The purpose of this study is to investigate the effect of lumbar stabilization training and additional thoracic joint mobilization on proprioception, spine curvature, static balance, pain and disability in patients with chronic back pain. The subjects of this study were 48 chronic back pain patients who were randomly allocated to an experimental group 1(n=16), experimental group2 (n=16) and control group (n=16) after thoracic mobility test. The experimental group 1(thoracic hypomobility positive) was applied with lumbar stabilization training and additional thoracic joint mobilization and The experimental group 2(thoracic hypomobility negative) were applied with lumbar stabilization training and additional thoracic joint mobilization. The control group(thoracic hypomobility positive) were applied with only lumbar stabilization training. The intervention was performed 3 times per week, 30 minutes per each time, for a total of 6 weeks. Thoraco-lumbar range of motion and joint reposition error was measured using the electro-goniometer, spine curvature was measured using backmapper, which is a 3D spine analysis system. Also static balance ability were measured using tetrax, which is posture analysis system. Subjects were measure using 100mm VAS for pain level, lumbar disability index were measured using ODI. The pain level decreased statistically significantly in experiment group 1(thoracic hypomobility, additional thoracic joint mobilization) compared to the control group (thoracic hypomobility, only lumbar stabilization training), and in terms of the amount of change, it decreased statistically significantly in the experimental group1(additional thoracic joint mobilization) compared to the control group(only lumbar stabilization training). There was no significant difference between thoracic mobility(thoracic hypomobility and thoracic partial hypomobility). There was no significant difference for the intervention(experimental group1 and control group)and thoracic mobility(experimental group1 and experimental group2) groups in the disability index. There were no significant difference in the thoracic kyphotic angle level for the intervention(additional thoracic joint mobilization and lumbar stabilization training) and thoracic mobility(thoracic hypomobility and thoracic partial hypomobility). The lumbar lordosis increased statistically significantly for those with thoracic hypomobility(experimental group1) compared to those with thoracic partial hypomobility(experimental group2), and the static balance level(fall index) decreased statistically significantly for those who received additional thoracic joint mobilization(experimental group1) compared to those who only received lumbar stabilization training(control group) and reduced thoracic hypomobility(experimental group1) more than thoracic partial hypomobility(experimental group2). The thoraco-lumbar range of motion level increased statistically significantly for those who underwent additional thoracic joint mobilization(experimental group1) compared to those who only underwent lumbar stabilization training(control group) in flexion and left lateral flexion. The joint reposition error angle decreased statistically significantly for those who received additional thoracic joint mobilization compared to those who received only lumbar stabilization training(control group) in flexion, extension, and right and left lateral flexion, and it decreased statistically significantly in those with thoracic hypomobility(experimental group1) compared to those with thoracic partial hypomobility(experimental group2) in flexion, extension, and right and left lateral flexion. The results of this study have three clinical implications, First, additional thoracic mobilization for chronic low back pain patients with thoracic hypomobility would be effective in pain reduction, static balanceimprovements, and proprioception recovery. Second, applying additional thoracic mobilization and thoracic partial hypomobility would be effective for increasing the thoraco-lumbar range of motion. Third, chronic low back pain with thoracic partial hypomobiliy would be effective for the recovery lumbar of the lumbar lordotic curve.
