본 연구는 서울시 지역주민을 대상으로 흡연, 음주, 운동, 비만 여부 및 인구‧ 사회경제적 특성이 삶의 질(EQ-5D)에 미치는 영향을 분석하기위해 시도되었다. 본 연구의 조사지역은 서울시 S구이며, 전체 17개동에서 만 19세 이상인 남녀 주민을 층화집락추출법을 통해 표집 된 1,234명을 대상으로 하는 면접식 횡단적 조사연구로 수행하였다. 건강행태 중 흡연은 ‘피움’, ‘현재금연’, ‘피운적없음’으로 분류하였으며, 음주는 알코올사용장애척도(Alcohol Use Dis-orders Identification Test: AUDIT)를 이용해 ‘정상음주자’, ‘문제음주자’, ‘알코올의존자’로 분류하였으며, 운동의 경우 ‘불규칙한 운동’과 ‘규칙적인 운동’으로 분류하여 규칙적인 운동의 기준은 일주일에 3번 이상 30분 이상 운동하는 경우를 기준으로 하였다. 체질량지수(BMI)는 대한비만학회의 분류에 따라 ‘과소체중’, ‘정상체중’, ‘과체중’, ‘비만’으로 분류하였으며, 삶의 질을 평가하기 위한 도구 EuroQoL 5 Dimensional Classifica- tion(EQ-5D)을 이용하였으며, 주요 결과는 다음과 같다. 첫째, 대상자의 흡연, 음주, 운동, 비만에 대해 분석한 결과 현재 흡연율은 전체 대상자의 18.9%(남성 40.8%, 여성 2.8%)이었으며, 음주는 전체대상자 중 문제음주자는 21.5%(남성 36.7%, 여성 9.8%)로 남성에서 높게 나타났으며, 이를 세분화하여 제시할 경우에 11.2%(남성 21.9%, 여성 3.0%)가 알코올의존자인 것으로 나타났다. 규칙적인 운동 비율은 절반이 넘는 53.4%(남성 51.4%, 여성 55.1%)로 남성에 비해 여성이 규칙적 운동을 하고 있는 것으로 나타났다. 비만은 21.9%(남성 28.3%, 여성 16.9%)인 것으로 나타나 남성의 비만 비율이 높은 것으로 나타났다. 둘째, 흡연, 음주, 운동, 비만 여부 및 인구‧사회경제적 특성에 따른 삶의 질(EQ-5D) 5개 차원에 대한 각 영역별 건강상태를 비교 분석한 결과 흡연에서 남성은 ‘현재금연’인 경우 통증과 불편(24.1%)을 호소하는 비율이 높게 나타났으며, ‘현재흡연’인 경우 불안과 우울(18.5%)을 호소하는 비율이 높게 나타났다. 여성은 ‘피움’인 경우 통증과 불편(47.4%) 및 불안과 우울(47.4%)을 호소하는 비율이 높게 나타났다. 음주는 알코올의존자인 경우 남성과 여성 모두에서 통증과 불편(남성 24.6%, 여성 25%)및 우울과 불안(남성 21.9%, 여성 45%)을 호소하는 비율이 높게 나타났으며, 특히 여성인 경우 문제 음주자인 경우에도 불안과 우울(31.8%)을 호소하는 비율이 높게 나타났다. 운동여부에 따라서는 불규칙한 운동인 경우 여성은 운동능력(12.4%), 남성은 불안/우울영역(12.4%)에 문제가 있는 것으로 나타났으며, 규칙적인 운동인 경우 통증과 불편(남성 21.7%, 여성 26.3%)을 호소하는 비율이 높게 나타났다. 비만 여성인 경우 운동능력(21.6%), 통증/불편(33.3%), 불안/우울(23.9%)영역에 문제가 있는 것으로 나타났으며, 비만 남성의 경우 통증과 불편(21.2%)을 호소하는 비율이 높게 나타났다. 셋째, 흡연, 음주, 운동, 비만 여부 및 인구‧사회경제적 특성에 따른 삶의 질 지수(EQ-5D index)를 비교 분석한 결과 흡연은 남성의 경우 유의한 차이는 없었으나, 현재금연(95%)인 경우 삶의 질이 낮게 나타났으며, 여성의 경우 현재 흡연을 하는 경우 유의하게 삶의 질이 낮게 나타났다(p<.0001). 음주수준에 따라서는 알코올의존자인 경우(p<.0068), 운동여부에 따라서는 유의한 차이는 없었으나, 불규칙한 운동(남성 94%, 여성 95%)을 하는 경우 규칙적인 운동에 비해 삶의 질이 낮은 것으로 나타났다. 비만은 남성의 경우 유의한 차이가 없었으나, 여성은 비만할수록 유의하게 삶의 질이 낮은 것으로 나타났다(p<.0003). 인구사회학적 특성을 분석한 결과 남성은 여성에 비해(p<.0058), 연령대가 낮을수록(p<.0001), 미혼인 경우(p<.0001) 삶의 질이 유의하게 높은 것으로 나타났으며, 자가 주택에 거주할 경우 유의한 수준은 아니었으나 삶의 질이 높게 나타났다. 반면, 교육수준이 낮을수록(p<.0001), 소득수준이 낮을수록(p<.0001), 기타/무직/주부인 경우(p<.0001) 삶의 질이 유의하게 낮게 나타났다. 이상의 연구결과에서 인구사회학적 특성별로는 여성이 남성보다, 연령이 증가할수록, 교육수준과 소득수준이 낮을수록, 기타 또는 월세에 주거하는 경우, 사별/ 이혼/별거, 기타/무직/주부인 경우에 지역사회 건강증진을 위한 중재 프로그램을 실시할 때 우선순위 대상자로 선정되어야 한다. 건강행태 별로는 흡연과 음주의 경우 남성과 여성 모두에서 불안과 우울을 호소하는 비율이 높게 나타났으며, 여성의 경우 통증과 불편을 호소하는 비율 또한 높게 나타났으므로, 불안과 우울을 감소시키기 위한 프로그램 개발 및 여성에 대한 흡연예방 교육 시 흡연이 통증과 불편, 불안과 우울과의 상관관계가 있음을 강조해야할 필요가 있다. 규칙적인 운동을 할 경우 통증과 불편을 호소하는 비율이 높은 것으로 나타나, 질환 유무 및 개인의 특성에 따른 올바른 운동에 대한 교육 및 처방을 해주는 프로 그램이 요구된다. 마지막으로 비만은 현재 전 세계적으로 관심을 가지고 있는 국민건강증진사업의 주요 부분이며, 우리나라에서는 비만과 관련된 대사증후군(Metabolic Syndrome)

The purpose of this study is to evaluate the health related quailty of life (HRQoL) and to identify smoking, drinking, exercise, obesity and sociodemo -graphic variables are related factors for general population in Seoul. The data for this cross-sectional study with face-to-face interview were obtained form 1,234 respondents aged over 19 in male and female residents sampled through a cluster-stratified sampling method in one district of Seoul(A total of 17 dongs in S-gu). Smoking variables classified as Smoking, Ex-smoking and None-smoking, drinking variables classified as Normal drinker, Problem drinker and Alcohol dependent were utilized by Alcohol Use Disorders Identification Test(AUDIT) and also exercise variables classified as Irregular exercise and Regular exercise, however regular exercise defined as the three or more times a week. 30minutes was based on a case. BMI(body mass index) classified as Under -weight, Normal weight, Overweight and Obesity according to Obesity Classification of the Korean Obesity Society. The health related quality of life was measured using the EQ-5D instrument and EQ-5D index scores were calculated by Nam's model which the main results were as follows. First, the results of analyzed data according to the smoking, drinking, exercise and obesity that current smoking rate was 18.9% of total respondents(male 40.8%, female 2.8%), problem drinker rate was 21.5% of total respondents(male 36.7%, female 9.8%) in male was higher than female if which present to fractionalize the 11.2%(male 21.9%, female 3.0%) were alcoholics. Regular exerciser were more than half of rate, 53.4%(male 51.4%, female 55.1%) revealed the female get more regular exercise than male. Obesity rate was 21.9%(male 28.3%, female 16.9%) revealed a high percentage of male were obese. Second, the results of comparative analyzed quality of life on 5 dimensions of each health status by smoking, drinking, exercise, obesity and sociodemo -graphic variables that in case of the ex-smoking respondents reporting problems on Pain/Discomfort(24.1%) and smoking respondents reporting problems on Anxiety/Depression(18.5%) was revealed a higher rate in male. In female, smoking respondents reporting problems on Pain/Discomfort (47.4%) and Anxiety/Depression(47.4%) was revealed significantly higher rate than male. According to the alcohol consumption which in case of alcohol dependents reporting problems on Pain/Discomfort(male 24.6%, female 25%) and Anxiety/Depression(male 21.9%, female 45%) was revealed significantly higher rate in both gender, but in female, Anxiety/Depression dimension was revealed higher rate than male, and also problem drinker respondents reporting problems on Anxiety/Depression(31.8%) was revealed significantly higher rate in female. According to the health behaviors that in case of smoking and drinking respondents reporting problems on Anxiety/Depression was revealed signifi -cantly higher rate, in female reporting problems on Pain/Discomfort was revealed significantly higher rate, therefore community health center should be provided the mental health program to reduce the depression and also needs to emphasize to smoking may develop the pain, discomfort and depression on the smoking prevention program. In case of regular exerciser reporting problems on Pain/Discomfort was revealed significantly higher rate, therefore provide to the individuals exercising guideline who has the chronic disease and functional problems or not. Finally, obesity was interested for a part of health in worldwide these days, and also obesity-related metabolic syndrome program is one of major health promotion plan in Korea which project have been proceeding at the 15 of community health center
