The present study is a methodological research to standardize the peri- operative nurses’ counting procedures and protect surgical patients from the risk of retained foreign bodies, and ultimately, to improve the quality of perioperative nursing through the development of a guideline for preventing count errors in operating room. Collected data were analyzed using SPSS PC 14.0, and the study was conducted through five stages as follows. First, in the stage of analyzing problems related to count errors in operating room, before drafting a preliminary guideline, the researcher conducted a questionnaire survey on risk factors and risk situations related to count errors in operating room with 42 perioperative nurses of C University Hospital in G Metropolitan City from the 23^(rd) to 27^(th) of June, 2008. Second, in the stage of deriving preliminary items of the guideline, the researcher reviewed domestic and foreign literatures from the 5^(th) of July to the 1^(st) of August, 2008, and had interviews on experiences in counting procedures with 8 perioperative charge nurses of C University Hospital in G Metropolitan City from the 11^(th) to 14^(th) of August, 2008. Based on the results of a previous questionnaire survey, literature reviews and nurse interviews, the preliminary guideline with a total of 161 items was derived, and then, it was categorized into ‘counting time’ and ‘counting items.’ The researcher with two clinical experts, from the 25^(th) to 26^(th) of August, 2008, refined the preliminary guideline, and as a result, it came to have a total of 142 items. Third, in the stage of testing the content validity of the preliminary guideline, the researcher formed a clinical expert group consisting of 13 perioperative nurses of C University Hospital in G Metropolitan City, and tested the ‘adequacy’, ‘applicability’ and ‘effectiveness’ of the 142 items in the preliminary guideline from the 1^(st) to 5^(th) of September, 2008. According to the results of content validity test by expert group, the mean CVI of the preliminary guideline was .98 for adequacy, .95 for applicability, and .98 for effectiveness. The researcher refined or deleted 4 items that received CVI under .80 in all of the three evaluation areas (adequacy, applicability, and effectiveness), and as a result, the preliminary guideline came to have 139 items. Fourth, in the stage of testing the clinical validity of the revised preliminary guideline, the guideline was applied to clinical practice by perioperative nurses of C University Hospital in G Metropolitan City from the 22^(nd) to 26^(th) of September, 2008. The application rate of the items in preliminary guideline was relatively high as 84.9% on the average, and there was no item the application rate of which is 0.0%. Therefore, with no more modification, the researcher decided to adopt every 139 items of the preliminary guideline according to the results of application rate. The most frequent reasons for not applying the preliminary guideline were ‘lack of personnel’ and ‘busy work(lack of time).’ In the general evaluation of the preliminary guideline, ‘Does the guideline contain sufficient information for preventing count errors in operating room?’ got the highest score (3.60±0.51) and ‘Is the guideline suitable for the work environment of operating room?’ got the lowest score (3.00±0.76). Fifth, in the stage of finalizing the guideline, the researcher finalized the guideline with a total of 139 items based on the results of applying the revised preliminary guideline to clinical practice. The guideline developed in this study can be used for perioperative nurses to perform the counting procedures accurately and for new nurses and nursing students in operating room to learn the counting procedures. Furthermore, as a methodological research for guideline development, this study is expected to contribute to promoting research for developing guidelines related to perioperative nursing.
