ABSTRACT Effect of A Mouth Exercise Program on Improvement of Functions and Quality of Life Related to Oral Health in the Elderly Dong-Yeol kim (Director: Prof. Jin-Ho Chun, M.D., Ph.D.) Department of Public Health, Graduate School, Inje University Purpose: The oral health of the elderly has a close relationship with maintaining their general health in a good status and their satisfaction with quality lives in the end. Therefore, this study was to evaluate and prove how the mouth exercise program makes a certain impact especially on oral health-related functions of the elderly and the improvement in quality of their lives. Methods: The total number of subjects people for this study was ninety two which was divided into two groups. One group (case group) was comprised of fifty two old persons who were served in "C" Skilled Nursing Facilities in Gyeong-ju city and in "N" Medical Institution and their ages were 65. The other group (control group) consisted of thirty nine old persons. For the purpose of this study, the two dental hygienists ran the program and they had been trained for the mouth exercise program from January to March in 2012 ahead of such program. For the performance of the program, Measurement t-test was carried out three times (0 week, in 6 weeks and, in 12 weeks) and Questionnaire was surveyed two times (before and after the program commencement) through individual interview. Both methods checked Streptococcus mutans, Mouth bad breath, Saliva secretion, maximum mouth opening and oral Diadochokinesis, respectively. For the analysis of gained data, SPSS (ver. 18.0, Chicago, Illinois, USA) was utilized to compare the two groups each other by way of chi-square test, t-test and paired t-test was performed before and after the education where the change by timing of measurement was assessed with the Repeated Measures analysis (RM-ANOVA). Statistical criterion of significance was a = 0.05. Results: According to the result gained from RM-ANOVA, there was no change in the control group while significant development in measurement index was recognized in the case group as below:In connection with S. mutans in the program, it was 6.49X107 CFU/ml before the program and 2.45X107 CFU/ml (p<0.001) after the program, respectively. With regard to the Mouth bad breath, it recorded 69.35 BBV (Bad Breath Value) before the program while it dramatically decreased to 22.50 BBV after the program (p<0.001). As for the amount of salivation, it recorded 0.20ml/min before the program while it increased to 0.27ml/min after the program (p<0.001). In connection with maximum mouth opening, it was 4.44cm before the program while it recorded 4.87cm after the program (p<0.001). With regard to the oral Diadochokinesis, it recorded 0.37 times/sec before the program while it increased to 0.60 times/sec after the program. It was brought to the attention that subjective symptoms such as xerostama, jaw function and function of expressing emotion were significantly improved, and OHIP-14 rating became lower. Satisfaction with overall program was high in general. Conclusion: It was proven that the mouth exercise program makes a favorable impact on the enhancement in major oral health-related measurement index, subjective symptoms and quality of life relating to oral health. Even if further efforts are required to overcome partial high level of difficulty and to expand the operation of program and invigoration of the same, the program was highly recommendable for cost-effective and high efficient one to increase quality of life for which enhancement in both oral health and function are required.
