본 연구의 목적은 사회적 지지가 편마비환자를 돌보는 가족의 부담감에 미치는 영향에 대해 알아보는 것이다. 이를 위해 서울지역 2개 대학병원 및 4개 재활병원에서 간병을 하고 있는 가족을 대상으로 직접조사 방법으로 180부의 설문지를 배포하였고 이를 토대로 SPSSWIN 12.0 프로그램으로 기술분석, t-테스트와 일원변량분석,피어슨 상관관계분석, 다중회귀분석 사용하여 분석하였다. 본 연구의 주요 결과는 다음과 같다. 첫째, 사회적 지지의 평균점수 중 정서적 지지의 평균점수가 43.47(SD=7.723)으로 높은 응답분포로 나타났고 사회환경적 지지 점수가 35.86(SD=5.098)의 순으로 나타났다. 가족 부담감의 사회기술은 정서적 부담감이31.90(SD=5.823),으로 가장 높은 응답분포로 나타냈고 그 다음으로 시간의존적 부담감의 평균점수가 15.00(SD=2.657), 경제적 부담감의 평균점수가 11.58(SD=2.366), 사회적 부담감의 평균점수가 10.75(SD=2.084), 신체적 부담감의 평균점수가 7.08(SD=1.830)의 순으로 나타났다. 둘째, 응답자의 일반적 특성에 따른 가족의 부담감의 차이를 살펴보면 응답자의 성별, 연령, 결혼상태, 직업, 예후에 따른 부담감의 차이는 크게 유의미하게 나타나지 않았으나 보호자의 월 평균 수입, 환자와의 동거여부, 환자의 질병상태, 친지나 주변으로부터 받은 도움의 정도에서는 부담감의 차이가 유의미하게 나타났으며 환자와의 동거여부 및 질병상태, 친지나 주변으로 부타 받은 도움의 정도에 따른 부담감의 차이에서도 유의미한 차이를 보였다. 환자의 일반적 특성에 따른 가족의 부담감의 차이를 살펴보면 환자의 성별이나, 연령, 결혼상태, 입원전 수입에 따른 부담감의 차이는 크게 유의미하게 나타나지 않았으나 환자의 학력, 가정에서의 부양책임, 치료받은 기간에 따른 부담감의 차이는 통계적으로 유의미하게 나타났다. 셋째, 사회적 지지와 부담감간의 상관관계를 살펴보면 사회적 지지와 부담감의 5가지 영역 즉 사회적, 경제적, 신체적, 정서적, 시간 의존적 부담감의 영역에서 모두 부(-)적인 관계를 보였다. 이는 사회적 지지가 높을수록 가족의 부담감이 적어진다는 것을 의미한다. 이러한 연구결과를 토대로 사회적 지지와 가족부담감간의 함의를 살펴 보면 다음과 같다. 첫째, 응답자의 일반적 특성에 따른 보호자의 부담감 차이를 알아본 결과 가족의 월 평균 수입, 환자와의 동거여부, 환자의 질병상태, 친지나 주변으로부터 받은 도움의 정도가 부담감에 많은 영향을 미친다는 것을 알 수 있었다. 이는 경제적인 부분(월평균수입, 가족이나 친지의 도움), 정서적인 부분(동거여부), 의료적인 부분(환자의 질병상태)등 전반적인 부분에 있어서 가족들이 부담감으로 느끼고 있다는 것을 의미하며 이들에게 개입함에 있어서 통합적인 접근이 필요함을 의미한다. 둘째, 환자의 일반적 특성에 따른 보호자의 부담감 차이를 알아본 결과 환자의 학력, 가정에서의 부양책임, 치료받은 기간이 가족들의 부담감에 영향을 미친다는 것을 알 수 있었다. 이는 환자의 경제상태보다는 사회환경적 요인이 부담감에 더 많은 영향을 미친다는 것을 의미하며 따라서 환자가족에게 개입함에 있어서 사회 환경적인 부분에서 개입을 해야 할 필요성이 있다. 셋째, 사회적 지지와 부담감간의 상관관계를 알아본 결과 사회적 지지가 사회적, 경제적, 신체적, 정서적, 시간 의존적 부담감등 모든 부담감 영역에 영향을 미치는 것을 알 수 있었다. 따라서 환자 가족에게 개입함에 있어서 특정 부담감의 영역이 아닌 모든 부담감 영역에 걸쳐서 개입을 해야 할 필요가 있다.

The purpose of this study was to investigate the impacts of social support on the burden of families taking care of hemiplegia patients. For that purpose, 180 questionnaires were distributed to families taking care of their ill family members at two university hospitals and four rehabilitation hospitals in Seoul. Using the SPSSWIN 12.0 program, gathered data were analyzed in descriptive analysis, t-test, one-way ANOVA, Pearson's correlation analysis, and multiple regression analysis. The main research findings were as follows: Firstly, the mean scores of emotional support were 43.47(SD=7.723), which marked a high response distribution and were followed by environmental support of 35.86(SD=5.098) points. As for social description of family burden, emotional burden recorded the highest response distribution at 31.90(SD=5.823) being followed by time-dependent burden at 15.00(SD=2.657), economic burden at 11.58(SD=2.366), social burden at 10.75(SD=2.084), and physical burden at 7.08(SD=1.830) in the order. Secondly, the differences in family burden were examined according to the general characteristics of the respondents. There were no significant differences in burden according to gender, age, marital status, vocation, and prognosis. However, significant differences were found in burden according to the monthly income of the caregiver, living with the patient, the patient's condition, and degree of help from relatives and acquaintances. As for differences in family burden according to the general characteristics of the patients, there were no significant differences in burden according to gender, age, marital status, and income before hospitalization. However, there were statistically significant differences in burden according to educational background, family responsibility, and treatment period of the patients. Thirdly, as for correlations between social support and burden, there were negative(-) correlations between social support and all the five areas of burden including social, economic, physical, emotional, and time-dependent burden, which means that the more social support they receive, the less they feel burdened. Those research findings have the following implications for social support and family burden: Firstly, the differences in the burden of the caregiver according to the general characteristics of the respondents indicate that burden is under the huge influence of family's monthly income, living together with the patient, the patient's condition, and degree of help from relatives and acquaintances. The result means that the families felt burdened in all the aspects including economy(monthly income and help from family and relatives), emotion(living together with the patient), and medicine(the patient's condition) and that an integrative approach is required for intervention for them. Secondly, the differences in the caregiver's burden according to the general characteristics of the patients reveal that family burden is under the huge influence of the patient's educational background, family responsibility, and treatment period. The result means that the patient's socio-environmental factor has more impacts than his or her economic state factor and that interventions are needed in socio-environmental aspects for the families. And finally, the correlations between social support and burden demonstrate that social support affects all the areas of burden including social, economic, physical, emotional, and time-dependent burden, which suggests that interventions for the families should be made in all the areas of burden instead of some.
