This manuscript is a study that investigates the changes in reaction that are displayed by elderly with dementia during clinical application of the Korean traditional music therapy program. The Korean traditional music therapy program is developed by applying Gukak, Korean traditional music, to music therapy. As a type of social therapy provided for the elderly with dementia, music therapy is a treatment that has been actively performed and researched in foreign countries. Korean traditional music contains the emotions of the Korean people. Thus, this study has developed the Korean traditional music therapy method centered on physical movements and snare drum(Sogo) rhythms through the rhythmic respiration method, which is regarded the most important factor in Korean traditional music, in order to alleviate symptoms of dementia experienced by the elderly with dementia in Korea. The purpose of this study is to research the effect of Korean traditional music therapy on the elderly with dementia by applying this program on relevant subjects and conducting clinical experiments. Although it is true that studies on musical therapy using Korean traditional music still remain unsystematic and few in number, future development of the relevant research field is expected as related studies are being increasingly conducted. Thus, it can be said that this study holds significance in that it plays an important part in facilitating systematic development of Korean traditional music therapy. The korean traditional music therapy program designed by the researcher of this study is as follows. First, for structured physical motions, form circles, learn half-beat or one-third beat respiration method, and perform rhythmic claps through physical motions according to respiration method. Second, sing or listen to folk songs through half-beat or one-third beat respiration method and play snare drum(Sogo) in accurate rhythms according to music. Third, therapist and subject perform together by each playing individual instruments. By composing and participating in activities of the three procedures stated above, this study researched the effect of the program on the elderly with dementia. The following conclusion was drawn. First, subjects were encouraged to express physical motions through the physical motion activity devised by the researcher based on respiration method as this activity helped subjects to straighten their cringed chests. Subjects that did not make any eye contact or showed a tendency to look down were encouraged to look forward to gradually increase eye contact in the middle and end phases of the program. Also, the physical motion of making a circle helped subjects to easily clap in rhythm by emphasizing the Up Beat rhythm of Korean traditional music. Furthermore, all 8 research subjects (elderly with dementia) generally presented greater changes in the size of physical motion activities as the experiment was progressed although individual differences were shown in size of change. In other words, subjects that did not follow the motions in the early phase of the Korean traditional music therapy program gradually performed the movements, and motions that were initially conducted waist high were enlarged and heightened to be performed chest high, at eye-level, or above the head. Second, in case of the snare drum rhythmic beating activity according to respiration method and devised physical motions, subjects presented higher accuracy in beating the drum (at least 5 times of accurate beating) in the middle and end phases of the program. However, although the changes in size of physical motions were enlarged in the latter phase, the changes were smaller when compared with rhythmic clapping through physical motions without instrument. Third, although certain subjects had presented less than 5 smiles, eye contact, or linguistic expressions before/after program or during the Korean traditional music therapy program, all 8 subjects presented more than 5 smiles, eye contact, or linguistic expressions in the latter part of the program. Also, subjects that had only presented smiles were able to make laughing sounds or express sentences in short answers or by using basic words. The researcher of this study realized that the physical motions according to respiration method were easy to learn, and observed increased eye contact and enlarged size of physical motions of subjects as the program was progressed. Also, it was observed that the snare drum beating activity through physical motions according to respiration method emphasized the Up Beat rhythm of Korean traditional music to induce subjects to easily copy beating movements, thus allowing subjects to gradually achieve consecutive drum beating. Furthermore, as the designed physical motions were natural, comfortable motions that allowed subjects to easily learn the respiration method of Korean traditional music, such motions aroused the interest of subjects to help them to not only achieve larger changes in size of physical motions and accurate and consecutive drum beating, but also helped subjects to present improved reactions, such as eye contact, smile (laugh), appropriate linguistic expressions (short/long answers, words related with activities, etc.) to the activities provided by the researcher during the Korean traditional music therapy program. Thus, the Korean traditional music therapy program applied with physical motions according to structured rhythm respiration method and instrument performance was verified to positively affect the ability of the elderly to make physical expressions.
