This is a double-blind, randomized, placebo-controlled parallel group pretest-posttest designed study in order to evaluate the effect of aroma oil inhalation therapy on appetite among middle-aged overweight women and to verify that aroma oil inhalation therapy can be used as a nursing intervention to improve health and quality of life among middle-aged overweight women. To evaluate the effect of the therapy, sixty women aged 40 years or more with regular menstruations and a BMI above 23.0 kg/m^3 were randomized evenly into the experimental group and control group, and were administered the intervention from 21 October 2014 to 24 October 2014. Six subjects were excluded during the intervention, and fifty-four women were analyzed. The experimental group (n=27) was given a blended oil of grapefruit, geranium, and neroli, diluted with almond oil at a final concentration of 2%, while the control group (n=27) was given 100% almond oil. Both groups were instructed to apply 1ml of oil on a fragrance pad and inhale it for 5 minutes, twice a day for three days. Appetite as determined by four visual analog scales (VAS) and four biochemical markers; sleep quality based on VSH (Verran Synder-Halpern) scale; and subjective stress indicators were measured before and after the treatment. The data collected were analyzed by SPSS Win 21.0 for description statistics, independent 2-sample t-test, Chi-square test, and Mann-Whitney U test. The results in this study are as follows: Hypothesis No. 1: The hypothesis that ‘the pretest-posttest mean change in subjective appetite between the groups will be different’ was partially supported as the experimental group showed significantly higher reduction than the control group in the three subjective appetite scales including ‘hunger’ (p=.008), ‘thought of foods’ (p= .001), and ‘desire to eat’ (p=.002). Hypothesis No. 2: The hypothesis that ‘the pretest-posttest mean change in physiological appetite between the groups will be different’ was partially supported as the experimental group showed significantly higher increase in serum level of GLP-1 (p=.009) compared with the control group. Hypothesis No. 3: The hypothesis that ‘the pretest-posttest mean change in subjective sleep quality between the experimental group and the control group will be different’ was partially supported as the experimental group showed significantly higher improvement in VSH score (p=.009) then the control group.
