국문초록 유창성 실어증 환자의 말하기, 읽기, 쓰기 과제의 오류 특성 비교 이은주 광주여자대학교 사회개발대학원 언어치료학과 언어치료전공 실어증은 언어습득 시기가 지난 후에 뇌의 신경학적 손상의 결과로 발생되는 후천적인 언어장애로서, 중추신경계 손상으로 말하기, 듣기, 읽기, 쓰기와 관련한 언어의 전 영역에 걸쳐 어려움을 보이며, 대부분 자발화에서 정상인에 비하여 언어를 표현하는 데에 어려움을 보인다. 즉, 이름대기 장애, 착어 오류, 대치 오류, 문법 사용의 어려움 등을 나타낸다. 따라서 정상인에 비해 다양한 어휘를 사용하지 못하고 정확하지 않은 표현으로 적절한 의사소통이 이루어지기 어렵다. 이에 본 연구에서는 유창성 실어증 환자를 중심으로 말하기, 읽기, 쓰기 과제시 나타나는 오류의 특성과 과제 수행시 주로 나타나는 오류 유형을 분석하였다. 오류 형태는 의미착어, 음소착어, 조사, 신조어 사용을 중심으로 연구하였다. 또한 세 가지 표현 언어 영역에서 유형별로 보이는 오류의 특징과 빈도를 구하여 유창성 실어증간의 표현 언어 능력과 과제에 따른 오류의 차이를 알아보고자 하였다. 연구 대상은 파라다이스․한국판-웨스턴 실어증 검사 결과 유창성 실어증으로 분류된 명칭 실어증 8명, 전도성 실어증 2명, 초피질 감각 실어증 2명, 베르니케 실어증 4명으로 총 16명이었다. 평가 영역은 말하기, 읽기, 쓰기 총 3가지 과제로 이루어졌으며 말하기 과제는 파라다이스․한국판-웨스턴 실어증 검사의 스스로 말하기 부분을 이용하였고, 나머지 읽기와 쓰기 과제는 본 연구자가 선행 연구를 참고하여 연구 목적에 맞게 구안하였다. 소요시간은 30분에서 1시간 정도 소요되 었다. 본 연구의 결과는 다음과 같다. 첫째, 말하기 과제 연구 결과 점수가 가장 높게 나타난 유창성 실어증 유형은 명칭 실어증이고, 가장 낮게 나타난 유창성 실어증 유형은 베르니케 실어증이었다. 둘째, 읽기 과제 연구 결과 가장 높게 나타난 유창성 실어증 유형은 전도성 실어증이고, 가장 낮게 나타난 유창성 실어증 유형은 초피질 감각 실어증과 베르니케 실어증이었다. 셋째, 쓰기 과제 연구 결과 가장 높게 나타난 유창성 실어증 유형은 전도성 실어증이고, 가장 낮게 나타난 유창성 실어증 유형은 베르니케 실어증이었다. 이상의 결과로 말하기, 읽기, 쓰기 세 가지 과제에서 가장 장애의 정도가 적은 것은 말하기 과제였고, 가장 장애의 정도가 심한 것은 쓰기 과제로 나타나는 것을 볼 수 있었다. 또 실어증 유형별로 살펴보면 베르니케 실어증은 말하기, 읽기, 쓰기 순으로 말하기 과제에서 가장 높은 점수를 보였고, 명칭 실어증은 베르니케 실어증과 동일한 순서를 보였다. 전도성 실어증은 쓰기, 읽기, 말하기 순으로 베르니케 실어증과 명칭 실어증과는 상반되는 결과를 나타내었고, 초피질 감각 실어증은 말하기, 쓰기, 읽기 순으로 말하기 과제에서 가장 높은 점수를 나타내었다.

ABSTRACT Comparison of Fluent Aphasia Patient’s Error Characteristics in Speaking, Reading, and Writing Tasks Eun-Ju, Lee Social Development Graduate School, Kwangju Women’s University Major in the Speech Therapy in the Department of Speech Therapy Aphasia is an acquired language disorder resulting in the brain’s neurological injury in a time after passing the language acquisition period. Due to an injury in the central nervous system, an aphasia patient shows difficulties in the whole linguistic ranges relevant to speaking, listening, reading and writing, and shows difficulties in expressing language during the spontaneous speech process comparing to normal people. That is, an aphasia patient shows the naming error, the paraphasia, the substitution error, the grammar difficulties. Therefore contrary to normal people, an aphasia patient does not use various vocabulary and are hard in appropriately communicating due to their use of inaccurate linguistic expressions. So this study analyzed the characteristics of errors seen in speaking, reading, writing tasks and the error types frequently seen at performing such tasks around the fluent aphasia patients. This study investigated the error types of meaning error, phoneme paraphasia, postposition error and newly-coined words use error. Also, this study aimed to identify the error differences between the fluent aphasia patients depending on different-degree expression abilities and tasks by analyzing the each error type’s characteristics and each error type’s frequency seen in 3 linguistic sections. The subjects of this study were 16 fluent aphasia patients who were classified in the Paradise. Korean-version Western Aphasia Battery: 8 naming aphasia patients, 2 conductive aphasia patients, 2 transcortical sensory aphasia patients, and 4 Wernicke’s aphasia patients. The test sections were composed of total 3 tasks of speaking, reading and writing. The speaking task in this study used the self-speaking task section of Paradise. Korean-version Western Aphasia Battery, and the other two tasks of reading and writing were created by the investigator according to this study’s purpose in reference to previous studies. The test time was taken from about 30 minutes to 1 hour. The results of this study are as follows. First, the fluent aphasia type being measured with highest scores in the speaking task was the naming aphasia, and the fluent aphasia type having the lowest scores in the same task was the Wernicke’s aphasia. Second, the fluent aphasia type being measured with highest scores in the reading task was the conductive aphasia, and the fluent aphasia types having the lowest scores in the same task were the transcortical sensory aphasia and the Wernicke’s aphasia. Third, the fluent aphasia type being measured with highest scores in the writing task was the conductive aphasia, and the fluent aphasia type having the lowest scores in the same task was the Wernicke’s aphasia. From the above results, it was found that the all types of aphasia patients showed the least handicap(difficulty) in the speaking task among the 3 tasks of speaking, reading and writing, but they showed the highest handicap(difficulty) in the writing task. And looking into each aphasia type, the Wernicke’s aphasia showed the highest scores in the speaking task and next came the reading task and the writing test in order, and nest came the naming aphasia’s score, and then the Wernicke’s aphasia’s score in order. Conductive aphasia showed the highest score in the writing task and next came the reading task, the speaking task in order. However the Wernicke’s aphasia and the naming aphasia showed the opposite-order results from the conductive aphasia. Finally, transcortical aphasia showed the highest score in the speaking and next came the writing task, the reading task in order.
