본 연구는 인증병원과 미인증병원 간의 비교분석을 통해 의료기관인증제도의 효과를 실증적으로 규명하는데 목적을 두고 있다. 이러한 연구 목적 달성을 위해 본 연구에서는 의료기관인증제도에 관한 이론적 고찰과 선행연구 분석을 통해 연구모형을 설정하였고, 이를 부산지역 의료기관 중 인증병원 4개, 미인증병원 4개 총 8개 병원을 표본으로 선정하였다. 그리고 최종적으로 356부를 대상으로 빈도분석과 집단 간 평균분석을 실시하였다. 본 연구결과를 요약하면 다음과 같다. 먼저, 인증병원과 미인증병원을 구분하지 않고 의료기관인증제도의 주요지표를 중심으로 병원구성원들의 반응을 분석한 결과 다음과 같이 나타났다. 즉, 안전보장활동, 진료전달체계, 환자진료, 환자권리존중과 보호, 의료정보관리에 관한 의료기관구성원들의 긍정적 반응은 60% 후반이상으로 나타났다. 그러나 지속적인 질 향상활동, 조직운영, 인적자원관리에 대한 의료기관구성원들의 긍정적 반응은 50% 대로 나타났다. 특히, 환자권리존중과 보호에 대한 긍정적 반응은 71.9%로 가장 높게 나타난 반면, 인적자원관리는 56.6%로 가장 낮게 나타났다. 그리고 세부 항목별로는 진료과정에서 환자권리와 의무존중 및 사생활 보호에 대한 긍정적 반응이 78.9%로 가장 높게 나타난 반면, 병원장의 윤리위원회 규정 수립 및 지원 절차 확립에 대한 긍정적 반응이 51.4%로 가장 낮게 나타났다. 그리고 의료기관인증병원과 미인증병원간 비교를 통해 의료기관인증제도 효과를 분석한 결과 모든 지표에서 인증병원이 미인증병원보다 높게 나타났으며, 이러한 차이는 모두 통계적으로 유의한 것으로 나타났는데 이를 구체적으로 살펴보면 다음과 같다. 첫째, 의료기관인증병원과 미인증병원간 의료기관인증제도 주요평가지표를 중심으로 분석한 결과 모든 항목에서 인증병원의 평균이 미인증병원보다 높게 나타났다는 점에서 의료기관인증제도의 효과를 실증적으로 확인하였다. 둘째, 인증병원과 미인증병원간 가장 큰 차이를 나타낸 지표는 지속적 질 향상활동이었다. 즉, 인증병원의 지속적 질 향상 활동 평균은 3.87로 나타났는데 비해 미인증병원은 3.46으로 나타났다. 셋째, 의료기관인증병원과 미인증병원간 차이가 가장 작게 나타난 것은 진료전달체계로 인증병원의 평균은 3.88, 미인증병원은 3.74로 나타났다. 끝으로 본 연구결과를 기초로 다음과 같은 정책방안을 제시하였다. 첫째, 본 연구 결과 지속적인 질 향상활동, 조직운영, 인적자원관리에 대한 의료기관 구성원들의 긍정적 반응이 상대적으로 낮게 나타났다. 그러나 의료기관의 서비스 질 제고와 환자안전보장을 확보하기 위해서는 지속적인 질 관리활동이 무엇보다 중요하고 이러한 활동이 효과적으로 이루어지기 위해서는 병원장의 적극적인 노력과 지원 및 합리적 의사결정 등과 같은 조직운영과 조직구성원들의 능력향상을 위한 체계적인 교육훈련과 서비스 질 제고노력을 유도할 수 있는 인적관리가 체계적이고 효율적으로 이루어져야 한다. 둘째, 의료기관인증제도의 효과를 높이기 위해서는 인증기준의 객관성과 타당성 수준을 높이기 위한 노력이 필요하다. 이를 위해서 조사과정의 객관성과 전문성확보가 선행되어야 하고, 중간 또는 수시조사를 통해 지속적으로 서비스 질과 환자안전관리 활동을 조사 및 보완하도록 하여야 한다. 셋째, 현재 의료기관인증평가를 신청한 의료기관이 많지 않은 것으로 나타났다는 점에서 보다 많은 의료기관들이 의료기관인증을 받기 위해 적극적으로 신청할 수 있는 유인제도와 더불어 의료기관인증제도의 효과를 극대화 할 수 있는 제도적 장치마련과 환경조성이 필요하다. 넷째, 의료기관 인증을 받은 병원들은 지속적으로 환자안전과 의료의 질을 유지할 수 있도록 자체평가와 프로세스의 개발뿐만 아니라 병원전체 운영과정을 종합적이고 체계적으로 분석하여 그 결과에 따라 지속적 개선활동을 하도록 유도하여야 한다. 다섯째, 의료기관인증제도에 대한 적극적이고 체계적인 대국민 홍보를 통해 의료기관인증제도에 대한 긍정적 인식을 제고하여 의료서비스 소비자로서 국민들이 적극적으로 의료기관들의 서비스 질 관리와 환자안전 관리에 많은 관심을 갖도록 하여야 한다.

This study empirically examines the effects of hospital accreditation system through a comparative analysis of accredited and unaccredited hospitals. In order to achieve the purpose of this study, a theoretical review and an analysis of previous studies that are related to hospital accreditation systems were conducted to set up a research model. A total of 8 hospitals in Busan area were selected. The selected hospitals consist of 4 accredited and 4 unaccredited hospitals. A survey was conducted among a target of 356 respondents. Frequency analysis and t-test were also conducted. The study results have been summarized as follows. First, in order not to separate accredited and unaccredited hospitals, by focusing on the main indicators of the hospital accreditation system, the following hospital employees’ reactions were observed. In relation to safety guarantee activities, health care delivery system, patient care, respect for patients’ right and protection and medical information management, there was a positive reaction of 60% by the hospital employees. However, in relation to continuous quality improvement activities, organization’s operations and human resource management, the hospital employees’ positive reaction was 50%. Respect for patients’ right and protection had high positive reaction of 71.9%, whiles human resource management had a low positive reaction of 56.6%. In addition, patients’ management, respect and privacy protection in the treatment process had the highest positive reaction of 78.9%. On the other hand, the establishment of ethics commission regulation and support procedures by the hospital director had the lowest positive reaction of 51.4%. Through a comparative analysis of the effects of hospital accreditation system on accredited and unaccredited hospitals, it was observed from all the indicators that the effects were higher in accredited hospitals than unaccredited hospitals. Therefore, there was a statistically significant difference between accredited and unaccredited hospitals. The specific differences have been examined as follows. First, focusing on the hospital accreditation system’s main evaluation indicators, all the analyzed results averages from the accredited hospitals were higher than the averages from unaccredited hospitals. Second, continuous quality improvement indicator exhibited the biggest difference between accredited and unaccredited hospitals. Comparatively, in relation to continuous quality improvement, accredited hospitals had an average of 3.87, whiles unaccredited hospitals had an average of 3.46. In conclusion, based on the study results, the following policy measures have been presented. First, positive reaction on continuous quality improvement activities and human resource management were relatively low. It is important to conduct continuous quality management activities in order to secure hospital service quality improvement and guarantee patients’ safety. In order to conduct such activities effectively, the hospital director’s active efforts, resources and decision making processes are necessary. There should be a systematic and efficient human resource management system that can induce systematic education and training and service quality improvement to enhance the organizations operations and employees abilities. Second, in order to increase the effects of the hospital accreditation system, it is necessary to increase the accreditation standards, objectives and validation levels. Objectivity and professionalism should be ensured in survey processes. Midterm and occasional surveys should be conducted to complement continuous service quality improvement and patients’ safety management activities. Third, though many hospitals have not currently applied for hospital accreditation evaluation, institutional mechanisms and the enabling environment should be created to serve as an incentive for many hospitals to actively apply for accreditation and also maximize the effects of the system. Fourth, in order to ensure that accredited hospitals continuously maintain patients’ safety and care quality, the development of self-evaluation processes should be accompanied by a general and systematic analysis of the entire hospital operation processes. Therefore, continuous quality improvement activities should be carried out on regular basis. Finally, active and systematic publicity should be conducted to create positive awareness on the hospital accreditation system. The citizenry should have active interest in service quality management and patients’ safety.
