This research has three main goals. First, it examines the relationship between chronic and oral disease in adults using data from 2009 National Health and Nutrition Examination Survey. Second, it identifies oral health hazard factors and thereby provides information to aid in establishing oral health policies that can promote oral health, preventing oral disease, and maintaining oral health and the date was conducive to oral health of Korean citizens. The research was conducted using data from the third year (2009) of the 4th National Health and Nutrition Examination Survey. Of 10077 adults over 19 years old who completed medical examination, health survey, and nutrition survey, 7893 people excluding 2184 inappropriate responses were selected as the final research subjects. Research results showed that differences in the rate of decayed missing and filled(DMF) permanent teeth, the rate of prevalence of periodontal disease, and the rate of prevalence of missing teeth were statistically significant in demographic and socioeconomic characteristics such as age, gender, marital status, education level, residential area, income level, and occupation. In case of the prevalence of dental caries, age, educational level, and income level were statistically significant. Health behaviors such as subjective health status, smoking status, drinking status stress, obesity, sleep time, and physical activity participation rate were statistically significant in the prevalence of periodontal disease. In case of the prevalence of missing teeth and the rate of DMF permanent teeth, subjective health status, smoking status, drinking status, stress, obesity, and sleep time were statistically significant. Dietary habits such as daily meal frequency, snack frequency, dining out frequency, and intake of fried food were statistically significant in the prevalence of periodontal disease, the prevalence of missing teeth, and the rate of DMF permanent teeth. Oral health behaviors such as subjective oral health status, daily frequency of tooth brushing, experience of dental check up, usage of oral hygiene products, and discomfort in mastication were statistically significant in the prevalence of periodontal disease, the prevalence of missing teeth, and the rate of DMF permanent teeth. In case of the prevalence of dental caries, subjective oral health status, experience of dental checkup, and discomfort in mastication were statistically significant. Chronic diseases diagnosed by doctor such as hypertension, hyperlipidemia, stroke, myocardial infarction, angina, diabetes, and osteoporosis were statistically significant in the prevalence of periodontal disease and the prevalence of missing teeth. In case of the rate of the DMF permanent teeth, hypertension, hyperlipidemia, stroke, angina, diabetes, and osteoporosis were statistically significant. Individuals with following factors were more affected by periodontal disease: higher age, residence in small or medium-sized city, discomfort in mastication, low subjective oral health status, and smoking (p<0.001). Individuals with following factors were more affected by missing teeth: higher age, female, lower education level, smoking, discomfort in mastication, and low subjective oral health status (p<0.05). Individuals with following factors were more affected by dental caries: higher age, lower income leve, and low subjective oral health status (p<0.05). When risk of chronic disease due to oral disease was examined, periodontal disease and missing teeth had significant effect on hypertension, hyperlipidemia, stroke, myocardial infarction, angina, diabetes, osteoporosis.(p<0.05) while dental caries was not significantly related. When synergistic effect of periodontal disease and missing teeth on chronic disease was analyzed, it was found that there was a significant effect on hypertension, hyperlipidemia, stroke, myocardial infarction, angina, diabetes, osteoporosis. (p<0.05).
From the results above, it was found that oral disease, especially periodontal disease and missing teeth, increase the risk of chronic disease. It is thought that epidemiological study in this area, along with research on the mechanism for cause of chronic disease, is required. In the future, problems in oral health of Korean citizens should be identified and specific programs to solve these problems is needed. Also, the development of public oral health program for preventing oral disease that accounts for regional needs is necessary.
