연구배경 : 현대사회는 심혈관계 질환의 증가, 인구의 노령화, 스트레스 유발로 인한 심정지 환자가 점점 증가하고 있다. 심정지 환자의 발생은 예측이 불가능 하며 환자 대부분은 119 구급대에 의하여 병원으로 이송되어진다. 이 때 병원 전 심정지 환자에서 주로 보이는 리듬은 제세동이 가능한 심실세동과 무맥성 심실빈맥인데, 이 두 리듬에 제세동이 적용하는 시간에 따라 생존이 달려있다.
이에 본 연구자는 119 구급대를 통해 이송되는 병원 전 심정지 환자 중 초기 심전도 리듬이 제세동이 가능한 심전도 리듬을 보이는 경우 병원 도착 전 자발순환회복에 미치는 영향을 파악하고자 본 연구를 시행하였다.
대상 및 방법 : 2010년 01월 01일부터 2011년 12월 31일까지 2년간 J도의 119 구급대에 신고 되어 병원으로 이송된 2298명 중 제세동이 가능한 심실세동과 무맥성 심실빈맥 심전도 리듬을 가진 병원 전 심정지 환자 222명을 연구 대상으로 하였다. 연구 방법은 연구자가 근무하는 J도의 구급활동일지를 분석하였으며, 심전도 리듬의 평가는 심폐정지세부상황표의 심전도 기록지를 직접 확인 하였다. 자발순환 회복과 관련된 영향을 분석하기 위하여 Fisher's exact probability test, Mann-Whitney test와 Logistic Regression Analysis을 하였다.
결과 : 병원 도착 전 119 구급대의 자발순환 회복은 전체 222명 중 7명(3.2%)에서 나타났다. 자발순환 회복에 미치는 영향을 분석해 본 결과 첫째 119 구급대 신고부터 119 구급대가 출동에 반응하는 시간 (P=0.001), 신고에서 부터 119 구급대가 현장에 도착하는 시간(P=0.030)에서 유의한 차이를 보였다. 특히 119 구급대가 3분 이내에 현장에 도착한 경우 자발순환 회복에 유의한 차이를 나타냈다(P=0.026). 둘째로 환자의 일반적 특성 중 연령에서 자발순환 회복에 유의한 차이를 나타냈다(P=0.019). 셋째 119 구급대에 의한 처치로는 현장에 정맥로 확보한 군에서 자발순환 회복에 유의한 차이를 나타냈다(P=0.012). 넷째로 119 구급대가 현장에 도착하기 전 목격자에 의한 심폐소생술이 시행된 경우 8.85배 자발순환 회복에 큰 영향을 주었다(OR 8.85, CI95% 1.206-65.029).
결론 : 본 연구를 통해 119 구급대가 이송한 심정지 환자에서 자발순환 회복 군에 영향을 미치는 것은 목격자에 의한 심폐소생술이 이루어 진 경우 순환 정지시간이 짧아져 자발순환 회복에 큰 영향을 미쳤다. 또한 119 구급대 신고에서부터 빠른 출동 시간, 빠른 현장 도착 시간이 이루어 진 경우 제세동 가능한 심전도 리듬에서 조기 제세동이 가능하였으며 이는 결국 자발순환 회복에 유의한 차이를 나타냈다.

Purpose: Today, the number of cardiac-arrest patients is increasing as a result of increasing cardiovascular diseases, aging, stresses and so forth. It is unpredictable; most of the patients are transported to a hospital by 119. At this moment, majority patients are showing defibrillation-able rhythms: ventricular fibrillation or pulse-less ventricular tachycardia; a matter of life and death depends on defibrillation timing. This research was performed to examine how defibrillation-able rhythms on pre-hospital step affect the recovery of spontaneous circulation before the arrival at the hospital.
Method: Among 2298 cardiac-arrest patients who were reported to 119 at ‘J’ Do during two years (2010.01.01-2011.12.31), I selected only 222 of them who had shown defibrillation-able rhythms. I analyzed ‘J’-do’s rescue log and rescue information system. I also checked the electrocardiogram (ECG) recording paper for ECG rhythm assessment. Fisher’s exact probability test, Mann-Whitney and Logistic regression analysis were performed to elucidate the factors associated with ROSC (recovery of spontaneous circulation)
Results: Before arrival at the hospital, ROSC occurred to 7 people (3.2%) among 222 patients. First of all, 119’s response time (P=0.001) and the time from the first report to the scene arrival (P=0.030) showed a difference. Especially, when 119 arrived at the scene in 3 minutes ROSC shows a difference (P=0.026). Secondly, patients’ age also affected ROSC (p=0.019). Thirdly, paramedics emergency treatment also leads to difference in ROSC (P=0.012). Fourthly, when CPR was performed by witnesses before 119’s arrival, ROSC was 8.85 times higher (OR 8.85, CI95% 1.20-65.02).
Conclusion: CPR performed by witnesses shortens the circulation pause time and had significant impact on ROSC. In addition, 119’s quick response time and scene arrival time helped earlier defibrillation which ultimately led better ROSC. Among the paramedics’ emergency treatments, ensuring vein affected ROSC. So, for better ROSC of cardiac-arrest patients, community involvement and activation of the emergency medical service system are crucial. Also, 119 needs faster response time and even more technical CPR. These efforts will play great role helping many patients to leave the hospital alive.
