The purpose of this study was to examine the characteristics of the object relations of depression patients by the type of depression after classifying depression patients into subtypes. A cluster analysis was made to confirm whether the subtypes of depression classified in this study would be identical to those in earlier studies, and then the characteristics of the object relations of the patients of the subtypes, their dominant interpersonal concerns and interpersonal behaviors were checked. Depression is concurrent with lots of dysfunctions in terms of interpersonal relationships, and finding out the object relations of depression patients makes it possible to let them know how they perceive themselves and others and how their perception affects their relations with people around them. As object relations are under the influence of both of consciousness and the unconscious, TAT that is one of the typical projective tests to reveal the dynamic aspects of interpersonal relationship was conducted, and SCORS-Q that was to evaluate object relations was utilized to analyze the object relations characteristics of the depression patients from various angles in structural, thematic and behavioral dimensions. Three research questions were posed:
1. Are depression patients classified into subtypes?
2. Are there any differences in object relations in the structural dimension according to the subtypes of depression in the SCORS-Q?
3. Are there any differences in object relations in the thematic and behavioral dimension according to the subtypes of depression in the SCORS-Q?
The subjects in this study were 43 inpatients and outpatients who were diagnosed with depression in the department of neuropsychiatry at a hospital located in Seoul between January 2008 and October 2010. Three selected raters rated the TAT data by utilizing the SCORS-Q in the structural, thematic and behavioral dimensions, and the depression patients were categorized into subtypes based on the data to grasp their object relations characteristics.
The findings of the study were as follows:
First, the cluster analysis was made to see whether the depression patients could be categorized into subtypes or not by selecting the self-directedness and reward dependence of the TCI as cluster variables. As a result, they were divided into two. One was an autonomy-intimacy type that scored low in both of self-directedness and reward dependence, and the other was an avoidance-rejection type that scored high in both of the two. The avoidance-rejection type was prevalent among the men who were in their 20s, and those men suffered severe psychological disorder and depression and lagged behind in terms of adjustment.
Second, ANOVA was utilized in conjunction with the eight components of the structural dimension of the SCORS-Q to see whether there were any differences in object relations according to their subtype of depression. The patients of the avoidance-rejection type scored lower in affective quality of representations, emotional investment in relationships, emotional investment in values and moral standards, experience and management of aggressive impulses and self-esteem. It indicated that the patients of the avoidance-rejection type relatively fell behind the others of the autonomy-intimacy type in object relations, and that the former was more pathological.
Third, the collected data were analyzed in conjunction with the 50 thematic items of the thematic and behavioral dimension of the SCORS-Q to see whether there were any differences in the themes of interpersonal relationships according to their subtype of depression. As a result, it's found that although positive themes such as achievement, autonomy and affiliation were found in the patients of the avoidance-rejection type, more dysfunctional themes were found in them like aggression, resignation, loss and suicide, which appeared when their needs for achievement, autonomy or affiliation weren't satisfied. Besides, each subtype's typical cases were selected to look into the characteristics of interpersonal relationship themes in each case in detail to find out common denominators and differences among the subtypes.
The significance of this study could be described as below:
Frist, the depression patients were classified into subtypes to look for differences in their object relations and interpersonal relationship themes instead of merely viewing them as the same group. And a cluster analysis was carried out to categorize the patients instead of using existing questionnaires that already drew a clear line among the subtypes.
Second, this study is expected to give some clinical suggestions on how to understand the way of relationship building among depressed clients and their expectations for the relationship according to their subtype of depression and how to relieve depression symptoms and prevent the recurrence of the symptoms by correcting their negative interpersonal cognition and boosting their interpersonal interaction.
Third, existing studies attempted to evaluate object relations by conducting self-administered surveys, but projective tests such as TAT can be more accurate since object relations are one of unconscious elements that are based on one's initial experiences.
In this study, just the depression patients who visited the department of neuropsychiatry at a university hospital located in Seoul were examined, and the number of samples wasn't large enough to represent the population. Therefore the findings of the study might not be generalizable. When the depression patients were selected to acquire three years of psychological test data, the number of the selected patients was smaller than expected. Those who had depression disorder added by psychotic symptoms and who had bipolar disorder were ruled out, and the number of samples wasn't consequently large enough although all the cases of patients who participated in both of self-reported survey and TAT te
