Lower extremity muscle weakness is a common symptom from stroke patients. It reduced the function of the lower extremity and result in difficulty in standing and walking and balance. Hip abductors serve as an important stabilizing muscle of the pelvis in the process of walking. Especially, gluteus medius is considered important in relation to pathological. The purpose of this study was to examine the impact of gluteus medius (GM) strengthening training using the pressure biofeedback unit (PBU) for lower extremity muscle function and balance ability in chronic stroke patients. Twenty-seven stroke patients (14 men, 13 women) were randomly divided into the PBU GM training group (n=9), general GM training group (n=9), and control group (n=9). Regardless of the group, all subjects were conducted the general neurological physical therapy for 30 minutes five times a week. Additionally, the PBU GM training group and general GM training group were carried out gluteus medius strengthening training for 20 minutes three times a week to four weeks. They were evaluated muscle strength, endurance and balance ability. Paired t-test and ANOVA was used for statistical analysis. Scheffe adjustment was used for post hoc analysis. The result of this study, the PBU GM training group and general GM training group were significant differences in muscle strength and Berg Balance Scale(p<.05). Only the PBU GM training group was significant differences in muscle endurance, LOS velocity and Lateral Reach Test(p<.05). These findings suggest that selective gluteus medius strengthening training using the pressure biofeedback unit leads to positive effects in order to improve muscle endurance and static and dynamic stability in chronic stroke patients. Also it showed an improvement in neuromuscular control ability over a significant decrease in the LOS velocity. Therefore, intervention in this study which allows a selective gluteus medius strengthening inhibiting the compensation by applying a pressure biofeedback unit can be used as a new intervention program and can suggest that effective in improving muscle function and the ability to balance their chronic stroke patients.
