old or older, and our high risk postpartum women showed a variety of clinical cases such as gestational hypertension 8 women(13.6%), abnormal ultrasonography findings and critical diseases affecting pregnancy(e.g. diabetes, hypertension, thyroid disease, cardiopathy, renal disease, autoimmune disease, etc) 5 women(8.5%), preterm labor 4 women(6.8%), premature membrane rupture 3 women(5.1%), gestational diabetes and incompetent internal os of cervix(IIOC) 1 women(1.7%), two findings 14 women(23.7%), three findings 6 women(10.1%) respectively. 2. According to measurement using BDI, high risk postpartum women scored 9.27(±5.65) points on the average in their postpartum depressison. In view of the distribution of moderate postpartum depressison, it was found that more than half of respondents 34 women (57.6%) had normal mental conditions(0∼9 points), which was followed by light depression group(10∼15 points; 17 women, 28.8%), moderate depression group(requiring psychologic therapy)(16∼23 points; 7 women, 11.9%), and severe depression group(24 or higher points, 1 women, 1.7%) respectively. 3. According to measurement of the maternal identity of high risk postpartum women on the basis of semantic differential scale, all women respondents scored 69.1(±11.09) points on the average in maternal identity and their itemized scores averaged 4.06(±0.65) points out of full 5. These findings indicate that high risk postpartum women have relatively high maternal identity. 4. There were some general characteristics as a good function of significant differences in postpartum depression of our respondents, such as emotional conditions during pregnancy(F=7.68, p=.001), husband's assistance in housework and his emotional supports (F=7.02, p=.001) and marital satisfaction(F=6.91, p=.001). It was found that marital satisfaction(F=7.37, p=.001) was a critical determinant of maternal identity. 5. There were some obstetrical characteristics associated with significant differences in postpartum depression, such as the number of delivery by Caesarean section(F=5.69, p=.01) and experiences in hearing of information on prenatal high risks concerning high risk pregnancy(t=2.29, p=.05). The number of delivery by Caesarean section(F=7.29, p=.01) was a critical determinant of maternal identity. 6. Postpartum depression was in significantly negative correlations with maternal identity(r=-.595, p=.001). Conclusively, it is found that high risk postpartum women tend to show relatively serious postpartum depression, and the higher postpartum depression they feel is associated directly with the lower maternal identity they have. Based on these findings, it is expected that early detection of signs on postpartum depression of high risk postpartum women and positive preventive care of their depressive tendencies will contribute to improving their maternal identity.
