Since the introduction of the long-term care insurance in 2008, Korea has witnessed drastic changes in social service delivery system. For the purpose of improving the quality of long-term care services, the Korean government adopted a market mechanism, the ‘provider competition and user’s selection,’ to the existing social service system. However, despite such policy changes, many studies indicate that variations in quality of care have still existed among facilities. Moreover, service quality complaints have grown among the users as well. Recently, the quality improvement of the LTCS (long-term care services) facilities has been highlighted as key politic issues. In this situation, the accurate service quality evaluation and the knowledge of the determinants of it in institutional settings are prerequisite to the enhancement of service quality. However, previous researches of the LTCS evaluation have demonstrated that there is a limit to accept service users’ preferences and to evaluate their perceptions of the quality of service. Therefore, in this research, for the purpose of better quality of care in the LTCS facilities, I would emphasize on the importance of perceived service quality evaluation and its determinants by older adults living in the LTCS facilities. For this purpose, this research designed two research questions. First, it evaluates the performance of perceived user’s service quality in the LTC residential facilities, utilizing the SERVPERF model. This instrument evaluates the following service quality domains: tangibility, reliability, responsiveness, assurance, and empathy. Second, it analyzes the factors influencing user’s perception of quality of service, using hierarchical linear models, and it clarifies the effects separately between the persons and organizations related characteristics on the quality of service provided to individual residents. Since the data has hierarchical structure of individual and organization levels, it is reasonable to assume that attributes of the individual resident may affect the quality of service they receive from the facility at the organizational level. Individual factors are classified into demographic characteristics and health characteristics. Likewise, organization factors are divided into the facility characteristics and organization culture. For this research, a survey was conducted to target 200 caregivers providing care to 240 residents in 45 LTC residential facilities located in Seoul, Korea, from May 1 to October 31 in 2014. The collected data was analyzed using two level Hierarchical linear model (HLM 7.0). The major findings and policy implications of this research are as follows. First, the level of perceived quality shows average of 3.89 out of 5-point scale, which falls short of the good quality level (4-point), although several sub service quality indicators showed scores higher than 4-point. The ranking of service quality indicators in its average score is: assurance> reliability=responsiveness> empathy> tangibility. In other words, the users gave higher score to assurance service quality but lower score to tangibility. This implies that older adults in the LTC residential facilities expected to have a warm and human relationship with their caregivers rather than to have mechanic and technical contact with them. In terms of the nature of the social care, it requires an intimate interaction between the service providers and the users, and moreover responds to individual residents' desires. Therefore, factors, such as reliability, responsiveness, and empathy are considered much more important in service quality than the factors related to tangibility. Second, at the resident level, four variables in the resident’s demographic characteristics were found to influence the perceived quality of residents: gender, the level of income, the carer’s existence, and the marriage status. Neither age nor the level of education was statistically significant. For the health characteristics, the subjective health variable is significantly related to perceived quality, but both the physical function and the cognitive function are not statistically significant. This implies that demographic characteristics factors compared to health-related factors influences more on the perceived service quality of the institutionalized older adults. Prior studies emphasized on the nursing and medical services to get better the health condition of the elderly living in the LTCS facilities. However, in case of measurement of the perceived quality of residents in the LTC facilities, demographic characteristics are more influential than health-related factors. It indicates that users in the LTCS facilities have similar physical and functional conditions, which means that the former has more influence on the perceived quality than the latter. The results of this study suggest that the future policies should enforce the caregivers' ability and their characters to facilitate delivering care management service, according to the residents’ individual characteristics. Care providers form relationship with residents, and they seek to understand and address their individual needs. Third, the results of a null model with random effects estimates 44.24% of the total variance in the quality was explained by organizational level variables, consisting of the facility characteristics and the organizational culture. It implies that there is a significant variability among facilities in the quality of service they provided to their residents. Also, it is important to note that policy makers consider organizational level variables as policy variables to the quality improvement of the LTCS. Fourth, at the organization level, the size of facility variable in the facility characteristics was found to be associated with the perceived quality of service. Meanwhile, the running period of the facility and the ownership of the facility variables are not statistically significant. In terms of organization culture variable, the dominant organization culture type represented hierarchical culture. When examining the effect of organization culture on perceived quality in the LTC facilities, the factors that enhance perceived quality were found to be higher relationship oriented group culture and innovation oriented development culture. Hierarchical culture and rational culture are not statistically related to service quality. These research findings imply that the size of facility is the important factors influencing service quality. Also, it should be recognized that organization culture can affect the perceived quality of residents. It verifies that older adults in the LTC facilities have positive relationship with the service quality when their caregivers perceived their organization as a group culture type and a development culture type. In order to improve perceived service quality of the facilities, the LTCS facilities build on Group and Development organizational culture of work environment for the caregivers. In conclusion, this research has implications that it conducted service quality evaluation targeting institutionalized older adults who are often excluded from the discussions for improving quality. In addition, it expanded the levels of analysis verifying determinants of perceived service quality with both individual level and organization level.

However, previous researches of the LTCS evaluation have demonstrated that there is a limit to accept service users’ preferences and to evaluate their perceptions of the quality of service. Therefore, in this research, for the purpose of better quality of care in the LTCS facilities, I would emphasize on the importance of perceived service quality evaluation and its determinants by older adults living in the LTCS facilities. For this purpose, this research designed two research questions. First, it evaluates the performance of perceived user’s service quality in the LTC residential facilities, utilizing the SERVPERF model. This instrument evaluates the following service quality domains: tangibility, reliability, responsiveness, assurance, and empathy. Second, it analyzes the factors influencing user’s perception of quality of service, using hierarchical linear models, and it clarifies the effects separately between the persons and organizations related characteristics on the quality of service provided to individual residents. Since the data has hierarchical structure of individual and organization levels, it is reasonable to assume that attributes of the individual resident may affect the quality of service they receive from the facility at the organizational level. Individual factors are classified into demographic characteristics and health characteristics. Likewise, organization factors are divided into the facility characteristics and organization culture. For this research, a survey was conducted to target 200 caregivers providing care to 240 residents in 45 LTC residential facilities located in Seoul, Korea, from May 1 to October 31 in 2014. The collected data was analyzed using two level Hierarchical linear model (HLM 7.0). The major findings and policy implications of this research are as follows. First, the level of perceived quality shows average of 3.89 out of 5-point scale, which falls short of the good quality level (4-point), although several sub service quality indicators showed scores higher than 4-point. The ranking of service quality indicators in its average score is: assurance> reliability=responsiveness> empathy> tangibility. In other words, the users gave higher score to assurance service quality but lower score to tangibility. This implies that older adults in the LTC residential facilities expected to have a warm and human relationship with their caregivers rather than to have mechanic and technical contact with them. In terms of the nature of the social care, it requires an intimate interaction between the service providers and the users, and moreover responds to individual residents' desires. Therefore, factors, such as reliability, responsiveness, and empathy are considered much more important in service quality than the factors related to tangibility. Second, at the resident level, four variables in the resident’s demographic characteristics were found to influence the perceived quality of residents: gender, the level of income, the carer’s existence, and the marriage status. Neither age nor the level of education was statistically significant. For the health characteristics, the subjective health variable is significantly related to perceived quality, but both the physical function and the cognitive function are not statistically significant. This implies that demographic characteristics factors compared to health-related factors influences more on the perceived service quality of the institutionalized older adults. Prior studies emphasized on the nursing and medical services to get better the health condition of the elderly living in the LTCS facilities. However, in case of measurement of the perceived quality of residents in the LTC facilities, demographic characteristics are more influential than health-related factors. It indicates that users in the LTCS facilities have similar physical and functional conditions, which means that the former has more influence on the perceived quality than the latter. The results of this study suggest that the future policies should enforce the caregivers' ability and their characters to facilitate delivering care management service, according to the residents’ individual characteristics. Care providers form relationship with residents, and they seek to understand and address their individual needs. Third, the results of a null model with random effects estimates 44.24% of the total variance in the quality was explained by organizational level variables, consisting of the facility characteristics and the organizational culture. It implies that there is a significant variability among facilities in the quality of service they provided to their residents. Also, it is important to note that policy makers consider organizational level variables as policy variables to the quality improvement of the LTCS. Fourth, at the organization level, the size of facility variable in the facility characteristics was found to be associated with the perceived quality of service. Meanwhile, the running period of the facility and the ownership of the facility variables are not statistically significant. In terms of organization culture variable, the dominant organization culture type represented hierarchical culture. When examining the effect of organization culture on perceived quality in the LTC facilities, the factors that enhance perceived quality were found to be higher relationship oriented group culture and innovation oriented development culture. Hierarchical culture and rational culture are not statistically related to service quality. These research findings imply that the size of facility is the important factors influencing service quality. Also, it should be recognized that organization culture can affect the perceived quality of residents. It verifies that older adults in the LTC facilities have positive relationship with the service quality when their caregivers perceived their organization as a group culture type and a development culture type. In order to improve perceived service quality of the facilities, the LTCS facilities build on Group and Development organizational culture of work environment for the caregivers. In conclusion, this research has implications that it conducted service quality evaluation targeting institutionalized older adults who are often excluded from the discussions for improving quality. In addition, it expanded the levels of analysis verifying determinants of perceived service quality with both individual level and organization level.
