Designating a performance-related pay system as an important factor in the physician-induced demand, the present study carried out in-depth analyses, from medical professionals’points of view, of how the system externalized medical professionals’internal motives and decisions concerning the health services they provided. We also examined the causal relations between performance-related pay systems and how the professionals behaved in their practices. The results were as follows: First, a pay system based on performance was found to be a means to create profits from medical services provide, and this naturally led to a situation where health specialists were pressured to increase their workloads. The performance-based pay system has gained in popularity and has now been established as a fairly universal payment method. This, coupled with a decreasing number of new hospitals and an increase in staff status increased, means that larger hospitals receive disproportionately more patients than their smaller counterparts. Second, the pay system was observed to distort the general understanding of health care in various ways. Health care providers believed that overtreatment was the ‘norm’ in medical practice, negatively influencing the doctors, as well as the patients, to accept excessive amounts of medical consultations and treatments. Furthermore, the payment scheme transformed the cooperative collaborations amongst different departments into more competitive interactions, resulting in an overall inefficiency in the whole system. These findings led us to suggest that the performance-related pay system might not be suitable for the fields of medicine and health care. Our study revealed several factors involved in the pay system that affected how medical service providers practiced their specialties. Following our study outcomes, those factors were employment status uncertainties, absence of professional ethics and integrity, inefficient payment systems that Fee-For-Services, and abuses of private health insurance. To foster sustainable and respectable health care, based on our findings we suggest there should be policy interventions in the payment schemes for medical professionals, a restructuring of university medical education, payment system streamlining, and rethinking private health insurance roles. All of these must be preceded by a strengthening of the public health system, which highlights the public aspects of health care and the important role of the government. The government should properly intervene in the payment system for health specialists who are the main players in medical care, and establish a system where any abuses and inappropriate behaviors are monitored and curbed. This might be one of the biggest present challenges the country faces in its attempts to develop into a future welfare state.
