The purpose of this study is to implement sit-to-stand(SitTS) and stand-to-sit(StandTS) training for stroke patients to examine their static and dynamic standing balance abilities and the effect of foot position during SitTS and StandTS movements on their paretic side lower extremity muscle activities. In the study, 30 subjects were randomly assigned — 10 subjects in the control group, 10 subjects to the training group that performed the SitTS and StandTS training in postures with the both feet located symmetrically(SYM group), and 10 subjects to the training group that performed the SitTS and StandTS training in postures with the both feet located asymmetrically so that the paretic side foot was located at the back(ASY group). The control group received only the existing physical therapy, whereas the two training groups received the existing physical therapy along with SitTS and StandTS training three times a week for a total of six weeks under the guidance of the researchers. A Balance Performance Monitor(BPM) was used to measure the patients’ static and dynamic standing balance before and after the training, and an EMG apparatus(MP30) was used to measure changes in the muscle activities of the paretic side lower extremity during the SitTS and StandTS movements. Data were analyzed statistically using paired t-test and one-way ANOVA. 1. Based on the results of analysis of static standing balance abilities, the ASY group showed statistically significant differences in weight bearing on the paretic side, sway area, sway path, and maximal sway velocity before and after the training(p<.05). The SYM group showed statistically significant differences in sway area before and after the training(p.05). 2. Based on the results of analysis of dynamic standing balance abilities, the ASY group showed statistically more significant differences in dynamic mean balance, anteroposterior angle, and mediolateral angle before and after the training(p<.01). The SYM group showed statistically significant differences in dynamic mean balance, anteroposterior angle, and mediolateral angle before and after the training(p<.05). Moreover, a significant difference was evident among the three groups after training(p<.05). According to the post-hoc test of the dynamic mean balance after the training, the ASY group showed a more significant increase than the SYM group(p<.05). According to the post-hoc test of the anteroposterior angle and mediolateral angle after the training, the ASY group had a more significant increase than both the SYM group and the control group(p<.01). 3. Based on the results of analysis of the paretic side lower extremity muscle activity during SitTS movement, the ASY group showed statistically more significant differences in rectus femoris and tibialis anterior muscle activities(p<.01) and a significant difference in gastrocnemius muscle activity(p<.05) before and after the training. The SYM group showed a statistically significant difference in rectus femoris muscle activity before and after the training(p<.05), and a significant difference in tibialis anterior muscle activity was evident among the three groups after training(p<.05). According to the post-hoc test, the ASY group had a more significant increase than the SYM group(p<.05). 4. Based on the results of analysis of the paretic side lower extremity muscle activity during StandTS movement, the ASY group showed a statistically more significant difference in gastrocnemius muscle activity(p<.01) and significant differences in rectus femoris and tibialis anterior muscle activities(p<.05) before and after the training. The SYM group showed statistically significant differences in rectus femoris and gastrocnemius muscle activities before and after the training(p<.05), and the significant difference in rectus femoris was evident among the three groups after training(p<.05). According to the post-hoc test, the ASY group had a more significant increase than the control group(p<.05). In conclusion, these results showed that the SitTS and StandTS training in postures with both feet located asymmetrically so that the paretic side foot was at the back not only improved stroke patients’ static and dynamic standing balance abilities but also was effective in increasing the paretic side lower extremity muscle activity during SitTS and StandTS movements.
