The purpose of this study was to develop a Survey Instrument to assess Korean Patient Safety Culture for hospitals and to evaluate its validity and reliability. To develop the survey instrument, a literature review was conducted in the areas of patient safety culture and existing safety culture instruments. Also interview was conducted to identify major patient safety culture issue with healthcare provider and patient safety experts. Through literature review and interview a preliminary survey instrument was developed. It was consisted of three major dimensions ; organization, department, individual. Content validity of the items and scale was confirmed by experts composed of nurses, patient safety specialists and professors. A pre-study was conducted to test the face validity of the items. Twenty-two health care professionals were asked to respond to the questionnaire and comment on the wording and the time required. The final survey instrument contained 49 items about patient safety culture. To test validity and reliability of this instrument, a survey was performed from September to October, 2014. The participants of this study were 467 employees(i.e. doctors, nurses, pharmacists, medical technicians) in one tertiary hospital in Seoul, South Korea. After obtaining consent from all participants, the anonymous self-reported questionnaires were distributed and the response rate was 95.5%. The construct validity of the instrument was assessed using exploratory factor analysis and confirmatory factor analysis. The Kaiser-Meyer-Olkin measures of sampling adequacy and Bartlettʼs test of sphericity were used to determine whether the data were appropriate for factor analysis. Items with a factor-loading score of less than 0.5 were eliminated. To test criterion validity, inter-correlation of Korean Patient Safety Culture survey factors and subjective patient safety culture score were tested. Also relationship between report number and Korean Patient Safety Culture survey factors were tested. Cronbachʼs alpha coefficient was used to evaluate the internal consistency of each dimension. From the results of the analysis, 35 final items were selected from 49 items on the preliminary instrument. Items with low correlation with total scale were excluded. After factor analysis, 7 factors were extracted contributed 60.98% of the variance in the total scale. The factors were labeled as leadership(9 items), patient safety policy and procedure(4 items), patient safety improvement system(4 items), teamwork(6 items), non-punative environment(4 items), patient safety knowledge and attitude(5 items), patient safety priority(3 items). After criterion validity test, we conclude that Patient Safety Culture survey instrument was same concept as subjective patient safety culture scale. The overall reliability of the Korean Patient Safety Culture Survey Instrument had a Cronbachʼs alpha for internal consistency of .93, whereas leadership, patient safety policy and procedure, patient safety improvement system, teamwork, non-punative environment, patient safety knowledge and attitude, patient safety priority of .91, .81, .79, .84, .73, .80, and .66, respectively. In one tertiary hospital in Seoul, the patient safety culture score was 3.82 measured by Korean Patient Safety Culture Survey Instrument(5 point Likert scale) and patient safety culture of the doctors showed the lowest. Finally Korean Patient Safety Culture Survey Instrument containing 35 items with 7 factors was developed.
