논문개요 본 연구의 목적은 2% chlorhexidine 침상목욕 간호가 MRSA 발생률과 BSI 발생률에 미치는 효과를 파악하여 피부 세정제를 이용한 치료적 침상 목욕에 대한 구체적 지침을 마련하기 위함이다. 본 연구는 2% chlorhexidine 침상목욕 간호를 제공받은 실험군과 기존의 계면활성제 성분의 고형 비누와 액상 세정제를 이용한 전통적 침상 목욕 간호를 제공받은 대조군간의 MRSA 발생률과 BSI 발생률을 비교한 무작위 대조군 사후 실험설계 (Randomized control-group posttest-only design) 연구이다. 본 연구의 대상은 2010년 11월부터 2011년 2월까지 서울에 소재한 900병상의 C 대학병원 내․외과계중환자실에 입원한 환자 전수로 연구에 참여한 순서대로 홀수는 실험군, 짝수는 대조군으로 선정하였으며, 다음 기준에 부합되는 사람으로 실험군 109명, 대조군 129명, 총 238명을 대상으로 하였다. 본 연구 대상자의 구체적 선정 기준은 첫째, 병원 감염의 정의에 따라 중환자실 입원 48시간 이상이 되는 환자, 둘째, 본 연구의 목적을 이해하고 참여를 허락한 환자로 하였으며 임산부, 2세 이하의 환아(FDA 권고), 중환자실에 입원 당시 MRSA, BSI 감염의 이력이 있는 환자, chlorhexidine에 알레르기 기왕력이 있는 환자, 피부 손상의 범위가 20% 이상인 환자는 연구대상에서 제외하였다. 수집된 자료는 SPSS 15.0 program을 이용하여 빈도, 백분율, t-test, test, Fisher's exact test의 방법으로 분석하였다. 연구결과는 다음과 같다.
1. MRSA 감염은 실험군에서 10건, 대조군에서 29건 발생하였고, 이를 재원일수 1,000일 기준 으로 환산했을 때 MRSA 발생률은 실험군 10.04, 대조군 18.88로 실험군에서 MRSA 발생률이 8.84 적었고 이는 통계적으로 유의한 차이를 보였다(p=.006).
2. BSI 감염은 실험군에서 1건, 대조군에서 7건 발생하였고, 이를 재원일수 1,000일을 기준으로 환산했을 때 BSI 발생률은 실험군 1.00, 대조군 4.55로 실험군에서 BSI 발생률이 3.55 적어 발생률 자체에서는 차이를 보였으나 통계적으로 유의한 차이는 없었다(p=.327*).
이상의 결과로 볼 때 2% chlorhexidine을 이용한 침상목욕 간호가 MRSA와 BSI 감염 감소에 효과적인 간호중재로 생각된다. 그러나 제한된 연구기간으로 인해 BSI 발생률에 통계적으로 유의한 결과를 얻지 못해 좀 더 오랜 기간 적용하여 그 효과를 입증할 필요가 있으며, 효과적이고 정확한 2% chlorhexidine침상 목욕 간호의 시행을 위해 중환자실 간호사들에게 지속적인 교육과 정보제공이 필요할 것으로 사료된다.

The purpose of this study was to identify the effectiveness of the bed bathing with 2% chlorhexidine on the incidence of methicillin-resistant Sta- phylococcus(MRSA) and bloodstream infection(BSI) and to suggest guidelines on the therapeutic bed bathing using skin cleaner. This study was designed to compare the incidence of MRSA and BSI of the experimental group who received the bed bathing with 2% chlorhexidine with the incidence of MRSA and BSI of the control group who received the existing bed bathing with soap and skin cleaner. A research design used in the study was a randomized control-group posttest-only design. Data collection was performed from November 2010 through February 2011 with all patients who were admitted to a intensive care unit(ICU) of one university hospital located in Seoul. In the order in which they were admitted, the odd patients were selected for the experimental group and the even patients were selected for the control group. The total 238 patients were assigned to the experimental group and the control group. The experimental group were 109 patients and the control group were 129 patients. This study was intended for the patients who stayed for more than 48 hours after ICU admission and who understood the purpose of the study and allowed to participate in the study. Exclusion criteria included pregnant women, patients younger than 2 years old, patients with a previous history of MRSA and BSI, patients with a previous history of allergy to chlorhexidine and patients who have more than 20% body surface area disruption of skin integrity. The collected data were analyzed by SPSS 15.0 program for frequency, percentage, t-test, Chi-square test, and Fisher's exact test.
The results of this study are as follows;
1. The incidence of MRSA of the experimental group was 10 cases and the incidence of MRSA of the control group was 29 cases. The experimental group was associated with the statistically significant decrease in the incidence of MRSA(10.04 vs 18.88 per 1000 patient days, p=.006). 2. The incidence of BSI of the experimental group was 1 case and the incidence of BSI of the control group was 7 cases. The experimental group was less likely to develop BSI(1.00 vs 4.55 per 1000 patient days, p=.327*), but there was no statistically significant reduction in the incidence of BSI.
According to the present study, it can be concluded that the bed bathing with 2% chlorhexidine is an effective nursing intervention to decrease the incidence of MRSA and BSI. However, it is necessary to prove the effectiveness of the bed bathing with 2% chlorhexidine on the incidence of BSI through a longer period study, because the statistically significant reduction in the incidence of BSI did not appear due to a short period study. And it is considered that continuous training and information for the effective and exact bed bathing with 2% chlorhexidine should be provided to ICU nurses.
